1. GENERAL INSTRUCTIONS
PURPOSE OF DOCUMENT
The purpose of these documents is to provide a comprehensive, uniform set of documents that clearly
defines the responsibilities of each participant. It is the desire of US Bank and Commercial Consultants
Group to provide all participants with a document capable of delivering the required services and
products in a fair, consistent, and ethical manner.
ACKNOWLEDGEMENT OF BID PACKAGE
US Bank has provided these documents with an understanding that each bidder shall familiarize
themselves with the totality of said documents prior to submittal of their binding quotation. The
contractor's signature under " ACKNOWLEDGEMENT OF BID PACKAGE" shall serve as
acknowledgement that the bidder has read, understands, and agrees to disseminate information as
needed to their office staff, field superintendents, jobsite foreman, and other employees as needed. It
shall remain the responsibility of the contractor to inform all personnel of these documents and the
intent of the owner as it relates to the entirety of these Specifications.
GENERAL DEPICTION OF AREAS /BUILDINGS
The areas and /or buildings covered under these specifications are shown below as a quick reference
(not to scale) of this project. Actual detailed maps, measurements, etc. shall be provided later in this
document. The areas of potential replacement will be (as marked) Sections I a, I b, I c, and 9. The map
as shown herein is for general reference only.
GENERAL SCOPE -OF -WORK
1.1.1 The Scope -Of -Work shall include all
procedures required to comply with US
Bank specifications, bidding documents,
and good roofing practices as defined by
the National Roofing Contractors
Association (NRCA). Bidders shall refer
to Articles contained herein for detailed
description of the scope of work for this
project.
1.1.2 All Bidders are required to familiarize
themselves with the scope of work,

1.1.3 The work listed herein shall constitute the cost of all labor and selected materials, as
determined on the Proposal Fom,, including but not limited to, fasteners, metal work, and
other materials deemed to be a part of the overall system as specified in the accompanying
drawings, documents, and specifications.
1.1.4 Bidders are to determine their capability to install the specified roofing system(s) and must
meet all the specified Manufacturers qualifications. Proposals or bids to install roof systems
other than what is specified shall be considered only under the guidelines set forth in these
documents.
CHANGES, ADDITIONS, & DELETIONS
These documents are the standard for the project indicated herein, including the bidding process as well
as the actual scope -of -work. Any changes, additions, and /or deletions of this document, regardless of
cost implication, shall be acceptable only through the use of a written (I) Addendum or (2) Change
Order approved and signed by US Bank. In all respects, changes must be confirmed in writing and a
valid signature by the owner or consultant must accompany any documentation. Verbal changes to
these documents shall not be acceptable.
DEFINITION OF TERMS
1.1.5 Whenever the term "US Bank" or "Client" is used herein, it shall be indicative of an
abbreviation of the proper name of US Bank, which shall be considered the Owner. The
Owner is the person or entity identified as such in these documents and shall be referred to
throughout these documents as if singular in number. The term "US Bank" shall mean the
Owner or the Owner's authorized representative.
1.1.6 The term "Commercial Consultants Group" as used herein shall be indicative of the
abbreviated term for Commercial Consultants Group, LLC, a Wisconsin limited liability
corporation, and the recognized Consultant for the project(s) as defined in these documents.
Commercial Consultants Group is the person or entity identified as such in these documents
and shall be referred to throughout these documents as if singular in number. The term
"Commercial Consultants Group" shall mean the Consultant or the Consultant's authorized
representative.
1.1.7 The term "Bidder" shall be indicative of the person, firm, or corporation in possession of
these documents for the express purpose of submission of a proposal for work as indicated
in these documents. The Bidder is the person or entity identified as such in these documents
and shall be referred to throughout these documents as if singular in number. The term
"Bidder" shall mean the Bidder or the Bidder's authorized representative.
1.1.8 The term "Contractor" shall be indicative of the person, firm, or corporation under contract
with the Owner to supply labor, equipment, and materials specified herein. The Contractor is
the person or entity identified as such in these documents and shall be referred to
throughout these documents as if singular in number. The term "Contractor" shall mean the
Contractor or the Contractor's authorized representative.
1.1,9 The term "Subcontractor" shall be indicative of the person, firm, or corporation under
contract with the Contractor to supply labor, equipment, and materials specified herein. The
Subcontractor is the person or entity identified as such in these documents and shall be
referred to throughout these documents as if singular in number. The term "Subcontractor"
shall mean the Subcontractor or the Subcontractors authorized representative,

INTERPRETATION OF DRAWINGS AND SPECIFICATIONS
1.1.10 Should a Bidder find discrepancies or ambiguities in, or omissions from the Drawings and
Specifications, or should the Bidder be in doubt as to their meaning, the Bidder shall at once
notify Commercial Consultants Group for an interpretation. Where applicable, an addendum
will be forwarded to all Bidders and each Bidder shall acknowledge the receipt of each
addendum on their bid in the spaces provided.
1.1.11 Any instructions or decisions, unless confirmed by numbered addendum, will not be
considered valid, legal, or binding. All discrepancies or requests for clarification must be
submitted to Commercial Consultants Group not less than five (5) working days prior to the
scheduled opening of bids. No additional changes or addenda will be accepted after such
time.
DELIVERY OF DOCUMENTATION
The timely distribution of these, and all related documents, is essential to the overall faimess of the
Bidders' submittals. All Bidders shall be "Pre- Registered" and a complete set of Specifications for each
pre - registered bidder will be available at the Pre - Bidding Conference, These specifications shall include
all General Bidding Requirements, Scope -of -Work Forms, and Bid Proposal Forms, Related drawings
shall also be provided where applicable.
OWNERSHIP AND DISTRIBUTION OF DOCUMENTS
The Drawings, Specifications, and all other documents contained herein are intended as instruments by
which the Bidder can execute the work associated herein.
1.1.12 Neither the Bidder, Contractor, Subcontractor, material or equipment suppliers shall own or
claim a copyright to these drawings, specifications, or other documents contained herein.
1,1.13 Unless otherwise indicated in writing, Commercial Consultants Group shall be deemed the
author and exclusive owner of all documents contained herein and will retain all common
law, statutory, and other reserved rights in addition to the copyright where applicable.
1.1.14 Any duplication or distribution of these documents for any purpose other than those
specifically required as a part of this project shall be prohibited. Any person(s) causing the
copy or distribution of these documents without the express written consent of Commercial
Consultants Group shall be subject to applicable legal actions,
1,1.15 Further, these documents are not to be used by the Bidder, Contractor, Subcontractor, and
material or equipment supplier on other projects or for addenda to this project outside the
scope of the work as specified.
GOVERNING INSTITUTIONS
These documents are intended to reflect a standard, set by US Bank and Commercial Consultants
Group, for quality of materials and the proper application of those materials. The methods used to
define those standards and to provide consistent guidelines by which those standards can be judged
shall be based on, but not limited to, the following goveming institutions.

INDEMNITY
The Bidder, by submission of these documents, assumes full responsibility and agrees to indemnify and
hold harmless US Bank and Commercial Consultants Group against any loss, damage, expense, injury, or
claim of damage to person or property caused by the Bidder, his or her employees, or his or her
Subcontractors. This includes any loss or claim of damage or injury with respect to employees, agents,
or property of the Contractor or Subcontractor.

2. BID REQUIREMENTS
SITE CONDITIONS
2.1.1 Each Bidder shall, prior to submission of quotation, become familiar with the project site and
make allowance for site conditions including access, available storage, existing structures, or
work in progress. Further, the Bidder will be responsible for the verification of all
underground utilities including, but not limited to, gas lines, telephone lines, sewers, water
mains, etc. The submission of a bid will be construed as evidence that such examination has
been made and no subsequent allowance will be made in this connection,
2.1.2 The Bidder shall be responsible for all temporary excavations, scaffolding, guardrails, vehicle
and pedestrian passageways, and other temporary work that extends beyond the boundaries
of the Owner's property. The Bidder shall make all necessary arrangements and agreements
with the owners of adjoining property and with public authorities. Cost of such arrangements,
agreements, and work shall be included in the Bidders' base proposals.
SITE ACCESS
No access will be granted to the building, roofing area, or any other area(s) pertaining to this project
without the express written consent of Commercial Consultants Group, as the designated
representative for US Bank. Any Bidder wishing to investigate this project, review on -site details, or
otherwise gain access shall be required to request authorization to do so not less than twenty -four (24)
hours prior to the desired right of entry. Such authorization may be granted by contacting Commercial
Consultants Group at the numbers listed within this document.
CORE ANALYSIS
Each Bidder shall, prior to submission of quotation, become familiar with the construction makeup of
each roofing section and make allowance for all such conditions. Where results of core samples are
provided herein, inclusive of Commercial Consultants Group's Roof Inventory Sheets, it shall be for
general informational purposes only. The Bidder, as in all other Articles of this document, shall remain
responsible for collection of all site data necessary to provide a substantial quotation. The submission of
a bid will be construed as evidence that such examination has been made and no subsequent allowance
will be made in this connection.
MEASUREMENTS AND QUANTITIES
Any drawings of respective buildings and roof areas included herein, are intended to show the
approximate shape, size, and location of the areas referred to in these documents. Each qualified Bidder
shall be solely responsible for the accuracy of all measurements and for assessment of the material
quantities (pursuant to published 2 -Part Bidding Guidelines) required to satisfy these specifications.
WORKMANSHIP & LABOR
2.1.3 All Workmen shall be thoroughly experienced in the particular class of work in which they
are employed. US Bank and /or Commercial Consultants Group reserve the right to dismiss
any Workmen who do not have available the skill(s) necessary to properly complete any job
they are so assigned.

