This citation is scheduled for court on 8-6-97. Would you please have the Deposit amended to
$141.50 in lieu of the $1132.00 stated on the permit. Mr. Kita has corrected the violation, however
it does not warrant a complete dismissal. Thank you for your assistance.

NOTES: An inspection on April 23, 1997 revealed that a roof truss addition had commenced
over a flat roof area of Mr. Kita's contractor shop. There have been no plans submitted for
review for this activity, nor had Mr. Kita inquired with this office (in recent memory) about
erecting such an addition.
Notice was sent out on April 24, 1997 as a Final Notice to secure plan approval and obtain the
required Building Permit. Initially Mr. Kita and I traded phone call attempts, with the last
attempt by me to return his call. This was well over 1 month ago.
To date Mr. Kita has not made application for a permit, nor has an architect or engineer contacted
me about preparing the required plans for review so a permit might be issued. Mr. Kita has not
commenced removing the addition either, indicating he may wish to pursue completion of the
addition.
Mr. Kita is a Commercial HV AC contractor and is well aware of the requirements that work such
as this cannot commence without first securing the required permits. For this reason and the fact
he has failed to follow through in contacting me and proceeding with the correct steps to secure
the required permit, I am requesting a 7 day Citation to be issued. It is comprehendable that
people who do not work in the construction industry might not know about permit requirements,
however, Mr. Kita has no excuse.
