The submittal described above has been reviewed for conformance with applicable Wisconsin

. No permits shall be released, nor work commenced onthis project until plans are released
and can be provided on-site. If the conditionally approved plans are in the field, the
inspection group can safely assume all required fees have been appropriately paid. Failure to
pay within 30 working days will result in a denial.
Submit
. Approval of Plumbing Plans is not included with this review. Submittal of Plumbing Plans
and Calculations in compliance with mc 2901.1 and Comm 81 to 87 may be required
depending on the number of fixtures and type of facility. Approval of Plumbing Plans, if
required, shall be obtained prior to commencing construction.
A copy ofthe approved plans, specifications and this letter shall be on-site during construction
and open to inspection by authorized representatives of th~ Department, which may include local
inspectors. If plan index sheets were submitted in lieu of additional full plansets, a copy of this
approval letter and index sheet shall be attached to plans that correspond with the copy on file
with the Department. All permits required by the state or the local municipality shall be obtained
prior to commencement of construction/installation/operation. If this construction project will
disturb one or more acres of land, an Erosion Control No~ice of Intent (NOI) shall be filed with
the department 14 days prior to any earth disturbing activities.
In granting this approval the Division of Safety & Buildings reserves the right to require changes
or additions should conditions arise making them necessary for code compliance. As per state
stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for
designing a safe building, structure, or component.

Good Morning Mr. Ward;
I was advised by Shem Lemke of Hunzinger that you would be the most
appropriate person to contact about the following issue:
I was contacted by a local HVAC contractor that had been contacted by
your company about modifying an existing HVAC system to address the odor
issue created by the Nail Salon and is affecting air quality for the
adjacent Aurora Quick Care Center at the Oshkosh, Wisconsin Wal-Mart.
Please note this information has revealed there are Code Issues to be
corrected at this site.
Per the HVAC Plans for the Quick Care Center, these two spaces share one
HVAC System and the return air duct for this system is located in the
Nail Salon. As a result the return air is transferring the odors from
the Nail Salon into the adjacent Quick Care Center.
Please note this situation creates the following violations (or
potential violations) of the State of Wisconsin Building & HVAC Code:
1. The Nail Salon, per COMM table 64.0403, would be classified as a
Beauty Salon and as such is required to provide 1/2 cfm per s.f. of
floor area of exhaust (to be exhausted directly to the outdoors.) It is
unknown if this has been provided.
2. Comm 64.0403 3. Where mechanical exhaust is required by Table
64.0403, recirculation of air from such spaces shall be prohibited. All
air supplied to such spaces shall be exhausted, including any air in
excess of that required by Table 64.0403.
In layman's terms, no air from the Nail Salon (because it is
contaminated/odored) can be introduced into other areas of this
building.
As a result, revised HVAC Plans shall be required to be submitted to the
State for review and approval to address this design oversight. Please
note, the present State Approved Plans did not identify that a Nail
Salon existed and would be sharing the HVAC system with the Quick Care
Center.
Please advise me what steps will be taken to comply and the time frame
that is anticipated in complying.

Our tech has met with manager Carlos Redriquez at this location and the
manager is not aware of any issue about an extension to a vent pipe
No other members of management quite knew about it either when I called

let them know that we would have someone out to take care of that, and

said to find out from Carlos.

Good Morning Mr. Ward;
I was advised by Shem Lemke of Hunzinger that you would be the most appropriate person to contact about the
following issue:
I was contacted by a local HV AC contractor that had been contacted by your company about modifying an existing
HV AC system to address the odor issue created by the Nail Salon and is affecting air quality for the adjacent Aurora
Quick Care Center at the Oshkosh, Wisconsin Wal-Mart. Please note this information has revealed there are Code
Issues to be corrected at this site.
Per the HV AC Plans for the Quick Care Center, these two spaces share one HV AC System and the return air duct for
this system is located in the Nail Salon. As a result the return air is transferring the odors from the Nail Salon into the
adjacent Quick Care Center.
Please note this situation creates the folIowing violations (or potential violations) of the State of Wisconsin Building
& HV AC Code:
1. The Nail Salon, per COMM table 64.0403, would be classified as a Beauty Salon and as such is required to provide
1/2 cfm per s.f. of floor area of exhaust (to be exhausted directly to the outdoors.) It is unknown if this has been
provided.
2. Comm 64.0403 3. Where mechanical exhaust is required by table 64.0403, recirculation of air from such spaces
shalI be prohibited. AIl air supplied to such spaces shall be exhausted, including any air in excess of that required by
Table 64.0403.
In layman's terms, no air from the Nail Salon (because it is contaminated/odored) can be introduced into other areas of
this building.
As a result, revised HV AC Plans shalI be required to be submitted to the State for review and approval to address this
design oversight. Please note, the present State Approved Plans did not identify that a Nail Salon existed and would
be sharing the HV AC system with the Quick Care Center.
Please advise me what steps will be taken to comply and the time frame that is anticipated in complying.

Our tech has met with manager Carlos Redriquez at this location and the
manager is not aware of any issue about an extension to a vent pipe
No other members of management quite knew about it either when I called to
let them know that we would have someone out to take care of that, and they
said to fmd out from Carlos.
