RE: Change Order #1 for Public Works Contract No. 10 -20; for Four -Way Construction of WI for
Mini -Storm Sewer/ Storm Laterals ( +$5,989.77)
BACKGROUND
Change Order #1 for Contract 10 -20 is scheduled for consideration by the Common Council at the December
14, 2010 meeting. The contract was originally awarded to Four -Way Construction of WI in September of 2010.
This change order is for additional work required to complete the contract and finalizes the quantities.
ANALYSIS
Taking part in the Mini -Storm Sewer / Storm Lateral Program is voluntary on the part of the residents. After the
Contract had gone out for bids, one resident decided not to take part anymore. At that time, the next resident on
the waiting list was included in the Contract. Below is a summary of the changes that occurred based on this
change in location:
— Paving — Additional sidewalk and driveway removal and replacement. Reduction in amount of
street pavement removal and replacement.
— Storm Sewer — Additional 6" mini -storm sewer.

The entire contract is being paid for by the Storm Water Utility. There is adequate funding available in the 2010
Capital Improvements Budget.
RECOMMENDATIONS
recommend approval of the Change Order #1 to Contact No. 10 -20 in the amount of $5,989.77 for Four -Way
Construction of WI.