2.1.4 All Contractors and Subcontractors employed within the scope of work designated herein
shall be required to conform to the labor laws of the State of Wisconsin as being performed
and the various acts, mandatory and supplementary thereto and all other laws, ordinances
and legal requirements applicable thereto. All labor shall be performed in the best and most
workmanlike manner by mechanics skilled in their respective trades.
2.1.5 It shall be the duty of every Contractor engaged in this work to enforce among all Workmen
directly or indirectly employed by that firm rules which the Owner may lay down for conduct
of Workmen on the premises.
2.1.6 All subcontractors shall be required to submit Insurance Certificates directly to Commercial
Consultants Group (pursuant to the Insurance guidelines as specified in these documents)
prior to on site work.
MATERIAL WARRANTIES & WARRANTY CHARGES
2.1.7 All work depicted herein shall be considered to be completed under the Standard 2 -Year or
5 -Year Contractor Warranty — Refer to Bid Form for duration of warranty. A Standard 10-
Year Material "Only Warranty" from the manufacturer, unless specifically indicated otherwise
shall be included at no cost on all projects.
2,1.8 All manufacturer's costs associated with a No- Dollar -Limit Warranty, when specifically
requested by the Owner, shall not be included in the Bid Proposal Form. All such costs shall
be provided by the manufacturer directly to US Bank, upon their request.
PERMITS, FEES, AND TAXES
2.1.9 The Bidder shall execute all work in strict compliance with federal, state, and local laws,
codes and regulations. This directive shall include the procurement of all construction permits
and /or licenses as required by law. The Bidder shall promptly notify Commercial Consultants
Group if these Specifications are in discordance with any applicable law, ordinance, or
regulation. Costs associated with such permits and /or licenses shall be included in the cost of
proposed work.
2.1.10 The Bidder shall be responsible for the payment of federal, state, and local sales and user
taxes, where applicable. All social security taxes, unemployment taxes, withholding taxes, and
any other federal state, and local taxes applicable to the performance of these Specifications
shall be the responsibility of the Bidder. Costs associated with such taxes shall be included in
the cost of proposed work.
INSURANCE REQUIREMENTS
2.1.1 I The Bidder shall maintain in force such insurance with the coverage and limits set forth below
to protect against claims arising from or related to this Agreement including, but not limited
to, use of owned or non -owned vehicles. All such insurance shall name US Bank and
Commercial Consultants Group as ADDITIONALLY INSURED and will provide a copy of
said policy with amendments prior to the start of any work

2.1.12 The Bidder shall procure and maintain during the life of this contract bodily injury and
property damage liability insurance under a comprehensive general form and a
comprehensive automobile bodily injury and property damage liability contract. Amounts of
such bodily injury and property damage liability shall not be less than (a) bodily injury liability
in a minimum amount of $1,000,000.00 for injuries to any one (1) person and in a minimum
amount of $2,000,000.00 on account of any one (1) accident; (b) property damage insurance
in a minimum amount of $1,000,000.00 for damage on account of all accidents other than
automobile property damage accidents; and (c) property damage insurance in a minimum
amount of $100,000.00 on account of any automobile accident, The Bidder's comprehensive
liability insurance coverage is to include completed operation insurance for a period of one
(1) year after completion of this contract.
2.1.13 The Bidder shall maintain during the life of this contract statutory worker's compensation and
employer's liability insurance for all his or her employees engaged in work on the jobsite.
2.1.14 Before work may commence the Bidder shall fumish US Bank a properly signed and
notarized statement on his or her letterhead stating that he or she has complied with all
provisions of this specification relating to insurance coverage and that none of the insurance
coverage specified will be canceled or materially altered except after thirty (30) days written
notice has been received by US Bank.
2.1.15 The Bidder, by executing a statement of insurance compliance, acknowledges to US Bank
that he or she has complied completely with coverage requirements of the contract
documents. The Bidder further acknowledges that US Bank has no responsibility to provide
coverage of any nature.
2.1.16 US Bank shall effect and maintain fire and extended coverage insurance on a completed value
form or monthly reporting form of policy upon entire structure on which work of the
contract is to be done. US Bank will not carry insurance for glass. The Bidder is responsible
for glass and may, at his or her option and cost, obtain glass coverage. Amount of insurance
shall be one hundred per cent (100 %) of insurable value of project.
2.1.17 The Bidder, by executing a statement of insurance compliance, acknowledges to US Bank
that he or she has complied completely with the coverage requirements of the contract
documents. The Bidder further acknowledges that US Bank has no responsibility to provide
coverage of any nature except as specified. Failure by the Bidder to request, in writing, a copy
of US Bank policy within thirty (30) days of date of contract awards does thereby waive US
Bank responsibility to provide a copy said policy.
SUBCONTRACT BIDDERS
2.1.18 This Contract may not be assigned or encumbered nor may the Bidder subcontract the work
in whole or in part unless written authorization is first obtained from US Bank. When
necessary, the Bidder shall be prepared to submit to US Bank and Commercial Consultants
Group, in writing, the names of all proposed Subcontractors. The competence and ability of
the Subcontractor(s) may be taken into recognition prior to the award of this Contract. This
and all other writings notwithstanding, there shall be no contractual relationship between US
Bank and any Subcontractor.
2.1.19 There shall be no discrimination against any employee or applicant for employment because
of age, race, handicap, creed, color, sex, ancestry, marital status, military status, or because of
their status as a Vietnam Era Veteran. Qualified persons with physical and /or mental
disabilities shall be accommodated unless accommodation would cause undue hardship.
Persons with physical and /or mental disabilities that would place in danger their health or
safety, or the health or safety of others, while performing an essential function of their job, as
certified by a medical practitioner, shall be exempt,

2.1.20 US Bank mandates the implementation of appropriate actions to eliminate, any discrimination
in employment, promotion, and training of employees, where applicable by law.
BIDDER DISQUALIFICATION - BID PROPOSAL FORM
2,1.21 Incorrect use of the "Proposal" as provided by US Bank shall be defined as the addition,
deletion, omission, or alteration of any part shown on said Bid Proposal Form. The Proposal
is designed to afford all Bidders an equal format from which to present their bidding
amounts. Any changes in said format shall be accepted only when directed to do so by US
Bank and /or Commercial Consultants Group in writing prior to the submittal date.
2.1.22 Contact between US Bank and the Bidder after the bid opening and prior to the award of
said Contract with the express intent to influence or alter the decision of US Bank shall be
cause for disqualification,
2.1.23 Additions, deletions, omissions, or alteration of any part of the Bid Proposal Form shall cause
the owner to summarily reject the submission in ft's totality.
BIDDER DISQUALIFICATION - FAILURE TO COMPLY
US Bank or Commercial Consultants Group may consider any one (or more) of the following to be
sufficient cause for disqualification of a Bidder and the rejection of his or her bid:
2.1.24 The failure to make payment (when requesting printed documentation) to Commercial
Consultants Group for specifications, drawings, and related forms, prior to the opening of
sealed bids.
2.1.25 Evidence of collusion between Bidders. Participants in such collusion will receive no
recognition as Bidders for future work and legal action shall be taken where appropriate.
2.1.26 Where the Bidder is shown to lack certification and /or other qualifications as required by
these documents and /or based on their lack of capability to install the specified roofing
system.
2.1.27 Lack of proficiency as shown by past work or incomplete work under other contracts which,
in the judgment of US Bank, might hinder or prevent the prompt completion of additional
work if so awarded or any involvement in any legal actions that relate to past or present
performance. This includes, but is not limited to, lawsuits, court appointed actions, and /or
ongoing litigation and shall be determined by US Bank and /or Commercial Consultants
Group,
2.1.28 US Bank also reserves the right to reject any Subcontractor bids if the Subcontractor fails to
satisfy US Bank of his or her ability to perform the Contract in a manner desired by US Bank
and as specified herein.
ADDENDA
All addenda shall be provided not less than three (3) days prior to submission date verbally, followed by
fax, email, and /or US mail. Any information needed to review and /or formulate the needed
information for such addenda, where applicable, must be provided to Commercial Consultants Group
not less than five (5) days prior to the required submission date.

SUBMISSION OF BIDS
The following instructions apply to the submission of all written proposals for the work contained within
these documents. Bids for the work described herein shall be received in the offices of US Bank as
indicated on the face of the attached proposal form. US Bank shall receive all proposals until the hour
and date as set forth in these documents. All proposals received after such designated time shall be
retumed to the Bidder unopened.
2.1.29 Facsimile Proposals: Any facsimile of the original bidding proposal forms shall not be
acceptable to US Bank.
2.1.30 Email Proposals: Any email of the original bidding proposal forms shall not be acceptable to
US Bank.
2.1.31 Preparation of Proposal: The Bidder shall submit his or her proposal on the forms furnished
with these documents. All text and figures shall be typewritten or legibly written in
permanent ink. The Bidders proposal must be signed in ink by an officer of the corporation
or by an authorized agent of the Bidder.
2.1.32 Use of Envelopes: All envelopes containing bid documents shall be clearly marked as follows:
2.1.32.1 Envelope I shall be clearly marked "ROOF PROJECT PROPOSAL" with the
name of the Bidder clearly indicated on the outside of said envelope. These
envelopes shall be delivered at the same time and place as listed above.
2.1.32.2 Envelope 2 shall be clearly marked "BID BONDS" covering all projects
requiring a Bid Bond and placed in a separate envelope. These envelopes shall
be delivered at the same time and place as listed above.
2.1.33 It is the Bidders' sole responsibility to deliver their bids at the proper time, date, and location.
Commercial Consultants Group shall not accept, deliver, or otherwise participate in the
handling of unopened proposals.
2.1.34 Any bids and /or changes that are submitted after the time as indicated on the proposal form
shall be considered invalid and thus rejected and the Bidder disqualified. US Bank will not
open the sealed bids of any Bidder that has been disqualified. US Bank reserves the right to
reject any or all bids and /or to accept any bid that, in his or her opinion, is in the best interest
of US Bank.
2.1.35 Written Bid Modifications: Bidders desiring to make changes to their bids after they have
been submitted may do so in writing provided such modifications are received prior to the
scheduled time for bid opening. The modification must clearly state that it is a modification to •
a previously submitted bid and must be sign @d by persons legally authorized to bind the
Bidder into a contract with US Bank.
BID OPENING AND INTENT TO AWARD CONTRACT
2.1.36 At the time of the opening of bids, each Bidder will be presumed to have inspected the site,
to have read, and to be thoroughly familiar with the plans and specifications including all
addenda. The failure or omission by any Bidder to examine any form, instrument, or
document shall in no way relieve Bidders from any obligation in respect to their bids.

