An application for a CBRF license has been received by our Department requesting that this facility be licensed as a Class A facility with maximum resident bed capacity of 12 adults. This building is still under construction, but will be completed by June 1, 1990. A license is required according to State Statute 50.02(1) and a building inspection by your Department is required for a CBRF with 9 to 20 beds before a license can be issued. Please contact Roger Kotenberg (414/231 -4740) for an appointment to inspect the facility. Please inform me of the results of your evaluation as our issuing a license to Toward Tomorrow, Inc. is dependent upon your determination of compliance with the regulations of Chapter Ind. 61.
