WALLEYE FEST
Approval to the Otter Street Fishing Club to exclusively utilize the docks in Millers Bay from
12:01 a.m. on June 18 through 6:00 p.m. on June 20, 2010, for the purpose of running Walleye
Fest 2010, in accordance with the attached letter of request.
CHILDREN'S FISHEREE
Approval to the Otter Street Fishing Club to utilize the shore of Millers Bay, north of New
York Avenue, to the breakwater at the end of Menominee Drive, on Saturday, August 14, 2010,
from 5:00 a.m. to 3:00 p.m., for their annual Children's Fisheree, in accordance with the
attached letter of request.

Approval to the University of Wisconsin - Oshkosh to close Pearl Avenue, from Osceola Street
to the entrance of Parking Lot 413 on campus, on Monday, September 6, 2010, from 2:00 p.m.
to 8:00 p.m., for their Rec Fest event, in accordance with the attached letter of request.
NEIGHBORHOOD PICNIC
Approval to United Lutheran Parish to close Minnesota Street, from 9 1h Avenue to 11 Avenue,
on Sunday, September 12, 2010, from 6:00 a.m. to 4:00 p.m., for their Annual Neighborhood
Picnic, in accordance with the attached letter of request.
BLOCK PARTY
Approval to the residents of South Eagle Street to close South Eagle Street, from 6 th Avenue to
7 Avenue, on Saturday, September 18, 2010, from 3:00 p.m. to 10:00 p.m., for their
neighborhood block party, in accordance with their recently submitted Block Party Request
Form.

Dear Mr. Rohloff,
The Otter Street Fishing Club is the sponsor of Walleye Fest
2010, to be held at Menominee Park on June 18,19,& 20th. Each
year we have encountered problems with access to the two large
docks in the park. Boats tied up at those docks remain there and
prevent access by the over 280 boats registered for the
tournament. We are informed by the police department that the
current ordinance allows boats to remain at these docks up
to 18 hours.
We would like to request the City Council to grant an exclusive
use permit to the Otter Street Fishing Club for these docks from
12:01 am on June 18th until 6 pm Sunday, June 20th for the
purpose of running this event. The Fishing Club has contributed
half the funds to construct these docks and would greatly
appreciate their exclusive use during these three days. We thank
you for granting this request last year and ask your
consideration again this year. Also a copy of the permit (if
granted) for on site use is requested.
The Otter Street Fishing Club will cooridante the final details
of this event with the Oshkosh Parks Dept., and the Oshkosh City
Police Department.

I am aware that my participation in the tournament listed on my entry form and as in all fishing tournament(s) creates the risk of serious
personal injury or death. My participation could also lead to loss or damage to personal property which may result from my participation in
the Otter Street Fishing Club Walleye Tournament.
In exchange for being permitted to participate in the Otter Street Fishing Club Walleye Tournament, I hereby RELEASE AND DISCHARGE the
tournament administrator, the Otter Street Fishing Club, the officers, directors, members and volunteers, and the tournament sponsors,
Mercury Marine, Yar -Craft Boats, Bergstrom, Inc., Budweiser, Bemis Co., Inc., Pepsi Cola, Buyer's Guide, Lakewood Roofing, Inc., The Cumulus
Radio 99.5 Wolf, Blue Moose Convenience Stores and Battle on Bago from the liabilities or injuries, damages or other loss sustained by me caused
by the negligence or strict liability of the Released Parties. I do not release the Released Parties from liability for wilful or intentional conduct.
I expressly recognize that loss of property, injury or death could arise from (1) the fishing and boating activities contemplated by the
tournament, (2) the fact that competition rules contemplate that participants will fish in boats belonging to OTHER participants in the contest,
as well as their own boats, (3) the fact that the risks normally encountered in open water fishing from a boat may be enhanced by the
existence of competition.