2.1.37 It is US Bank intent to award a Contract to the Bidder submitting the lowest acceptable bid.
However, US Bank reserves the right to reject any or all bids, to accept any bid submitted, to
waive any formalities and to negotiate with the low Bidder or Bidders on any changes that
US Bank considers necessary for his or her own interests.
2.1.38 The Bidder's attention is directed to the fact that all applicable state laws, municipal
ordinances, and the rules and regulations of all authorities having jurisdiction over
construction of the project shall apply to the contract form the same as though herein
written out in full,
POST BIDDING CONFERENCE
All designated individuals shall, when so requested, be required to attend a post bidding conference
when an apparent successful roofing contractor is determined. Said conference shall be attended by the
apparent successful roofing Contractor and a representative of each subcontracting firm (i.e., sheet
metal contractors, demolition contractors, etc.), Further, all Subcontractors shall be required to
acknowledge the timetable of this project and their ability to provide their services and /or materials in a
timely fashion.
SCHEDULE OF EVENTS
The Bidder shall complete all Work within the time and schedule as provided by US Bank. Submission
of a signed Bid Proposal Form constitutes acknowledgement that Bidder is prepared to start within
thirty (30) days of the issuance of a legal contract (purchase order).

3. SITE ANALYSIS
PRECONSTRUCTION CONFERENCE
A Preconstruction Conference shall be conducted not less than seven (7) working days prior to the
anticipated project start. The agenda will include, but is not limited to, the following:
3.1.1 The Preconstruction Conference shall be attended by a Representative of US Bank, the
Contractor, the Contractor's designated Foreman, and the Commercial Consultants Group
Representative.
3.1.2 The Contractor shall furnish US Bank and Commercial Consultants Group with a completed
"CONSTRUCTION CERTIFICATE" as provided as a supplement article in these
specifications.
3.1.3 A review of all project documents shall be conducted including Scope of Work, On -Site
Safety, Behavior Requirements, Construction Schedule, etc.
3.1,4 The Commercial Consultants Group Representative and the Contractor shall make a
complete walkover (where applicable) and inspection of the buildings and related grounds.
Any preexisting deficiencies shall be recorded in order to indemnify the Contractor and his
or her personnel.
3.1.5 A written report shall be submitted to US Bank with verification of this meeting with a
review of the topics addressed during said meeting.
3.1.6 The plans, drawings, and specifications furnished as a part of these documents shall be
available on the jobsite during each workday. Said documents shall be well cared for and
available on the job -site by the foreman.
ACCESS TO SITE
3.1 .7 The Contractor shall permit only authorized personnel into the work areas including, but not
limited to, actual project construction areas. The Foreman shall accept responsibility for
identifying all persons seeking access to work areas and secure all such areas from non -
authorized personnel. As directed, the Foreman shall allow access to the work areas only to
persons approved by US Bank.
3.1.8 Special requirements for identification tags, keys, etc. may also be required by virtue of US
Bank security requirements. All such requirements shall be, where applicable, delivered in
writing to the Contractor prior to the start date. Please refer to "Owner Secur
Requirements" for additional information regarding US Bank policy.
3.1.9 Notification of this limited access policy should be placed in a visible location so that
enforcement of this policy is uniform,
OWNER SECURITY REQUIREMENTS
US Bank has developed specific security programs germane to his or her type of business and location
needs. The Contractor shall abide by all security requirements listed in these documents and /or as
provided in writing by US Bank at the Pre - bidding and /or Preconstruction Conferences.

UTILITY SERVICES
The OWNER WILL BE RESPONSIBLE for the disconnection and reconnection of all gas pipelines,
electrical lines, air handling gases and /or liquids, and other utilities as necessary and to provide it in a
timely fashion so that Contractor work is not impeded. It shall be the CONTRACTORS
RESPONSIBILITY to notify the Owner with a sufficient amount of lead time so that the appropriate
"trades" can be available for such work.
ON -SITE COMMUNICATIONS
The Contractor shall insure that on -site communications be implemented to the fullest extent possible.
The Foreman on site must have the ability to communicate in a clear and accurate manner consistent
with the primary language used by the Owner as well as Commercial Consultants Group and support
staff.
ON-SITE SAFETY
The following on -site safety requirements are provided by US Bank and Commercial Consultants Group
as a fundamental part of these documents. Under no circumstances should any person jeopardize his or
her safety performing any act specified herein. It is the desire of both US Bank and Commercial
Consultants Group to ensure that safety is paramount in the implementation of this project. Should any
Worker deem any act as dangerous, or unsafe, he or she shall report said concem directly to the
Consultant or Owner for immediate review,
The Contractor shall be responsible for initiating, maintaining, and supervising all safety precautions and
programs in connection with the performance of the work described herein. The Contractor shall
comply with all federal, state, and local safety regulations including, but not limited to, the Occupational
Safety and Health Act of 1970, as amended and in force during this project. Further, the Contractor shall
comply with all requirements as directed by the Owner's safety and security regulations including the
following regulations:
3.1.10 Clothing: Proper cotton apparel shall be wom at all times. Long sleeved shirt, properly fitted
pants without cuffs, and approved shoes. Hard hats shall be wom where there are overhead
hazards,
3.1.1 1 Fall Protection: The Contractor shall be responsible for following all current OSHA
Regulations as it relates to Fall Protection. Fall Protection including, but not limited to, slide
guards, guard rails, safety nets, waming lines, and personal fall - arrest systems must be used to
protect employees who are working on roofs where applicable.
3.1.12 Ladder and Access Areas: Ladders must be placed on firm ground, reasonably level, and clear
of wires and overhead obstruction. All ladders shall be anchored at the roofline to a
permanent attachment or an object with a minimum weight of 250 pounds. Side rails of the
same shall extend a minimum of three (3) feet above the rooftop. All ladders must be kept
away from power lines and used exclusively for the transportation of persons. All ladders
shall be properly secured at the end of each workday.
3.1.13 Ladder Safety: All ladders must have nonslip feet at the base and shall conform to the
industry standard of a 4:1 set -up ratio.
3.1,14 Barriers: The Contractor shall take all reasonable precautions for the safety of his or her
employees and all other persons who may be on the project site and shall erect and maintain
such barriers, signs, and /or watchmen as will protect and wam pedestrians and vehicles and
prevent access of unauthorized persons to the project site.

3.1.15 Perimeter Flags: Safety flags /perimeter lines shall be placed, where applicable, in any area
where the distance from roof to ground or roof level to roof level exceeds six (6) feet.
These flags are to be placed in all areas where work is in progress and must be placed,
anchored, and monitored in strict accordance with OSHA requirements.
3.1.16 Propane Tanks: All propane tanks and gasoline cans, where stipulated by law, shall be taken
off the roof at the end of each working day and stored away from the building fresh air
intakes and any openings into the buildings. All such materials shall be stored in such a
manner as to prevent unauthorized access.
3.1.17 Medical Equipment: Proper first aid kits will be located in the set -up area of each jobsite.
These basic first aid kits shall contain the sufficient materials for the treatment of bums,
fractures, etc. The Foreman of each project should have training in the application of first aid
and knowledge of the nearest medical facilities and phone numbers of emergency personnel.
3.1.18 Fire extinguishers: Properly maintained fire extinguishers shall be present on the jobsite at all
times. All extinguishers should be checked no less than once each month and serviced by
qualified personnel on a yearly basis. There shall be one (1) fire extinguisher present for each
torching unit being used on the project. Said extinguisher will be located within 100 feet of
the torch being used. There will be at least one (1) serviceable Type "C" minimum 20 -pound
size extinguisher located within 100 feet of each kettle location.
ON -SITE BEHAVIOR
All personnel present on site, or in the pursuit of said project (in transit to and from the Contractors
place of business and the Owner's facilities) shall conform to, but not be limited to, the following
behavioral procedures at all times:
3.1.19 Any personnel using abusive language or presenting an offensive appearance will not be
allowed to remain on the property after such discovery.
3.1.20 Absolutely no alcoholic beverages or drugs or any nature (except certain approved
prescription medications) will be allowed on US Bank property and shall not be used by any
worker for a minimum of eight (8) hours prior to the scheduled start time for any project.
3.1.21 The use of tobacco products shall be prohibited at all times on company premises when said
Owner has established a policy of such restriction. Further, the possession of firearms and /or
ammunition shall be strictly prohibited on company premises. US Bank property shall
include all property owned by, rented by, or under the control of US Bank,
3.1.22 All sound devices including, but not limited to, radios, "boom- boxes," etc. will not be allowed
on the jobsite.
CONCURRENT OPERATIONS
3.1.23 Because other activities of US Bank will be proceeding at the same time as the work covered
by these documents, the Contractor shall collaborate with the Commercial Consultants
Group Representative to ensure that all contract work progresses in a manner that does not
conflict with other activities.
3.1.24 The Contractor shall, in harmony with US Bank, designate a "Contractor Parking Area" to be
used by all Contractor and employee vehicles.
3.1.25 The Contractor shall confine all equipment and storage of materials to areas that will allow
US Bank to maintain normal traffic and shall not unreasonably encumber the premises.

