HomeMy WebLinkAbout2007-Fire Dept Code Violation Notice OSHKOSH FIRE DEPARTMENT CODE VIOLATION NOTICE .,.: August 20, 2007 DELTA CHI FRAT INC 254 MEADOWBREEZE N CT APPLETON WI 54915 Dear DELTA CHI FRAT INC The Oshkosh Fire Department is required by Wisconsin State Statute 101.14 to provide for fire code and safety inspections for properties within the City of Oshkosh. In doing so, we have recently performed a fire inspection of DELTA CHI FRAT INC located at 911 WISCONSIN St . Our records indicate that you are the" responsible party" for this property and as such are responsible for correction of any and all fire code violations. Below are the fire code violations that were found during this inspection. HOUSEKEEPING/308.3.1, Open Flame Cooking Devices: No open flame cooking device shall be operated on combustible balconies or within ten (10) feet of combustible construction. Exception: LP cooking devices using one (1) pound LP containers COMMENTS: LP GRILL ON COMBUSTIBLE FRONT PORCH Found on: 01/14/2004 By: PIPER, Greg HOUSEKEEPING/313, Fueled Equipment Storage: Fueled equipment shall not be stored, operated or repaired within a room or building unless that room or building is designed for such use. Fueled equipment includes, but is not limited to, items such as motorcycles, mopeds, lawn care equipment, snowblowers and portable cooking devices. COMMENTS: LA WNMOWER STORED UNDER FRONT PORCH BELOW FRONT MEANS OF EGRESS Found on: 01/14/2004 By: PIPER, Greg MEANS OF EGRES/1006.1, Illumination of Egress Path: All means of egress pathways, including the exit discharge, shall be illuminated at all times when the building space served by the means of egress is occupied. At least one (1) foot-candle of illumination shall be provide at the floor level along the complete egress pathway. COMMENTS: EGRESS FROM BASEMENT TO OUTSIDE NEEDS TO BE LIT Found on: 01/14/2004 By: PIPER, Greg MEANS OF EGRES/1009.11, Handrails: Stairways shall have handrails on each side. The height shall be at least thirty-four (34) inches and no more than thirty-eight (38) inches above the tread nosing. The handrail shall be circular in cross section and have a diameter of at least one and one quarter (1 1/4) inches and no more than two (2) inches. The handrails shall extend at least twelve (12) inches beyond the top riser and continue to slope for the depth of one (1) tread below the bottom riser. The distance between the handrail and the wall shall be at least one and one half (1 1/2) inches. The handrail and the adjacent wall shall be free of any sharp or abrasive elements. Found on: 01/14/2004 By: PIPER, Greg FIRE PROT SYS/907.20, Smoke Detectors Insp. Test & Maintenance: Inspection, testing and maintenance of smoke detectors and other fire alarm or fire detection systems shall be in accordance with this Section and with NFP A 72. The building owner shall be responsible for the inspection, testing and maintenance. Personnel performing service on fire alarm and detection systems shall meet the requirements ofNFPA 72. Written records of all inspections, testing and maintenance shall be maintained on premise and shall be available for inspection be the Fire Code official. COMMENTS: 1. 1 ST FLOOR SMOKE DETECTOR - BATTERY MISSING 2. NO SMOKE DETECTOR AT TOP OF EAST STAIRS AT SECOND FLOOR. 3. SMOKE DETECTOR IN BASEMENT TO BE PROPERLY MOUNTED. Damage to any element shall require proper repair, restoration or replacement. All openings which penetrate fire resistive rated construction shall be properly protected so as to maintain the rating of the construction system or element COMMENTS: BASEMENT CEILING HAS PENETRATIONS IN PLASTER Found on: 01/14/2004 By: PIPER, Greg FIRE PROT REQU/703.2, Fire/ Smoke Doors: All fire/ smoke doors shall be maintained in operable condition. Fire door shall not be blocked or held in the open position by any means except with an approved automatic closure/ hold open device. These automatic closure/ hold open devices shall be maintained in proper working condition. If an automatic closure/ hold open device is inoperable, the door shall remain in the closed position. All fire/ smoke doors shall have a closing device installed. The closing device shall completely close and latch the door. COMMENTS: FURNACE AND WATER HEATER ROOM - DOOR CLOSURE DISCONNECTED Found on: 01/14/2004 By: PIPER, Greg HAZ MAT/2703.1, Storage, Use & Handling: The maximum allowable storage per control area for hazardous materials shall comply with Tables 2703.1.1(1) through 2703.1.1(4) COMMENTS: GASOLINE CAN UNDER FRONT PORCH BELOW MEANS OF EGRESS Found on: 01/14/2004 By: PIPER, Greg HAZ MAT/3003.3, Security of Compressed Gas Containers: Compressed gas containers, cylinders, tanks and systems shall be protected from physical damage. OSHKOSH FIRE DEPARTMENT CODE VIOLATION NOTICE Compressed gas containers shall be secured to prevent falling. LP TANKS TO BE SECURED FROM FALLING. TOW ON PORCH AND ONE UNDER PORCH. Found on: 01114/2004 By: PIPER, Greg HAZ MAT/3004, Upright Container Storage: Compressed gas containers, cylinders and tanks shall be stored in the upright position with the valve end up. Exception: cylinders and tanks designed for use in the horizontal position. COMMENTS: PROPER STORAGE OF LP CYLINDERS ON AND UNDER FRONT PORCH Found on: 01/14/2004 By: PIPER, Greg HAZ MAT/3004, Upright Container Storage: Compressed gas containers, cylinders and tanks shall be stored in the upright position with the valve end up. Exception: cylinders and tanks designed for use in the horizontal position. COMMENTS: Propane tank under the front porch. Internal illumination shall provide at least five (5) foot-candles of illumination and be listed for such use. External illumination shall provide at least five (5) foot-candles of illumination. A back-up power supply shall be provide where required in Section 1026.5 COMMENTS: Exit signs needed in basement. The numbers shall be arabic numerals at least four (4) inches in height and have a minimum stroke width of one-half (1/2) inch. The numbers shall contrast with the background. All systems shall be repaired or replaced when defective. The inspection, testing and maintenance shall be in accordance with the standards listed in Table 901.6.1. The records for the last three (3) years of inspections, testing and maintenance shall be kept on site and available for inspection by the Fire Code official. COMMENTS: MISSING MONTIIL Y FIRE EXTINGUISHER CHECK. FIRE EXTINGUISHERS NEED TO BE MOUNTED. LOOSE COMPUTER CABLE ON THE THIRD FLOOR COULD BE A TRIPPING HAZARD. Found on: 06/26/2006 By: GRISWOLD, Bill ELECTRICAL/605.6, Open/Improper Wiring - Dirty Motors: 1. Open wiring, open electrical boxes and damaged wiring shall be prohibited. 2. Electrical motors shall be maintained free ofthe accumulation of dirt, oil, waste and debris. 3. Temporary wiring is allowed for a period not to exceed ninety (90) days. 4. All temporary wiring shall be installed in an approved manner. COMMENTS: BROKEN LIGHT BULB IN SOCKET NEEDS TO BE REPLACED. Internally illuminated signs shall be listed and installed in accordance with the manufacturers instructions. These signs shall be illuminated at all times. Externally illuminated signs shall comply with Sections 1011.5.1 through 1011.5.3 COMMENTS: EXIT SIGN REMOVED AND NEEDS TO BE REPLACED.
