This submittal was reviewed under Cbs. Comm 50-64, 66 and 69 as appropriate because the project it was
originally submitted prior to July I, 2002.

COMM 51.02( 15). The decorative combustible trim on the exterior walls of the building exceeds the
percentage limits in Table 51.02. Decorative Combustible Material, (EIFS) is required to have a Wisconsin
Material Approval Number that verifies its use as a noncombustible material on a Type 6 building. Please
submit a Wisconsin Material Approval Number for this product and have it available for the field inspectors.
COMM 63.01 Prior to installation, lighting plans and calculations shall be prepared in compliance with the
code and properly signed and sealed. The plans shall be available at the job site as requested by the Department
representative or local official.

COMM 50.115(1), (2) & (3) The erosion control information section of the plans approval application and/or a
review of the site plan indicates that the area to be disturbed is 5 or more acres and therefore a notice of intent is
required. The notice of intent shall be filed on form SBD-I0376 either with COMMERCE or with the certified
municipality or county. This form is to be filed at least 14 working days prior to commencement of

COMM 50.12 This review does not include heating, ventilating or air conditioning. The owner should be
reminded that HV AC plans, calculations, a completed SBD-118 application form and appropriate fees are
required to be subllÙtted for review and approval prior to installation.
COMM 50.155 This structure is indicated as being fully protected by an automatic fIre sprinkler system (see
NFPA 13). This approval does not include a review of the system The owner shall have and make available
upon request by the department a copy of the reports docwnenting the acceptability of the completed system
(see NFPA 13-1999).
As of this date, our records indicate the department has received partial payment for the service requested, the plans
