The following conditions shall be met during construction or installation and prior to occupancy or use:
. IMC 504/IFGC 613 Provide clothes dryer exhaust ducts such that they meet listed requirements which include,
but are not limited to, vertical cleanouts, required metal ducts, limited duct lengths, minimum duct size, and
pr011ibiti,on of screws to connect duct sections. Per codethe.clothe~ dry~rexhaustducts are shown as being
independent of all other systems and shalf convey moistttCe and products ofcombustlori outside the building.
. Comm 61.31(2) The additional loads cre(lted by the equipment shown on the plans were not included in the
structural calculations submitteci with the original building plan submittal, note revised building plans (Trans. ID
number 1464148) have been submitted and are scheduled to. be reviewed in Mid December, the revised plans
appear to address the HVAC equipment and access loads and information.
. IMC 507.12 & 13 The HV AC designer indicated that a Type II commercial kitchen exhaust hood will be
installed, that no cooking that would require a Type I commercial kitchen exhaust hood will be done in the
kitchen/prep area. Plan submittal of Type II hoods is not required, but must be installed per code.
. IFGC 401(Comm 65.0400 All gas piping installations shall comply with NFP A 54, NationalFuel Gas Code as
required by Comm 65.0400. ' .
. IMC 606 Provide a ductsmoke detection system in new duct systems, with appropriate controls, unless (1) the
return air rate is 2,000 cfm or less (See balancing report Comm 64.0313), OR (2) the air distribution system is
incapable of spreading smoke beyond the enclosing walls, floor and ceiling of the room or space in which the
smoke is generated. The smoke detection system shall shut down the air distribution system upon activation.
Smoke detectors shall be connected to a fire alarm system. The activation of a smoke detector shall activate a

visible and audible supervisory signal at a constantly attended location unless exceptions are met. The detectors
shall be located in the return duct. . .
. Revised plans were required to be submitted on the Oct. 11,2007 additional inform~tion.letter. IheJee
requested for the plan re-submittal received prior to a denial action was requested and indicated as being $50 +
$100 submittal fee = $150. As a service the. division will be invoising the designer for this amount.
. This review does not include approval for the installation of Mechanical Refrigeration Equipment on this plan.
The installation of any system using a Group Al or B 1 refrigerant and having a capacity rated at or greater than
50 horsepower, 50 tons or 50,000 volt-amperes; OR any system using a Group A2, B2, A3 or B3 refrigerant and
having a capacity rated at or greater than 10 horsepower, 10 tons or 10,000 volt-amperes. Registration shall be
in writing on Form SBD-34. The required Department forms may be obtained from the Division of Safety &
Buildings, Material Orders, P.O. Box 2509, Madison, WI 53701~2509, telephone 608/266-3151,608/264-8777
(TTY), or at the Safety & Buildings web site at http://www.coromerce.state.wi.us/SB/SB-
DivForms.html#MechRefrig. Contact the Refrigeration/Boiler Safety Inspector listed at the end oftlrii'lletter
with any questions.
. Conun 61.36(1)(c) This approval will expire 1 year after the date of this letter if the work covered by this
approval is not completed and the building ready for occupancy within that year.
A copy of the approved plans, specifications and this letter shall be on-site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. If plan index sheets
were submitted ~ lieu of additional full plansets, a copy of this approval letter and index sheet shall be attached to
plans that correspond with the copy on file with the Department. All permits required by the state or the local
municipality shall be obtained prior to commencement ofconstruction/installation/operation.
If this construction project will disturb one or more acres ofland, an Erosion Control Notice ofIntent (NO!) shall be
filed with the department 7 days prior to any earth disturbing activities. You will need to either file the NO! and an
erosion control plan summary on-line at wwW.commerce.wi.gov/sb or submit a completed NO! form and either a
plan summary or complete plan to us, with additional fees, or to the certified municipality.
In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review
shall relieve the designer of the responsibility for designing a safe building, structure, or componeni.
