As the sheets indicate, Zoning and the Department of Public Works are requiring revised/additional information for the project. The Transportation Department has included an email communication advising of the status of on- street parking issues related to the project. The comments provided by the Oshkosh Police Department are advisory comments only and WPS does not see any issues with the project from their perspective. Also, the Oshkosh Fire Department is requesting information related to the loading capacity of the pavement to be utilized for the proposed pedestrian walkway and service drive. The following comments are general comments pertaining to the plans: A. The site has been assigned the address of 635 Elmwood Avenue. Please incorporate into your documents. B. It appears that the disturbed area will be greater than one acre. Therefore, a Notice of Intent shall be filed with the Wisconsin Department of Natural Resources. Please provide a copy of the application as well as a copy of the certificate of coverage when it is received. C. No construction activity, except utility connections, is allowed in the Elmwood Avenue right -of -way beginning March 2011 through the completion of the Elmwood Avenue reconstruction. Please note that elevations along Elmwood Avenue will need to be coordinated with the City of Oshkosh street design. D. Please add a note to the plans that states that curb and gutter in the right of way is to be repaired and replaced. E. Please show sidewalk through all driveway approaches. No partial panel replacement is allowed. F. Please add a note to the plans that states that a City of Oshkosh Right -of -Way Permit is required for any work performed within the City of Oshkosh right of way. G. Please add a note to the plans that states that all work performed within the right -of -way or any easements conforms to City of Oshkosh specifications. H. Please add a site data table to the plans that includes the total area, the disturbed area, and the impervious area before and after development. 2. The following comments pertain to the Erosion Control Plan (Sheet C101): A. Please provide Type D inlet protection in all inlets across the street from the site on all adjacent roads. B. Please change the reference of Type B inlet protection in the round inlet manhole on Algoma Blvd to Type D. C. Please provide a detail for the proposed erosion mat installation adjacent to the rain gardens and biofilters. D. Please add to the note on the construction entrance off of Elmwood Avenue that construction traffic shall not enter or exit the site from March 2011 until the end of construction on Elmwood Avenue for the city contract. 3. The following comment pertains to the Site Demolition Plan (Sheet C102): A. Please note that security fence locations may be subject to change for city work on Elmwood Avenue, as well as on Algoma Boulevard from Elmwood to Osceola. 4. The following comments are comments pertaining to the Site Utility Plans (Sheet C300) A. Please note that proposed sanitary sewer shall be Schedule 40 within the right -of -way. B. Please note that the water main shall be Ductile Iron, Class 52 with polywrap within the right -of -way. C. Please note that the lid on proposed Storm Manhole ST1 shall be Neenah R -1710, and shall be stamped "STORM ". Please add appropriate notes to the plans and the detail. Enclosed is a City of Oshkosh Storm Manhole Detail for your reference. D. Please note that the bury depth of markerballs is a maximum of 5'. Additionally, one markerball shall be installed at the main, one 6' off of the right -of -way into the right -of -way, and one at all horizontal and vertical bends. Please make all appropriate changes to General Notes 12 and 13 (see enclosure for detail). E. Please label the biofilters and rain gardens on the Utility Plan. F. The storm sewer sizing table states that the pipe from CB3 to the existing storm sewer in Algoma Boulevard is a 15" pipe, but it is a 12" on the plans. Please ensure all pipes are adequately sized and labeled. G. It appears that the existing 12" storm sewer in Algoma Blvd is inadequately sized to accept the proposed flows from the site. Please make appropriate changes to the storm sewer layout to ensure existing pipes have the capacity to convey the onsite flows. It appears that a 12" PVC storm sewer is proposed to be connected to an existing 12" storm sewer in Algoma Boulevard. Please provide a storm manhole at this location if this connection is to remain. I. Please explain why the proposed water service is being connected at its particular location in Elmwood Ave, and is being routed so far to connect to the building to the south east. It is suggested to connect the water service to the building closer to the connection in the road. 5. The following comment pertains to the Site Plan (Sheet C200): A. It is suggested to have the proposed driveway entrance on Elmwood Avenue be 18' at the right -of -way. 6. The following comment pertains to the Grading Plan (Sheet C400): A. Please provide spot elevations at all driveway approaches at the back of walk, front of walk, and at the end of the driveway apron. 7. The following comments pertain to the Site Paving Plan (Sheet C500): A. Please note that pavement in Algoma Blvd and Elmwood Ave is asphalt over concrete. Please add appropriate notes to the plans to ensure all public roads not subject to immediate reconstruction are restored to their original condition. B. If utility connections are made before Elmwood Ave is paved with the city contract, a temporary patch is acceptable. 