CONTINUITY OF CONSTRUCTION - WATERPROOFING
Contracts are awarded based on the continuity of all phases of this construction. The primary roofing
Contractor, unless otherwise stipulated in writing, is solely responsible for the succession of work from
job start to job completion.
3.1.26 Non - weather - related work stoppages in excess of five (5) working days should be deemed
an interruption of the construction sequence. It is understood that all contracts under these
specifications specifically reject any interruption of construction as defined herein. Penalties
may be imposed by US Bank if the originating Contractor, in the sole judgment of US Bank,
could have avoided such interruptions.
3.1.27 When excessive stoppage of work occurs, additional penalties may be imposed where
applicable and may include payment for an independently performed infrared scan and the
correction of any deficiencies that may be detected because of said scan. Penalties may also
include the cost involved in the subcontracting of any or all remaining work.
3.1.28 The primary Contractor shall ensure that all completed waterproofing materials be installed
in such a manner as to create a watertight condition of the jobsite for a period consistent
with the approved Continuity of Construction —Sheet Metal Work Termination (see below)
of all flashings, edge work, and other projections shall be sealed in a watertight manner,
consistent with any delays as approved herein.
CONTINUITY OF CONSTRUCTION -SHEET METAL WORK
Installation of sheet metal work, as defined in these documents, shall be provided an approved
exception to the "Continuity of Construction — Waterproofing" clause as defined herein specifically in
the following manner.
3.1.29 The Contractor or Subcontractor responsible for the installation of all sheet metal shall be
afforded a period of five (5) working days immediately following the completion of the
installation of the waterproofing membrane to gain access to the site.
3.1.30 The aforementioned five (5) day period shall be utilized to properly measure, design, and /or
determine the method(s) of installation for all sheet metal as required in these specifications.
3.1.31 The Contractor or Subcontractor shall then be allowed ten (10) working days to complete
any fabrication work necessary for the proper installation of all sheet metal work as required
in these specifications.
3.1.32 The installation of all sheet metal, therefore, shall commence no later than fifteen (15)
working days from the completion of the installation of the waterproofing membrane, as
determined by Commercial Consultants Group.
3.1.33 The Contractor or Subcontractor shall provide an adequate number of skilled workers to
complete the installation of all sheet metal in a timely and professional manner,
3.1.34 When excessive stoppage of work occurs, additional penalties may be imposed where
applicable and may include payment for an independently performed infrared scan and the
correction of any deficiencies that may be detected because of said scan. Penalties may also
include the cost involved in the subcontracting of any or all remaining work.

TEMPORARY UTILITIES
3.1.35 US Bank shall, where readily available, provide existing utilities for the use of small tools only.
The Contractor will fumish all connections to the electrical system.
3.1.36 The Contractor will provide any portable electrical generating equipment for emergencies,
temporary lighting, or other related needs. All temporary services and /or facilities provided
for said project shall be immediately removed at the completion of said work.
3.1.37 The Contractor shall provide drinking water and sanitary facilities for the Workmen on this
project. US Bank will not be accountable for any portable sanitary facilities located on the
jobsite.
3.1.38 Contractor shall be prepared to adhere to and comply with owner equipment "lock -out"
procedures during the course of work.
MUNICIPAL ORDINANCES
The Contractor shall be responsible for providing personnel who are fully informed of, and prepared to
comply with, all local ordinances.
ROOF LOADING AND SAFETY
3.1.39 When more than one (1) day's supply of materials are planned to be stored on the roof, a
plan delineating the location and weight shall be submitted to Commercial Consultants
Group for review.
3.1.40 US Bank and Commercial Consultants Group will review and approve or reject the request.
A request for an engineering evaluation may be made to grant approval.
3.1.41 When a concern regarding the superimposed weight of materials on the roof arises, the
Contractor shall fumish, at his or her cost, a structural or civil engineer's calculations and
report, sealed by a registered professional engineer in the state where the project is located
no later than the published and scheduled Pre -job Conference.
MATERIAL HANDLING AND STORAGE
3.1.42 All materials must be delivered in original protective containers clearly labeled with all
pertinent information. Materials delivered in hot bulk equipment shall be accompanied by a
certification as to quality and melt point, where applicable.
3.1.43 Acceptance of all materials on site shall be the sole responsibility of the Contractor. At no
time shall US Bank be responsible for, or involved in, the acceptance, loading, or unloading of
any roof related materials. This includes materials provided under Direct Purchasing as well as
Contractor materials.
3.1.44 All rolled roofing materials shall be stored in an upright position and on single pallets. The
Contractor shall remove any damaged rolls prior to storage. The Contractor must ensure
that all materials are properly protected from the elements with suitable covers properly
secured.
3.1.45 All insulation boards shall be stored on pallets and protected at all times from the elements
with suitable covers properly secured.

3.1.46 The Contractor assumes full responsibility for the safekeeping of all materials delivered to the
jobsite. All materials should be stored with as much precaution as possible against theft,
vandalism, or weather - related damage. All products should be secured with positive
anchorage devices designed and sized to withstand stresses, vibration, and high winds. In the
event of damage or destruction from any cause, the Contractor shall promptly replace said
materials at his or her own expense.
CONTRACTOR EQUIPMENT
3.1,47 All equipment to be used on the roof should be of best quality, in good condition, and of
proper quality to complete the work listed herein. All equipment must be reviewed and
approved prior to project start date.
3.1.48 The Contractor shall maintain a working and calibrated thermometer on all kettles, tankers,
and tuggers. Further, all kettles and tankers shall be equipped with automatic temperature
controls,
3.1.49 Motorized equipment and small, heavy equipment items must be evaluated considering the
roof deck, spans, and spacing of the underlying structural members.
3.1.50 When a concern regarding the superimposed weight the equipment imparts to the structure
arises, a request for further investigation will be made by Commercial Consultants Group.
3.1.51 The Contractor shall fumish a structural or civil engineer's calculations and report, at his or
her cost, sealed by registered professional engineer in the state where the project is located
when asked to provide assurance of the safety of the use of certain roofing equipment.
ENVIRONMENTAL REQUIREMENTS
3.1.52 Care shall be taken to protect all HVAC or other mechanical equipment as well as all
skylights and other rooftop accessories during the roof removal and installation process from
all dirt, debris, adhesives, asphalts, etc,
3.1.53 Protective measures shall be provided to prevent the migration of dirt, dust, or debris
through air intake ducts located on or adjacent to the roof as well as in the exterior walls,
where applicable.
3.1.54 Protective measures shall be taken to prevent any dirt or debris from entering the roof
drains. All such plugs, where in use, shall be removed at the conclusion of each workday.
3.1.55 All materials sensitive to cold temperatures (containers or materials marked with "Keep from
Freezing ") shall not be installed if ambient temperatures are predicted to fall below 40° F.
3.1.56 Only as much roofing shall be removed in one day as can be completely replaced the same
working day. Particular care shall be taken to make the building watertight at the close of
each working day. This shall include water cutoffs, temporary flashings, seam integrity, repairs
to adjacent roofs, and all other items that might aid in this regard. Failure to protect the deck
and roofing from moisture will result in the removal of damaged materials or materials
containing excessive moisture at the Contractor's expense. Remove water cutoffs prior to
start of new work.
3.1,57 The Contractor shall refrain from installation work during rain, snow, or in the presence of
visible moisture.

3.1.58 Protect adjacent building surfaces against damage and bitumen spillage. Foot traffic or
material storage on adjacent building surfaces is not permitted.
HAZARDOUS MATERIALS
The identification, removal, abatement, and /or encapsulation of all hazardous materials shall be done in
accordance with all applicable Federal, State, and Local codes and regulations and with the following
special provisions:
3.1.59 The identification and testing of those materials determined to be hazardous by the Federal,
State or Local Govemment shall be the responsibility of the Owner. Neither Commercial
Consultants Group nor its employees are certified to identify, handle or monitor hazardous
materials in roofing, decking or insulation. Therefore, Commercial Consultants Group makes
no judgments on and hereby disclaim any responsibility for identifying, handling, or monitoring
hazardous materials.
3,1.60 Removal, abatement, or encapsulation of all identified hazardous materials shall be the
responsibility of the Contractor provided the Owner advises the Contractor of such
responsibility by means of an addendum prior to the bid due date or if such responsibility is
noted in the "Scope of Work" Article of this document. All costs associated with the
handling of hazardous materials under these circumstances shall be included in the base price
of the proposal forms.
3.1.61 Should hazardous materials be identified only during the construction and removal phase, it
shall be properly removed, abated, and /or encapsulated by the Contractor on a time and
material basis.
CERTIFICATION AND TRAINING
3.1.62 Roofing workers and supervisors who work with hazardous roofing materials must be trained
and certified by the Department of Health and Social Services or other approved state
and /or federal agency.
3.1.63 A roofing supervisor certified under regulations as set forth by the Department of Health and
Social Services (DHSS) must be on the jobsite at all times while work involving hazardous
roofing materials is being done.
3.1.64 Commercial Consultants Group has included information, where applicable, regarding
govemmental regulations on hazardous materials, However, Commercial Consultants Group
and US Bank assume no liability for providing information as would be provided by the
various regulatory agencies. All necessary precautions should be taken in strict accordance
with current EPA, AHERA and NSEHAPS Rules and Regulations and shall be implemented
by the Contractor upon discovery and /or announcement of such suspicion.
MATERIAL RESIDUE
3.1.65 The Contractor shall exercise extreme care to ensure that an absolute minimum amount of
residue and /or particulates resulting from any cleaning procedures, material removal, or
material application is permitted to enter the building, ventilators, air intake units, and any
potential entry shall be properly covered.

