Code 16-36A (1) Compliance No Compliance Date 02/06/2004 IMMEDIATELY
Every owner of two or more family dwelling units shall be responsible for maintaining in a safe, clean, good & code compliant
ondition inside & out all communal, shared or public areas of the dwelling and appurtenant premises. "Porches/decks shall be
kept free of refuse, furniture not intended for exterior use and/ or stored material". See Photos for locations and specifics on

Every occupant of a dwelling unit shall maintain in a clean, safe & code compliant condition that part of the dwelling unit or

unless it is safe and clean and complies with all provisions of the Minimum Housing Code and Property Maintenance Code.

Summary
It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Failure to confirm compliance {236-5137} may result in citations being issued and court action pursued. Please
submit a schedule of corrections to be made, along with compliance dates. Submitting the schedule will result in extended
periods of time to make repairs on those items not Life/Safety issues. 03/03/04- NO SCHEDULE SUBMISSION
Violations must be corrected and approved within 30 days unless otherwise noted. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections, the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 3/10/04 OWNER CONTACT
Office hours for obtaining permits are Monday through Friday 7:30-8:30 a.m. and 12:30-1 :30 p.m. or by appointment. To schedule
inspections please call the Inspection Request line at 236-5128 noting the address, permit number (when applicable), and the
nature of what needs to be inspected.

Disconnection of Electrical Service

Re: Disconnection of Electrical Service at: 612-614 Grand St
1. When the Electrical Inspector finds any electrical equipment to be
unsafe or dangerous to persons or property, the person owning or using
such equipment shall be notifed in writing to remove or cause to be
removed or make any changes or repairs determined by the Electrical
Inspector.
2. In the event of damage to an electrical service, whether by storm,
deterioration, vandalism, or other causes, all electrical service, 60 Amperes
or less shall be replaced.
3. At the time of service change, the wiring of rental dwellings shall be
brought up to at least the requirements of the Minimum Housing Code.
4. Failure to comply with orders to correct will result in Wisconsin Public
Service being directed by this office to be disconnected at
12:00 noon, March 10, 2004.

Per the correction notice Ijust received, I am enclosing a copy of the proposal from
Cumings Electric dated 2-16-04 to correct the problem.
I have asked Cumings to take care of it and they are scheduling it. I have not heard yet a
completion date. I will call them to expedite the matter.

Listed below are the specifications and estimates for property at 612 Grand Ave.
Oshkosh:
1. Furnish and install a duplex meter socket outside.
2. Furnish and install [2] 100 amp main circuit breaker panels in basement.
3. Install all circuit breakers and proper circuits to panels.
4. Install new pipe and wire outside to code.
5. Electrical permit.
6. Grounding per code.
We Propose to furnish material and labor-complete in accordance with the above
specifications for the sum of: $1,055.00 (One thousand fifty five dollars).
