RE: Floodplain Standards for a Single Family Dwelling/Vitale on Hickory Lane

You have asked me to send you the applications relative to filling in a floodplain for the above
referenced location. I believe you have a copy of the memo dated June 1, 1995, which
essentially lays out the course you will need to follow, as well as information required to be
submitted with the request, in order to remove the area in question from the floodplain. I've
enclosed another copy of that memo in case you may have misplaced your copy.
As far as a permit process and application form, your first step is to.apply for an
excavationlbuilding permit to place fill in a floodplain through the City's Chief Building
Inspector.' Again, please provide the appropriate information as indicated in the memo. Once the
fill has been placed and a survey, performed by a certified Surveyor, verifying that the
appropriate elevations have been met, you will then need to request an amendment to the
floodplain maps. You will find enclosed a general application form for this purpose. A public
hearing process will be required wherein notification to adjacent property owners will be
necessary, as well as a $25.00 filing fee.

The above process will satisfy the local and State regulatory standards, but will not change the
insurance requirements. Again, the level of flood insurance for a structure which will essentially
be placed below the flood elevation, to my understanding, is significant. I would recommend
that you proceed with a request to the Federal Emergency Management Agency (FEMA) to
amend the Federal Insurance Rate Maps (FIRM) prior to the commencement of construction.
This request is called a Letter of Map Revision (LOMR) and must be made by the
developer/owner directly to FEMA. I believe an insurance agent should be able to help obtain all
appropriate forms in this regard. However, if you have difficulty in obtaining any request forms
or applications for a LOMR, let me know and I will see if I can help track those down.
