The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes
and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. The owner, as defined in
chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements.
The following conditions shall be met during construction or installation and prior to occupancy or use:

• COMM 64.22(1) Boilers and pressure vessels must be constructed and installed in compliance with the
standards of the American Society of Mechanical Engineers, as adopted under chs. COMM 41 & 42.
Submit
• COMM 64.22 (1) This review does not include approval or registration for the installation of Boilers and
Pressure Vessels as prescribed by COMM 41.41. The installation of any Boiler or Pressure Vessel shall be
registered with the Department by the installer before the system is placed in operation. Registration shall be in
writing on Form SBD -6314. This form, and additional information, may be obtained via telephone at 608 -266-

Reminders
• COMM 63.29(2) Provide piping carrying fluids less than 55oF or more than 80oF with insulation to the
thickness, and insulative value shown in COMM Table 63.29 -1, except where it can be shown that the heat gain
to or heat loss from the piping without insulation will not increase building energy use.
• COMM 64.23(4) No pipe carrying hot water, steam or other fluid at a surface temperature exceeding 250oF
may be placed within one inch of any woodwork, pass through a combustible floor, ceiling or partition, unless
the pipe is protected by a metal tube one inch larger in diameter than the pipe or with approved pipe covering.
• COMM 64.53(1) HVAC system balancing report shall be made available to the department upon request.
• COMM 64.53(3) Operating manuals concerning the HVAC equipment shall be presented to the owner.
• ADAAG 4.2.5 Forward reach thermostat controls shall be located 48" above the finished floor.
A copy of the approved plans, specifications and this letter shall be on -site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. All permits
required by the state or the local municipality shall be obtained prior to commencement of construction/installation.
In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review
shall relieve the designer of the responsibility for designing a safe building, structure, or component.
