Dear Sir or Madam,
You recently took ownership of a property located at 1132 W 5th Ave. in Oshkosh, WI as
a result of a foreclosure. This letter is to inform you that the property is in violation of
the Property Maintenance Code for the City of Oshkosh. This office has issued
Correction Notices to the former owners to resolve these matters but the violations have
not been corrected. I have enclosed copies of the Correction Notices that were issued.
Please contact me immediately so we may discuss your plans to bring your property into
compliance. If you have any questions regarding the requirements of this notice I can
answer them at that time. If you are unable to bring your property into compliance within
the next 30 days a schedule of corrections with a reasonable timetable shall be submitted
in order to receive an extension. If you fail to submit a schedule of corrections or contact
me by August 9, 2009 an Abatement Request will be submitted in an effort to resolve this
matter. A re- inspection shall be completed to verify compliance before this file is closed.

Description The exterior of every structure or accessory structure shall be maintained by the owner, occupant or person authorized to use same free of
conditions reflective of deterioration and /or inadequate maintenance. Siding shall be completed. Hole in fascia /soffit shall be repaired.

Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Please contact this office on or before the scheduled due date(s) to schedule a re- inspection of the property. If
compliance cannot be met by due dates, a schedule with dates must be submitted before the first due date and approved by
this office. The schedule must be met in order for it to be valid. Does not apply to one week compliance dates or hazards.
Violations must be corrected and approved within 30 days unless otherwise noted. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections, the owner /contractor /agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of _ 8/8/2009 _
Office hours for obtaining permits are Monday through Friday 7:30 -8:30 a.m. and 12:30 -1:30 p.m. or by appointment. To schedule
inspections please call the Inspection Request line at 236 -5128 noting the address, permit number (when applicable), and the
nature of what needs to be inspected.