3.1.66 us BANK SHALL ACCEPT OWNERSHIP and responsibility for all particulates that enter the
interior of the building if any of these materials were a part of the original building
construction. This includes, but is not limited to, all existing roof materials, flashing materials,
insulation, decking or structural materials, and interior soundproofing or ceiling materials. If
any cleaning, decontamination, or general removal of these materials is necessary, it shall be
the responsibility of the Owner to properly accomplish this and at the Owner's cost.
3.1.67 The CONTRACTOR SHALL ACCEPT RESPONSIBILITY for all materials that enter the
interior of the building if any of these materials were delivered to the jobsite as a part of the
roofing project. This includes, but is not limited to, all new insulation, hot asphalt,
waterproofing materials, and mastics. If any cleaning or general removal of these 'materials is
necessary, it shall be the responsibility of the Contractor to properly accomplish this at no
cost to US Bank and to US Bank's satisfaction.
DEFICIENCY OF INFORMATION
The recommendations and specifications as set forth herein are based on visual inspections and other
information provided by US Bank. Every effort has been made to detect existing deficiencies prior to the
start of the project. Should a structural deficiency be detected during the project, it shall be determined
by US Bank and Commercial Consultants Group to what degree it will affect the long -term
performance of the new roofing system. The cost of correcting such deficiencies shall be borne by US
Bank and the following provisions shall prevail:
3.1.68 The Contractor shall remain responsible, as in all other areas of this Specification, to maintain
a watertight condition of any roof area that is so designated in these Documents. Should
progress be halted because of any changes, as determined by the Commercial Consultants
Group Representative and US Bank, the Contractor shall be responsible for providing a
watertight condition until the changes are approved.
3.1.69 US Bank reserves the right, should additional materials and /or labor be necessary for
compliance with said recommendations, to request competitive bids from at least two (2)
independent Contractors qualified to perform work in such related areas.
CONSTRUCTION PHOTOGRAPHS
3.1.70 Photographs of (1) damage or deteriorated structural components, (2) potential construction
or scope -of -work changes, or (3) pre - existing building anomalies must be taken and
submitted to Commercial Consultants Group prior to repairs wherever practical.
3.1.71 Photographs of specific items such as, but not limited to, replacement decking or roof deck
closures must be taken and submitted to Commercial Consultants Group in a timely fashion.
US Bank will reject any pay requests for items not so documented. Please refer to
instructions under "Additional Changes" in these documents.
ADDITIONAL CHANGES
3.1.72 Additional compensation for any labor or materials not clearly covered in the base quotation
will be allowed only when prior approval for such work is granted. The "Change Notice"
form should be available to the job -site Foreman and should be properly completed by the
Contractor prior to any departure from the base documents. All Change Notices should be
properly dated and numbered in sequence prior to submission.

3.1.73 All Change Notices should be properly completed and forwarded to the Commercial
Consultants Group Representative for preliminary approval and, if approved, will be
forwarded to US Bank for final, signed approval. Please note that a Change Notice should be
completed and properly signed even if the change is a "zero dollar" ($0) change.
3.1.74 Upon properly executed approval of all such forms, a copy will be distributed to the
Contractor so that he or she may comply with requirements associated with predetermined
billing procedures. Requests for payment covered under approved Change Notices shall be
accomplished in association with the regularly scheduled payment schedule as defined in
these documents under "Progress Payments" and "Final Payments."
3.1.75 Failure to receive said approval, in writing, shall indemnify US Bank from any costs associated
with such request unless said change in scope of work is deemed an emergency. Under such
circumstances, the contractor shall notify both US Bank and Commercial Consultants Group
verbally as quickly as is reasonable.
GROUNDS KEEPING,
Should damage develop relevant to grass, trees, shrubs, or other landscaped, paved, or asphalt areas it
shall be determined to what extent the damage occurred during the construction phase. The
Contractor, US Bank, and Commercial Consultants Group shall utilize Prejob Inspection photographs
and written notations as the definitive guidelines. All damage determined to have occurred during
construction should be properly repaired or replaced by the Contractor to the reasonable satisfaction of
US Bank.
DEBRIS REMOVAL
The Contractor shall be responsible for maintaining all work areas in a neat and orderly manner. The
Contractor shall provide adequate protection to the building surfaces so that bitumen and other related
materials do not encounter any related surfaces.
3.1.76 All ground areas shall be clean, neat, and orderly at the end of each day's work. All loose
roof-related materials should be properly stored in such a manner as to protect said
materials from inclement weather and /or potential vandalism.
3.1.77 All rooftop areas shall be clean and materials properly stored at the end of each day's work.

3.1.78 All waste receptacles shall be emptied or removed at the end of each day's work or properly
secured so that entry to such containers is restricted.
3.1.79 All drippage of bitumen or adhesives from walls, windows, floors, and finished surfaces shall
be the Contractor's responsibility.
PAYMENT REQUESTS & SCHEDULE
All payment requests for material on site, Change Notices, or work in progress shall be approved by US
Bank and Commercial Consultants Group prior to the issuance of any payments. The following
guidelines are designed to ensure that all valid requests for payment are handled in a timely, professional
manner, therefore ensuring the Contractor receives all payments as agreed:
3.1.80 All ORIGINAL INVOICES shall be drafted in compliance with the Purchase Order as issued.
3.1.81 A single copy of the original invoice shall be MAILED DIRECTLY COMMERCIAL
CONSULTANTS GROUP for pre - approval at the address as provided in these documents:

3.1.82 No DUPLICATE COPIES of any payment request and /or invoice shall be delivered via fax,
email, or US Postal Service to US Bank.
3.1.83 The Contractor may submit an invoice once each thirty (30) days for an amount equal to
work completed since the preceding invoice. In order to receive payment, the completed
work will be inspected and approved by US Bank and Commercial Consultants Group. A
retainage, at the discretion of the Owner, may be withheld.

4. DIRECT PURCHASES
US Banks shall require each Bidder to provide Direct Material Purchases information as reflected on the
Bid Proposal Form. US Banks shall acquire certain materials on a competitive basis and directly from
regional distribution based on the Bidder's required quantities as specified in the Bid Proposal Form.
Should the Bidder need assistance and /or clarification as it relates to direct purchases, please contact
Commercial Consultants Group for additional information. Additionally, the following SAMPLE of a
completed bid form (contractor required information highlighted in BOLD).

4.1 BUILDING INFORMATION
The Bid Proposal Form shall be pre - drafted to reflect the name of the owner, specific building, and
individual roof section covered by that SPECIFIC Bid Item.
4.2 BIDDERS BASE COST
The Bidder shall indicate the total dollar amount required to complete this individual roof section,
exclusive of the itemized materials as shown in the lower section of said bid form. The Bidder shall
include labor, all materials not reflected as Direct Purchases, as well as all other related costs as specified
in these documents.
4.3 DIRECT PURCHASES
The Bidder shall indicate in the "open" boxes of the Direct Purchases section, the exact quantities of all
listed materials as required by the Bidder to successfully complete this project.

4.3.1 Wisconsin Dells School District shall use the quantities listed by the successful Bidder to
solicit sealed, competitive quotations, from established Distributors, Wisconsin Dells School
District shall purchase said materials directly from the Distributor for the use on this project.
4.3.2 The successful contractor shall implement the delivery schedule with the materials
distributor(s) and will have all products as listed under Direct Purchases, delivered directly to
the jobsite for the Contractors use on this project, and pursuant to the Contractor's delivery
instructions.
4.3.3 The contractor shall be solely responsible for the acceptance of materials on site, including
the verification of quantities received for each shipment of materials.
4.3.4 Where a polyisocyanurate insulation board (flat, tapered, or saddle) is required, the Bid
Proposal Form shall have preemptively indicated the required quantities, generally in board
feet. The specific count, applied squares, and other information necessary to adequately bid
the labor to install said tapered, and /or flat stock insulation, shall be found within the
Inventory documents of this specification.
4.3.5 Any costs applicable to the loading, moving, unloading, and safe storage of all materials,
inclusive of Owner direct materials, shall be included in contractors dollar ($) amounts of
the Bid Proposal Form.
4.3.6 The purchase of any additional materials not specifically listed by quantity and size on the
original Bid Proposal Form shall be the responsibility of the Contractor under the constraints
of the originally quoted dollar amounts. The purchase of any additional materials must be
consistent with those provided under the Direct Purchase program.