8. The following comment pertains to the Site Construction Details (Sheet C600 -Sheet C604): A. It does not appear that the handicapped ramp in Detail 12/C600 is ADA compliant. Please revise accordingly. B. Please provide a detail for the proposed and existing limestone retaining wall located along Algoma Blvd. C. It appears that the footing for the retaining wall will be installed under the sidewalk (Sheet C604). Please shift the wall so that the footing is outside of the right -of -way. Any undermining of the sidewalk shall be filled with sand slurry only. 9. The following comments pertain to the Storm Water Management Report: A. The depth of Rain Garden #1 input into the WinSLAMM modeling does not appear to match the depth according to the detail in the plans. Please make appropriate changes to ensure all values correlate between the plans and the report. B. The "Depth of Biofilter that is rock filled" value for all rain gardens and biofilters in the modeling is input at 1', but is not 1' according to the details in the plans. Please make appropriate changes to ensure all values correlate. C. The Bottom Areas for Rain Garden #1 and Biofilter #1 do not match with the bottom areas as labeled in the plans on the Site Grading Plan. Please make appropriate changes to ensure all values correlate. D. It does not appear that the Disturbed Area Total in Table 6 has been calculated correctly. Please ensure all tables are tabulated appropriately. Please respond to each of these items in writing. Subject: RE: UWO Residence Hall Parking Kendra, I plan to forward this request for on- street parking to the Traffic Review Advisory Board for their consideration at its next meeting. While the street is relatively narrow, the street width with the bay appears to be adequate to support the proposed parking area. However, we have a few concerns on the street design of this section of Elmwood Avenue. • The installation of an on- street handicapped parking space would require some additional accommodation in the street design, including a flat grade, a much wider parking stall, and appropriate ramps to access the adjacent sidewalk system. For these reason, we typically do not encourage the provision of on- street handicapped parking spaces unless there is a demonstrated need. Based on the current layout, we don't believe that a handicapped parking stall can be accommodated in this location. • The sidewalk next to the parking stalls should be wider than 5 feet, in order to accommodate door swingout from parked cars. We recommend 8 feet of sidewalk width in that area. (We made a similar accommodation in this year's Main Street reconstruction project.) • Also, the City will need an easement to allow for moving the public sidewalk onto the UWO property. This would come through the Plan Commission and the Common Council. • Elmwood Avenue is scheduled for reconstruction in 2011, and it would make the most sense (in terms of both design and construction) to incorporate the parking area into the street reconstruction project. Would you foresee this impacting your construction schedule at all? • Finally, the UWO Master Plan proposes changes to Elmwood Avenue (Irving Ave to Lincoln Ave). Would these changes affect the need or value for parking on this section of Elmwood Avenue? The University intends to use the 4 proposed parking spaces along Elmwood Avenue for short -term (15- minute limit) parking. The building is scheduled to be completed in 2012, but approval of the parking is desired prior to construction. OSHKOSH NEW RESIDENCE HALL OSHKOSH, WI 54901 AUGUST 9, 2010 REVIEW CONDUCTED BY: OFFICER JOE NICHOLS OSHKOSH POLICE DEPARTMENT METHODOLOGY On Monday, August 9, 2010, Officer Joe Nichols reviewed the building plans for the proposed building of the New Residence Hall facility. After my review of this building proposal, I have identified potential concerns in regards to the following subjects and the recommended suggestions to alleviate concerns and the price of the product is estimated from various websites: NOTE: Survey recommendations, if implemented, should substantially reduce a criminal opportunity but are not to be construed as a guarantee that a crime will not occur on the premises. DISCLAIMER This security site review is being completed as a part of the City of Oshkosh Site Review Committee. There is no professional fee paid on the part of the submitting business to the officer conducting the survey. This survey report contains information designed to assist the business /organizational/home owner in increasing the physical security of the premises. The Oshkosh Police Department and the officer completing the report do not guarantee that implementation of any of the recommendations in this report will protect you and/or your property from criminal actions. Nor is there a guarantee that all deficiencies within and around your property have been identified and recorded within this report. However, implementation of the recommendations for those vulnerabilities will reduce the possibility of such criminal activity taking place. These lights are comparable in both price and durability to high pressure sodium and if a video system is used, the metal halide has better color rendering. All entrances /walkways should have ample lighting. Suggest using a pin pad/electronic card swipe for entrance into the building for residence hall occupants. VIDEO SYSTEM — Suggest video surveillance be used for the building on the entrance /exits to identify person/persons entering/leaving the residence hall. Video should be considered for rooms that have valuable items inside. UTILITY SYSTEMS - All electrical, telephone, and other utilities housed inside or outside of the residence hall should have proper locks on doors. OFFICES WITH COMPUTER EQUIPMENT AND OTHER VALUABLES — Offices that will have computers, data storage, money or other valuable should have proper deadbolts, video, and alarms. ALARM SYSTEM - Intrusion detection should be used for rooms where valuable items are stored. Motion sensors inside of offices or other valuable areas should be considered. VAULT OR SAFE — If money, vital records or other electronic storage device is used, special consideration into installing a fire resistant or burglar resistant safe is advised. The safe or vault should be visible during hours of darkness and lighted. KEY CONTROL - A record of assignments of keys distributed should be kept and updated when changes occur.