TOURNAMENT RULES

1. Entry fee will be $240 plus $60 conservation fee plus $20 big fish (cash and merchandise award) plus $30 for Toumament Memorabilia.
Total entry fee $350 per team must accompany application,
2. Entrants must be 18 years of age or older and possess a valid Wisconsin fishing license unless fishing with a parent or legal guardian.
3. All applications will be accepted on a first come basis. ALL APPLICATIONS MUST INCLUDE A STAMPED, SELF -
ADDRESSED ENVELOPE. ALSO ENCLOSE AN ADDRESS LABEL FOR EACH CONTESTANT FOR CORRECT SPELLING.
4. All contestants must register between 12:00 P.M. and 6:00 P.M. on Friday, June 18, 2010, at Menominee Park tournament
headquarters. Information meeting 6:30 P,M. One team member must attend,
5. Each team must supply a minimum of a 14' motorized boat. It must be seaworthy and comply with Wisconsin state law,
6. No alcoholic beverages will be allowed in contestant's boat, or in any tournament official boat during tournament fishing hours.
7. At no time during the fishing day may a contestant leave his /her boat unless he /she returns to tournament headquarters and
permission is obtained from a tournament official,
8. All contestants must leave and return to Miller's Bay boat landing, Boats will be subject to inspection prior to the fishing day. Boat
check -in will be between 4:00 and 6:00 A.M.
9. Boundaries are the shores of Lake Winnebago, the railroad bridge in the mouth of the Fox River in Oshkosh, the rivers in Neenah
and Menasha will be open up to the first dam. All shore line channels are out of bounds. Any boat outside the boundaries will
be disqualified.
10. All contestants must be off the water by 12:00 P.M. on the day before the tournament, Friday, June 18th. Fishing is allowed only
during tournament hours thereafter.
11. Earliest departure time will be 5:30 A.M. All boats must be checked in by 3:30 P.M. There will be an official timekeeper, There will be
a four flight start. Explanation of start will be explained at contestant's meeting.
12. No contestant may anchor closer than 50' to another contestant's boat or come within 20' when trolling or drifting,
13. Only Walleyes will be weighed, Possession limit is 8 fish per boat. Your 5 biggest Walleyes will be weighed, (You pick.) Saugers
will not be a legal fish in the Otter Street Fishing Club Tournament.
14. All walleyes must be 15 inches in length or longer and weighed in at tournament headquarters, ANY UNDERSIZE FISH BROUGHT
IN WILL RESULT IN LOSING THE LARGEST FISH OF THE DAY'S CATCH.
15, All prizes will be awarded by total team weight. A 1 /10th pound bonus will be awarded for each fish deemed
releasable by a live release judge. The release site will be declared off limits. In event of ties, final placing will be determined by the
largest weight on the second day,
16. Prizes will be awarded on Sunday, June 20th at the conclusion of the toumament,
17, a.) Any contestant may protest another contestant.
b.) Protest must be made in writing to toumament officials by 4:00 P.M. on the day of the tournament. Protest fee is $100,00 Protest
fee will be returned if upheld.
c.) ALL DECISIONS OF TOURNAMENT OFFICIALS ARE FINAL.
18. No refunds after May 1, 2010. At least one member of the original team must fish the tournament. No partner changes af-
ter start of tournament, in case of sickness or injury, contestant who fishes alone may catch 5 walleyes and weigh 3 fish.
19. ALL CONTESTANTS ARE REQUIRED TO SIGN A WAIVER AND SUBMIT TO A POLYGRAPH TEST AT A LOCATION OF OUR CHOICE,
IF REQUESTED. At least one contestant of a team must pass a polygraph test before receiving prize.
20. 80% cash payback guaranteed.
21. If verification is not received within 21 days of mailing your entry back, contact the Otter Street Fishing Club.
22. It is the intention of the Otter Street Fishing Club to provide an honest sportsmanlike tournament. 'Spirit of the Rule" will prevail in
all judgment cases. Please read the Waiver you signed again.
23. Any team failing to inform tournament officials of departure prior to the end of the tournament will be disqualified from
future tournaments.
24. REFUSAL TO WEAR TOURNAMENT SHIRTS AND HATS AT SUNDAY'S AWARDS CEREMONIES IS A RULE VIOLATION. CHECKS
MAY BE WITHHELD.
25. THE OTTER STREET FISHING CLUB WALLEYE TOURNAMENT COMMITTEE RESERVES THE RIGHT TO REFUSE ENTRY TO ANY INDIVIDUALS
WHO HAVE BEEN BARRED FROM OTHER TOURNAMENTS FOR RULES VIOLATIONS.
26. ANY APPLICATION RECEIVED WITH CHECK OF INSUFFICIENT FUNDS WILL BE RETURNED AND THAT APPLICATION WILL BE
DISQUALIFIED.
27. It's the responsibility of all contestants to read and understand all tournament rules.

The Otter Street Fishing Club is asking the Oshkosh City Council
and Oshkosh Parks Department to approve our request to sponsor our
19th Annual Free Children's Fisheree, Saturday morning,
August 14th, 2010 at Menominee Park.
The area we would like to use is the shore of Millers Bay, North
of New York Ave to the Break Water to the end of Menoninee Drive.
The Otter Street Fishing Club will coordinate the final details of
this event with the Oshkosh Parks Director, and the Oshkosh City
Police Department.
If Approval is granted, The Otter Street Fishing Club is requesting

Dear Mr. Rohloff:
On Monday, September 6, 2010, the University of Wisconsin Oshkosh Student Rec and
Wellness Center will be holding an event to welcome back students to campus. Rec Fest will
take place from 5:30- 8:00pm with many activities such as volleyball, a soccer shoot -out, pony
hop races, music, free food and prizes.
To keep this event connected to the Student Rec and Wellness Center and not have to spread into
the mall area of campus, we would like to close Pearl Avenue from Osceola Street to the
entrance of parking Lot 13 on campus. Since there will be some set up involved, we are
requesting that the time of closure begin at 2:00pm. This will allow us to take some of the
activities onto the street and utilize the area in front of our building.

Dear City Manager,
The congregations of United Lutheran Parish (formerly First
English Lutheran and Peace) are holding their annual neighborhood
picnic on Sunday, September 12, 2010.
I am requesting that the street of Minnesota between 9th and 11
be closed on that Sunday from 6:00 am to 4:00 pm.
Please accept my invitation to feel free to join us on that day for
good food, games and entertainment. This outreach has become a
huge success and we couldn't do it without your help. Our
attendance has grown to almost a thousand!

BLOCK PARTY REQUEST FORM FOR STREET CLOSURE
The purpose of a BLOCK PARTY is to provide social interaction among block
residents in order to strengthen a neighborhood. Closing of a street for such a
party should not adversely impact any of the street's residents. The timeframe
for the street closure should fall sometime between 9:00 a.m. and 10:00 p.m.,
must be affirmed by the Oshkosh Common Council, and be based upon a
recommendation from the City Manager. This form should be filled out
completely and returned to the City Manager's Office in time for appropriate
action by the City Council.