5. INSTALLATION - APPLICATION OF BALLASTED EPDM SYSTEM
Note: All instructions as provided herein shall be indicative of the minimum standards and details as
required. Instructions as provided herein shall serve as a general outline, and where or if,
inconsistencies or duplications exist, the manufacturer's published guidelines shall prevail.
The actual installation (Scope -of -Work) shall under all circumstances remain consistent with the
published guidelines for the Ballasted EPDM system provided by the manufacturer.
COMPLETE REMOVAL AND EXAMINATION (SECTION #1 A. #16, & #1C)
The Contractor shall remove and dispose of all existing materials as indicated in strict accordance with
city, state, and federal safety guidelines:
5.1.1 Remove existing roof system(s) to structural deck, including all insulation, and properly
dispose of all debris off site.
5.1.2 Repair /replace decking on unit price basis upon notification and acceptance by Owner's
Representative.
5.1.3 Verify that surfaces and site conditions are ready to receive work.
5.1 .4 Verify that deck is supported and secured.
5.1.5 Verify that deck is clean and smooth, free of depressions, irregularities, or projections.
5,1.6 Start of work means Contractor accepts existing surfaces.
5.1.7 The Contractor shall also remove all existing sheet metal and dispose of same.
COMPLETE REMOVAL AND EXAMINATION (SECTION #9)
5.1.8 The Contractor shall remove and dispose of all existing materials as indicated in strict
accordance with city, state, and federal safety guidelines as specified above.
5.1.9 In addition, on Roof Section #9 all skylights shall be carefully removed and set aside from
existing curbs for re- installation in a further step.
5.1.10 Contractor shall carefully examine each skylight and it's associated structure to insure that no
damage exists prior to removal. Should any skylight be damaged in any way prior to it's
removal said contractor shall photograph and note same with Owner and Consultant.
Note: All materials required to complete work on Section #9 shall be included
in the base bid, and will not be considered under Direct Purchases.

As in all parts of these Specifications, the bidding contractor is responsible for attaining his or her own
information, including measurements and /or the make up of the system. However, the following
information may be helpful and is based on a single core cut made by Commercial Consultants Group.

INTERNAL DEBRIS
5.1.18 The Contractor is hereby alerted that debris cannot be allowed to enter the interior of the
premise.
5.1.19 The Contractor and the Owner shall work in harmony to alert personnel of potential deck
openings or related work issues that could compromise work areas and /or equipment. The
Owner shall be notified of potential deck openings and /or the entry of debris not less than
twenty -four 24 hours prior to any planned occurrence,
MECHANICAL UNITS - RAISING AND MOVING
5.1.20 It shall be the responsibility of the Contractor to raise any electrical units, as necessary, to
facilitate the proper roofing procedures including, but not limited to, all flashings. It will also
be the sole responsibility of the Contractor to reset each unit on pressure treated wood
curbs in harmony with the specifications as detailed by the NRCA and /or the documents
contained herein. Any units not able to be moved or raised shall have slip metal detail
installed as per SMACNA guidelines.
5.1.21 The Owner will be responsible for the disconnection and reconnection of all gas pipe lines,
electrical lines, and other utilities as necessary and in a timely fashion so that Contractor work
is not impeded.
PROJECTION HEIGHT
All rooftop projections, curbs, etc. shall be a minimum of eight (8) inches above the waterproofing
membrane. Where necessary, the Contractor shall install additional nailers to attain such height prior to
flashing of areas.

PROJECTION REMOVAL.
The Owner shall inspect the entire surface of the replacement area prior to the start of project. All
unnecessary projections shall be marked and identified prior to the project start. The Contractor shall
be responsible for the removal of said items and shall repair decking on a unit/per square foot basis.
PROTECTION - EXTERIOR
The Contractor shall protect all building surfaces against damage from roofing work. The Contractor
shall file a written plan with CCG prior to the start of the project indicating specific steps to be taken for
the protection of the building and related areas.
DECK PREPARATION
If any of the existing metal or concrete decking is deteriorated, it shall be replaced or reinforced as
required in strict accordance with the recommendation of the Decking Manufacturer. Any such repairs
will be done on a per square foot basis, per the Bid Documents, and only after a Representative of the
Owner has been notified.
VERTICAL SURFACE REVIEW
The Contractor shall thoroughly examine all vertical wall areas to ensure that all areas above the
counterflashing are in sound condition. Any area that will be exposed after new flashings are installed
shall be properly repaired using approved repair procedures. All such repairs shall be done on a time
and material basis and only as approved by CCG or the Owner.
EXISTING SERVICE LINES
The Contractor shall exercise extreme care when working in the general area of any existing gas lines,
TV cables, phone lines, exposed electrical conduit, etc, currently installed on the roof. All such lines shall
remain in place throughout the project unless otherwise noted and shall be raised only to the extent
necessary to install new roofing below.
PRESENCE OF CONDUIT /OTHER
5.1.22 Electrical conduit may be present either above the deck and /or below the roofing system or
attached directly below the deck on some commercial buildings. It is the responsibility of the
Contractor to ascertain the extent to which such conduit may, or may not, be present. The
Contractor is mandated to notify CCG and /or the Owner of any potential conduit issues
prior to the submission of bids.
5.1.23 All conduit which is visible (i.e., mounted to parapets, etc.) shall be disconnected, removed,
and reinstalled by the Contractor.

5.1.24 The Contractor shall examine all existing wood nailers for soundness and stability. Any wood
nailers needing replacement will be replaced with treated lumber of original size, thickness,
and be mechanically fastened at the perimeter edge according to Factory Mutual Loss
Prevention Data. Provide one - quarter inch (1/ venting between adjacent pieces of lumber.
Nailers needing replacement will be on a unit price per lineal foot basis.
5.1.25 The Contractor shall install new nailers necessary to facilitate any new tapered system or
those necessary to facilitate any new insulation heights. New nailers to facilitate any new
insulation heights shall be included in the base quotation.
INSTALL OF EPS TAPERED INSULATION SYSTEM
5.1.26 All insulation as specified, both tapered and flat stock polyisocyanurate /EPS insulation shall, as
a standard, shall be designed and installed at a minimum starting thickness of not less than 1.0
inches. All tapered shop drawings shall clearly indicate a starting minimum thickness of 1.0"
at the lowest point in the system, not including any overlayment board which may be
specified later in these documents.
5.1.27 Contractor shall loose lay new EPS tapered system over existing deck, insuring that each
board is tightly butted to adjacent boards — no voids shall be allowed.
5.1.28 Tapered system shall be laid in strict accordance with the "shop drawings" which shall
provide direction for install of each numbered board.
ALTERNATE - INSTALL OF ADDITIONAL INSULATION
5.1.29 Contractor shall install an additional loose laid layer of 1.5" polyisocyanurate insulation over
the new EPS insulation. Each flat 4' x 4' board shall be butted to the adjacent board(s) to
insure a smooth substrate for acceptance of the waterproof membrane.
5.1.30 Note: This is an alternate procedure and the cost of material and labor should not be
included in the base bid(s). The cost of labor shall be reflected on the bid form and the
materials shall be provided by the owner on a per - square -foot basis, as needed.
C RtCKETS /SADDLES
An additional layer of insulation shall be installed as a cricket/saddle between all existing and /or new
drains where necessary, as well as in other areas designated in the attached shop drawings. The design
shall be completed in such a manner so that water flows freely to each drain and no ponding water is
present.
CONSTANT PERIMETER
Taper insulation shall not interfere with the requirement to attain an equal outside perimeter edge
consistent with adjoining roofing sections. All final taper designs shall be approved, in writing, by CCG
prior to Owner acceptance on site of any taper materials,

PHASED CONSTRUCTION
5.1.31 The Contractor is advised that any existing waterproofing membrane removed in any one
day shall be replaced with base sheet, insulation, and waterproofing system the same day.
5.1.32 The Contractor shall apply a light "glaze coat" of hot asphalt over all newly installed plies of
glasfelts. The glaze coat shall be sufficient to provide temporary waterproofing capabilities to
the exposed felts until the standard application of flood coat (described in a future step).
5.1.33 The Contractor shall ensure that all substrates are clean and dry before installing any
insulation products. Care should always be taken to ensure that each board of insulation is in
factory delivered condition; the installation of broken or damaged boards will not be
acceptable. Contractor shall install all tapered insulation in strict accordance with the shop
drawings as provided for this project.
CONTROL JOINT INSTALLATION
5.1.34 Install new control joints in areas as marked on drawings. The cost associated with NEWLY
CREATED JOINTS work shall be reflected on the bid form as a per lineal foot" cost, and
not included in the base cost of the work by contractor,
5.1.35 Install additional treated lumber, where applicable, to achieve a minimum height above
insulation line of not less than eight (8) inches for purposes of proper flashing.
5.1.36 Install all new roof system flashings up and over all newly installed nailers in accordance with
Manufacturer's requirements. All joints should be completely watertight prior to the
installation of sheet metal caps. Under no circumstances should the flashings be cut,
penetrated, or otherwise damaged while installing sheet metal work.
5.1.37 Install metal roof expansion joint covers as shown in SMACNA Detail 5 -5B ( "A" Frame
Style) in accordance with Manufacturer's instructions.
5.1.38 Where necessary to facilitate raised or tapered insulation, expansion joint must be raised
beyond standard height to facilitate proper flashing.
5.1.39 Where new joints are adjacent to an existing roofing assembly, reflash the "nonroof
replacement" side in accordance with accepted and warrantable practices for that type of
waterproofing system. All costs associated with reflashing nonreplacement roofing shall be
included in the base cost of the proposal.
BALLASTED MEMBRANE APPLICATION EPDM) ASE
5.1.40 Beginning at the low point of the roof, place the membrane without stretching over the
acceptable substrate and allow to relax a minimum of 30 minutes before attachment or
splicing.
5.1.41 Install the membrane with the minimum number of field formed joints. Use the largest size
factory formed sheets as is practicable.

