Box 1130 Oshkosh, WI 54902 -1130 Attention: Mr. David Patek, PE Director of Public Works Subject: Proposal for Construction Observation Services Sanitary Sewer / Watermain — Oshkosh Contract 09 -03 and 09 -05 Dear Mr. Patek: Thank you for the opportunity to provide construction observation services for the sanitary sewer and water main relocation work resulting from the upcoming USH 41 expansion project. We have enjoyed working with you on the design of this project, and look forward to continuing to provide quality engineering services to you on the construction phase of the project. Over the past five years we have provided construction observation services on numerous similar municipal projects in the City of Fond du Lac, the City of Ripon, and the Village of Jackson. We propose to use Ryan Arndt, Eric Jastromski and Brian Rogers as inspectors for the two projects. Ryan, Eric and Brian were the lead inspectors for a variety of municipal projects in the City of Ripon, City of Fond du Lac and the Village of Jackson last year, which consisted of sanitary sewer, water main, boring and jacking of casing pipe and new roadway construction. Hereinafter, the City of Oshkosh will be referred to as the OWNER, and Gremmer & Associates, Inc., the ENGINEER. SCOPE OF WORK The general scope of work for the project is to provide construction observation services for the sanitary sewer and water main relocation work to accommodate the upcoming USH 41 expansion project. Specifically, the construction observation services will be provided for City of Oshkosh contracts 09 -03 (work south of Lake Butte des Morts) and 09 -05 (work north of Lake Butte des Morts). A more detailed scope of work is as follows. The ENGINEER will provide the following services. Attend pre - construction meeting. 2. Provide full -time construction observation for the sanitary sewer, force main, water main, casing installation and associated road restoration items. The scope of the project is based on 24 weeks of construction for the 09 -03 project and 20 weeks of construction for the 09 -05 project. Furthermore, the scope is based on one full time inspector who will supervise both projects and will "float" between the two projects as necessary, one full time inspector for the duration of the 09 -03 project, and one full time inspector for the duration of the 09 -05 project. We will provide general specification conformance for items such as: a. Sanitary Sewer and Force Main. i. Check the contractor's procedures and materials for conformance with the project plans and specifications. ii. Check flow lines of manholes with an Auto Level and record for As -Built purposes. iii. Spot check line and grade of pipe. iv. Observe all necessary testing (Leakage Test and Go- No -Go) and provide test results to the OWNER. b. Water main. i. Check the contractor's procedures and materials for conformance with the project plans and specifications. ii. Check flow lines of water main fittings with an Auto Level and record for As- Built purposes. iii. Spot check line and grade of pipe. iv. Observe all necessary testing (Pressure Test) and provide test results to the OWNER. c. Casing installation. i. Verify that boring/casing work is within the tolerance of the specifications. d. Erosion control i. Perform erosion control inspections weekly and within 24 -hours of 1 /2-inch rainfall. e. Road restoration i. Observe all restoration of pavement and landscaping to ensure conformance with the project specifications. 3. Provide daily diary documentation of the work in accordance with OWNER requirements. 4. Provide the City of Oshkosh with quantities for pay estimates. 5. Provide an as -built plan. & A550CIATE5, INC. Provide construction staking services, if requested by the OWNER. It is our understanding that the City of Oshkosh is currently planning on providing the construction staking services for the project, however this staking item is provided in the event City staff is unable to provide staking services. Scope includes staking as noted for the following items. Additional stakes or re- staking shall be considered extra services. a. Sanitary sewer. i. Manholes with offsets (3 stakes). ii. End of casing pipes with offsets (3 stakes per end). b. Force main. i. Fittings with offsets (2 stakes per fitting). ii. End of casing pipes with offsets (3 stakes per end). c. Water main. i. Fittings/hydrants /valves with offsets (2 stakes per fitting). ii. End of casing pipes with offsets (3 stakes per end). OWNER'S RESPONSIBILITY The OWNER will provide the following: 1. Plan set. 2. Project specifications and bid documents. 3. Setting up and running the preconstruction meeting. 4. Public involvement/coordination concerning the project. 5. Construction staking. COMPENSATION ENGINEER'S fee for items 1 through 5 as outlined in the Scope of Work section of this document will be billed on a time and materials basis, not -to- exceed $274,755. This "not -to- exceed" fee is based on the contract schedule and associated personnel as outlined in the Scope of Work section of this document. If the contract schedule and /or associated personnel vary from what is outlined in the Scope of Work section of this document, the OWNER and ENGINEER agree to renegotiate the contract. ENGINEER'S fee for item 6 as outlined in the Scope of Work section of this document will be billed on a time and materials basis, estimated at $ 10,787. The ENGINEER shall prepare monthly invoices for actual services performed during the billing cycle, in accordance with the attached Professional Services Fee Schedule, dated March 1, 2009 and labeled Exhibit B. Invoices shall be paid by the OWNER within 30 days. Additional services beyond those called for in the Scope of Services will be billed according to the attached Professional Services Fee Schedule, dated March 1, 2009 and labeled Exhibit B. GENERAL TERMS AND CONDITIONS ENGINEER will perform services in accordance with the attached General Terms and Conditions dated March 1, 2009, and labeled Exhibit C. & A550CIATES, INC. CHANGE OF SCOPE If the execution of this proposal is delayed past April 10, 2009 by factors beyond the control of the ENGINEER, or if the Scope of Services is changed, the timing and compensation of this proposal shall be renegotiated. AUTHORIZATION AND TIMING If this proposal is acceptable, please sign below and return one copy to us for our files.