5.1.42 If possible start at high point of the roof and work towards the low point. Lap sheets so the
flow of water is not against the edges of the sheet.
5.1.43 Position the membrane so it is free of buckles or wrinkles. Do not stretch the membrane.
Lay the sheets with a minimum 4 inch lap.
5.1.44 Allow the membrane to relax for a minimum of 1/2 hour before securing or splicing.
5.1.45 Work the membrane tightly into all intersections of vertical and horizontal surfaces. Cut out
and patch all bridging or gaps in excess of 1/4 inch.
5.1.47 At gravel stops tum the membrane over the front edge of the nailer. Temporarily secure the
membrane to the wood nailer until the gravel stop and edge strip are installed.
5.1.49 At gravel stops extend the membrane over the edge of the water dam, terminate the
membrane at the base of the vertical portion of the water dam.
5. .51 At parapet walls, intersecting building walls, and equipment and skylight curbs, secure the
membrane to wood nailers. Nail the membrane 8 inches oc.
5. .53 At parapet walls, intersecting building walls, and equipment and skylight curbs, tum the
membrane up the vertical surface. Secure the membrane at the vertical and horizontal
intersection wfth the membrane manufacturer's anchor bar placed over the membrane and
secured to the vertical surface one foot oc. Cut off the membrane flush with the top of the
anchor bar.
5.1.54 At parapet walls, intersecting building walls, and equipment and skylight curbs, tum the
membrane up the vertical surface and terminate where shown on the Drawings.
SPLICING EPDM ROOF MEMBRANE LAP Jou'rrs:
5.1.55 Splice side and end lap joints of the sheet membrane wfth the manufacturer's inseam splicing
tape and seam cover tape. Do not use splicing cement.
5.1.56 Mark the bottom sheet along the edge of the top sheet with a marking crayon.
5.1.57 Cleaning and Preparing The Lap Joint:
5.1.57.1 Remove dirt and dust. Detergent -wash the splice area where dirt has adhered
to the membrane. Rinse with clean water and allow to dry thoroughly.
5.1.57.2 Solvent wash surfaces that will be in contact with inseam tape and cover tape
with natural fiber rags soaked in the manufacturer's recommended cleaning
agent. Clean the splice area until the sheet is clean and black, wfth no streaks,
and there is no trace of talc or foreign matter left in the splice area. Change
rags frequently to avoid spreading the talc or dirt.
5.1.57.3 The solvent wash is mandatory and cannot be eliminated regardless of the
manufacturer's requirements.
5.1.58 Installing Inseam Splicing Tape:
Spec20 I 0-Volume X I 5 -
5.1.58. I Apply the manufacturers primer to surfaces that will be in contact with the
inseam tape. Allow the primer to dry completely before completing the
splice.
5.1.58.2 Position the tape on the bottom sheet with the edge aligned with the
previously made markings. Roll the surface of the tape to insure good
adhesion.
5.1.58.3 Fold the top sheet over the tape. Trim the sheet as necessary so that 1 /4 -inch
of the tape is exposed.
5.1.58.4 Remove the release paper from the top surface of the tape and allow the
membrane to come in contact with the tape as the paper is being removed.
5.1.58.5 Roll the surface of the splice to insure good adhesion.
5.1.59 Installing Cover Tape:
5.1.59.1 Apply the manufacturer's primer to surfaces that will be in contact with the
cover tape. Allow the primer to dry completely before completing the splice.
5.1.59.2 Apply the cover tape centered over the seam, Roll the tape into position
while the release paper is being removed.
5.1.59.3 Adhere the tape to the underlying sheet so it is free of wrinkles, fishmouths
and voids.
5.1.59.4 Roll the surface of the splice to insure good adhesion.
PHASING OF SHEET MEMBRANE INSTALLATION:
5160 At the end of each working day temporarily seal the loose edge of the membrane so that
water does not flow beneath the covered portion. Spud off all existing aggregate (if any) in
the area to be sealed, remove all dirt, dust, and foreign matter. Install the temporary seal
using one of the following methods:
5.1.61 Apply a 1 2-inch wide application of hot bitumen over the area to be sealed. While hot,
embed the EPDM membrane into the bitumen. Before the Work resumes cut off and
discard all portions of the membrane that have been embedded in the hot bitumen.
5.1.62 Apply the membrane manufacturer's nite seal over the area to be sealed. Embed the EPDM
membrane into the nite seal. Apply a continuous weight over the membrane and nite seal.
Before the work resumes cut off and discard all portions of the membrane that have been
embedded in the nite seal.
5.1.63 Install flashings as the membrane is being installed (same working day). If the flashing cannot
be completely installed in one day, progress the installation until the flashing is in a watertight
condition.

FLASHING ROOF DRAINS (EXISTING):
5.1.64 Apply the manufacturer's water cut -off mastic around the perimeter of the drain body,
Embed the membrane into the mastic. Install the clamping ring and strainer.
5.1.66 Remove the existing clamping ring, coverstrips and lead flashing. Clean the contact area of
the drain body down to bare metal. Apply the manufacturer's water cut -off mastic around
the perimeter of the drain body. Embed the membrane into the mastic. Install the clamping
ring and strainer.
5.1.67 Secure the clamping ring with the existing bolts. Provide bolts to match existing to replace
bolts damaged or broken during the Work
5.1.69 Set the sheet metal roof drain over the sheet membrane. Strip in the flanges of the drain
with sheet flashing. Extend the flashing a min of 6 inches beyond the flanges. Splice the
flashing to the drain flanges and to the sheet membrane. Apply lap sealant at all exposed
edges.
INSTALLING UNCURED NEOPRENE OR UNCURED EPDM BASE FLASHING:
5.1.70 Complete the splice between the flashing and the roof membrane before bonding flashing to
vertical surfaces. Extend the splice on to the roof surface a minimum of 3 inches beyond all
fasteners.
5.1.71 Apply bonding adhesive to flashing sheet and to the substrate. Roll the flashing into the
adhesive.
5,1.72 Work the flashing tightly into all intersections of vertical and horizontal surfaces. Cut out and
patch bridging or gaps in excess of 1/4 inch.
5.1.73 At inside and outside comers fold the flashing around the comer so there are no breaks or
holes in the flashing. At outside comers, lap the adjacent flashing sheet a min of 2 inches; at
inside comers, lap the adjacent flashing sheet a min of 3 inches.
5.1,74 Nail the flashing across the top edge 12 inches oc.
5.1.75 Apply lap sealant at all splice edges.
INSTALLING EPDM ROOF MEMBRANE AS BASE FLASHING:
5.1.76 Apply bonding adhesive to the membrane and to the substrate. Roll the membrane into the
adhesive.
5.1.77 Work the membrane tightly into all intersections of vertical and horizontal surfaces. Cut out
and patch all bridging or gaps in excess of 1/4 inch.
5.1.78 Do not wrap the membrane around inside and outside comers. Form the comers with
sheet flashing.
5.1.79 Secure the membrane at the vertical and horizontal intersection with the membrane
manufacturers anchor bar placed over the membrane and secured one foot oc.

5.1.80 Strip in the anchor bar with 6 -inch wide sheet flashing centered over the bar and spliced to
the EPDM roof membrane.
5.1.81 Nail the membrane across the top edge 12 inches oc.
5.1,82 Apply lap sealant at all splice edges.
INSTALLING COMPRESSION BAR:
5.1.83 Where base flashing does not terminate beneath a cap flashing, seal the top edge as follows:
5.1.84 Set the top one inch of the sheet flashing in water cut off mastic.
5.1.85 Install a continuous metal compression bar over the sheet flashing and secure one foot oc.
5.1.86 Apply a bead of lap sealant along the top edge.
INSTALLING PIPE FLASHING:
5.1.87 Wherever possible flash all pipes thru the roof with the manufacturer's premolded pipe
flashing.
5.1.88 Cut the flashing to the proper diameter. Apply splicing cement to the bottom of the flashing
and to the contact surface of the membrane. Apply water cut off mastic between the
contact surface of the pipe and the flashing. Install the premolded flashing.
5.1.89 Install compression clamp around top of flashing. Apply lap sealant around the splice edge of
the flashing flange.
5.1.90 Where premolded pipe flashings cannot be used, use field fabricated flashing.
5.1.91 Coat the pipe, the flashing sheet and the contact surface of the roof membrane with splicing
cement.
5. .92 Install a square shaped piece of flashing over the membrane. Turn the flashing up onto the
pipe 1/2 inch.
5. .93 Wrap a second piece of flashing around the pipe. Extend the flashing 1/2 inch onto the
horizontal portion of previously installed flashing. Apply lap sealant at all splice edges and at
the top edge of the flashing.
INSTALLING BALLAST:
5.1.94 Before installing ballast underlayment and ballast, inspect the completed membrane and all
splice joints to insure that the system is watertight. Repair defects before proceeding.
5.1.95 Uniformly apply ballast at the rate of 1000 lb/square.

PROJECTIONS - SECUREMENT
The Contractor must properly reset and fasten flanges on all vent fans or roof projections that were
removed or raised during the course of this project. All such areas shall be properly stripped in per the
flashing system as specified. Any projection that cannot be moved or raised shall have a slip metal detail
installed as per NRCA guidelines.
SUPPORT ASSEMBLIES
The Contractor shall install two additional pieces to the waterproofing assembly wherever support
stands are present. At the base of any support stand (any leg or nailer), the Contractor shall install a
concrete pad not less than three (3) times the size of the surface of the base of the support leg. The
concrete pad shall be not less than 2 inches in thickness and shall have a precut pad of SBS material
placed directly below the concrete pad not less than one and one half times the surface of the concrete
pad.
WATER CUTOFF
At the end of the day's work, a water cutoff shall be constructed at all open edges. Construct the cutoff
with the same membrane and asphalt. Cutoff must be able to withstand extended periods of wet
weather. The water cutoff shall be completely removed prior to resuming the installation of the roofing
system.
TERMINATION OF WATERPROOFING
The Contractor shall note that all installed roofing systems must be completely watertight prior to the
installation of any sheet metal details. The Contractor shall notify CCG when all waterproofing materials
have been terminated within the constraints of the Manufacturer's published guidelines and in
conformance with published NRCA guidelines. CCG will inspect all termination points of the
waterproofing system prior to the application of sheet metal details as provided herein.
SKYLIGHTS - ROOF SECTION #9
5.1.97 Contractor shall re- install all skylights previously removed for purposes of roofing.
5.1.98 Each skylight shall be inspected prior to re- installation to insure that all components of said
skylights are fully functional and watertight.
5.1.99 Contractor shall prepare the existing curbs to accept each skylight in a fashion that will
maintain a watertight condition. This may include additional carpentry, foam backing, and /or
one piece metal flashings.
5.1.100 The final detail shall reflect a snug, water resistant fit for each skylight.

SHEET METAL DETAILS
All sheet metal details specified herein shall be installed in strict accordance with published guidelines as
provided in the Fifth Edition, 1993, with Addendum No. 1 October 31, 1997, Fourth Printing December
1999, of the Architectural Sheet Metal Manual. These specifications should also include all changes
including the 10 -31 -97 Addendum from the same manual. Unless otherwise stipulated in writing, all
details shall conform to current SMACNA standards. All metal shall be 24- gauge, hot - dipped galvanized
steel of commercial quality, extra- smooth primed and finished one side with Kynar® based
fluoropolymer coating. The Kynar® based finish shall have a prepainted side coated with a liquid applied
at the factory to induce a strippable film for protection of the finished surface during shipping,
fabrication, and installation.
5,1.101 One - Piece Counterflashings: Install new (to match existing) metal counterflashing on all
applicable areas, Caulk shall be applied in a smooth and continuous form and in accordance
with caulking guidelines set forth in these documents. All fasteners used shall have neoprene
washers and will be anchored not more than twelve (12) inches apart on center. The
Contractor shall install one (1) layer of Butyl Sealing Tape behind the leading, or upper, inside
edge of all counterflashing metal. A liberal amount of counterflashing sealant shall be applied
to the upper exterior edge of all metal counterflashing as a final step. All such sealants will be
installed in such a manner that no moisture (water) will pond on leading metal edge.
5.1.102 Metal Edge: Install all new metal edge (to match existing), where applicable, installed with
joint covers and 20 -mil. vinyl under all sheet metal in strict accordance with accepted
practices. Vinyl shall be trimmed at conclusion of project so that no loose or hanging vinyl is
visible, All metal edge exceeding eight (8) inches vertically shall be configured in general
harmony with Figure 2 -7, SMACNA. This guideline requires that all sheet metal configured
vertically in excess of eight (8) inches shall be joined to additional pieces of metal, allowing no
more than eight (8) inches of any single piece of finished metal to be exposed without
additional sections,
5.1.103 Coping Caps: Where applicable, coping caps shall be installed in lieu of a conventional
metal edge. When installing any coping cap, The Contractor shall ensure that the metal
attains a minimum slope of not less than 1/4" per foot from outside to inside. Wood support
shall be placed below the metal cap to ensure a secure surface; all supports shall consist of a
high - grade, tapered, treated lumber product consistent with good roofing practices.
5.1.104 Gutters & Downspouts: Contractor shall, where applicable, install new gutters and
downspouts of the same quality, color, and type as other sheet metal finish. All hanging
gutters shall be configured in harmony with Figure 1 -12, SMACNA. All downspouts shall be
open, 3 -sided configurations with the exception of the first seven feet (7') vertical from the
ground up. The first seven feet (7') shall receive a "front cover" to prevent the potential of
climbing and /or other damage. Contractor shall install gutters and downspouts as required
and additionally where marked on Roof Inventory Sheets.
5.1.105 Wood Fascia: All perimeter wood fascia areas where conjoined with metal edge and /or
gutter assemblies shall be clad with sheet metal in harmony with other sheet metal work All
sheet metal cladding shall be of the same type, quality, gauge, and color as the corresponding
gutter and /or metal edge assemblies.
5.1.106 Conductor Heads: All conductor heads shall be configured with the same type, quality,
gauge, and color as the corresponding metals, and in harmony with Figure 1 -25F, SMACNA.
5.1,107 Scupper: All scuppers shall be configured with the same type, quality, gauge, and color as
the corresponding metals, and in harmony with Figure I -29A, SMACNA.

5.1.108 Where roofing components cannot be lifted /removed, contractor shall install slip metal under
existing to insure a watertight condition.
CAULKING- PROPER USE
The Contractor shall note that all installed roofing systems must be completely watertight prior to the
installation of any caulking materials, The Contractor shall notify CCG when all waterproofing materials
have been terminated within the constraints of the Manufacturer's published guidelines and conforming
with published NRCA guidelines. CCG will inspect all termination points of the waterproofing system
prior to the application of caulking materials as provided herein.
5.1.109 The Contractor shall apply only high -grade caulking and only in areas as depicted on NRCA
drawings.
5.1,1 10 Caulking must be properly tooled and all excess material judiciously cleaned and removed.
5.1.111 The use of all caulking will be limited to a "secondary" application role. Under no
circumstances should the application of caulking be used as a "primary" waterproofing
procedure.
5.1.1 12 All caulking required to adhered end caps, etc. shall be applied under the cap and there
should be a limited exposure of caulking on the top of any cap areas.
5.1.1 13 The Contractor shall limit the use of caulking to a supplemental application of materials.
Excessive use of caulking materials is unacceptable.
CLEANING
5.1.1 14 Remove all construction markings from finished surfaces
5,1.1 15 In areas where finished surfaces are soiled by asphalt or any other source of soiling caused by
work of this section, consult Manufacturer of affected surfaces for cleaning advice and
conform to their instructions.
5.1.1 16 Repair or replace defaced or disfigured finishes caused by work on these sections. Project
Close Out.

6. CLOSE OUT
PROJECT CLOSE OUT
Project close out includes all general requirements of the agreement between the Contractor and US
Bank in preparation for substantial completion, project punch list, final inspection, and acceptance by US
Bank and Commercial Consultants Group.
6.1.1 Written notification by the Contractor of substantial completion shall be provided to
Commercial Consultants Group in a timely fashion.
6.1.2 A written list of items not completed should be noted with a reason and an expected date
for their completion.
6.1.3 All incomplete items (punch list) shall be listed with their scheduled values.
WARRANTY REQUIREMENTS - CONTRACTOR
The Contractor must provide a two (2) year or five (5) year written workmanship warranty at no cost
to US Bank, signed by the Owner of the contracting firm. This written wan-anty must be submitted to
Commercial Consultants Group as a part of the "Close Out" package. A five percent (5 %) retainage will
be held until this document is properly submitted to Commercial Consultants Group and US Bank. The
Contractor shall mark the "2 -Year Box" or " 5 -Year Box" on the Bid Proposal Form, indicating which
option the Contractor has selected.
FINAL INSPECTION
A final inspection will be scheduled when notice is given that all items required in the Project Close Out
have been successfully completed.
6.1.4 A final, visual inspection of the project will be attended by the Contractor, the Owner, and a
Commercial Consultants Group Representative.
6.1.5 Where a penalty clause of liquidated damages is included in the Contract, the completion
date to avoid such penalty or liquidated damage is the completion including final inspection.
Substantial completion is not to be interpreted as "project completion."
6.1.6 Inspection will be conducted by Commercial Consultants Group and US Bank on all work
listed within these documents, all adjacent building areas, and all ground and staging areas to
ensure compliance with these documents.
6.1.7 One final inspection will be performed by Commercial Consultants Group at no cost. All
other inspections resulting from noncompliance of work after the Final Inspection will be at
the expense of the Contractor.
SUBMISSION OF PAY REQUESTS AND WARRANTY DOCUMENTS
Final pay requests required by the Contractor to complete this project should be forwarded to
Commercial Consultants Group for final review and in accordance with the following:

6.1.8 The Contractor shall include with final payment requests any documents associated with the
request for issuance of a warranty, either material or contractor warranty, or both. This
would include any final inspection forms or other related documents. All such documentation
shall be forwarded directly to Commercial Consultants Group.
6,1.9 Final Lien Waivers and copies of signed and approved Change Notices and Payment Request
Forms must be sent directly to Commercial Consultants Group.
FINAL PAYMENT REQUESTS & SCHEDULE
A final request for payment can be accomplished in the following manner. These guidelines are designed
to ensure that all valid requests for payment are handled in a timely, professional manner, therefore
ensuring the Contractor receives all payments as agreed:
6.1,10 All ORIGINAL INVOICES shall be drafted in compliance with the Purchase Order as issued.
6.1.11 A single copy of the original invoice shall be MAILED DIRECTLY COMMERCIAL
CONSULTANTS G ROUP for pre - approval at the address as provided in these documents:
6.1.12 No DUPLICATE COPIES of any payment request and /or invoice shall be delivered via fax,
email, or US Postal Service to US Bank.
6.1.13 The Contractor may submit an invoice once each thirty (30) days for an amount equal to
work completed since the preceding invoice. In order to receive payment, the completed
work will be inspected and approved by U5 Bank and Commercial Consultants Group. A
retainage, at the discretion of the Owner, may be withheld.
6.1.14 The schedule of final payment shall be as follows:
6.1.14.1 Said final invoice must be received at the offices of Commercial Consultants
Group, LLC prior to the 10 of a specific month. Upon review and approval
of said valid payment request, the Consultant shall forward to the Owner
Commercial Consultants Group's comprehensive financial package, Project
2010 Cost Analysis, attached to the Contractor's original invoice.
6.1.14.2 Payment of invoices properly documented and processed shall be made by
the building owner, to the contractor, on or about the day of the following
month.
6115 A "FINAL WAIVER AND RELEASE OF LIEN" must be submitted with final payment
request.
