YOU ARE HEREBY NOTIFIED that the one-story wood frame house situated on the
following described property, to wit:
Lot Elevan (11) of Block Three (3) in GRUENWALD, HILTON & BUDICK'S
Subdivision, in the Fifteenth Ward, City of of Oshkosh, Winnebago County, Wisconsin.
Which premises are owned by you and known as, 1713 Ashland
Street, Oshkosh, Wisconsin has become damaged to such an extent as to be dangerous,
unsafe and unsanitary; otherwise unfit for human habitation, occupancy or use; is in
danger of structural collapse; and is in a condition making it unreasonable to repair the
same.
THEREFORE, you are hereby ordered to raze and remove this building within thirty (30)
days from the date of service of this order upon you.
YOU ARE FURTHER NOTIFIED that this order is served upon you pursuant to the
terms and provisions of Section 66.0413 Wisconsin State Statutes and in s~ph .case made

and provided. If you shall fail or refuse to comply within the time prescribed in this
notice, said building shall be razed and removed by the City of Oshkosh; and the cost of
such razing and removal shall be charged against the property, shall be a lien thereon, and
shall be assessed and collected as a special tax.
YOU ARE FURTHER ORDERED AND NOTIFIED that should you fail to comply with
this order to remove any personal property or fixtures in the above described structure(s)
in accordance with Sec. 66.0413(1)(i) and 66.0413(1)0), Wis. State Stats., within 30 days
from the date of service of this order upon you the City of Oshkosh may store the same,
or may sell it, or if it has no appreciable value may destroy the same. In case the property
is stored and the amount paid for storage shall be a lien against such property and against
the real estate and shall be assessed and collected as a special tax against the real estate if
the real estate is owned by the owner of the personal property and fixtures. If the
property is stored and not claimed by the owner it may be sold at the expiration of 6
months after it has been stored.

Brief history review:
On June 30, 2006 an inspection of the above property was conducted with Kelly Burnett. Photos were taken of
the interior an exterior of the building. The entire interior of the building has been gutted down to the' framing
with the exception ofthe first floor deck. Numerous structural deficiencies were noted and pictures were taken
to verify the deficiencies. The foundation is cracked and bowing in several locations. There are wall openings
that are not provided with proper headers to carry imposed loads. Floor joists have been notched and are
improperly spliced and do not appear to be able to carry imposed loads. The exterior has roof coverings
removed! or falling off, broken windows and peeling paint.
This structure is non-conforming in relation to setbacks established in the City Zoning Ordinance. City
Ordinance and State Statues state that structures where the cost to make structural repairs/corrections exceeds
50% ofthe current assessed value are deemed to be unreasonable to repair. The current assessed value of the
residence is $18300 therefore only $9150 could be spent to fix the structure. It is the opinion and determination
ofthis office, such repairs would exceed this 50% value. Therefore, I am requesting approval to have this
structure razed.

I spoke with the owner and he has not made any contacts to get his own estimates. So he told me to go ahead and
proceed. He would like us to remove the foundation in its entirety as he is developing plans to build a new house their this
spring (says he has a meeting set up with the bank.) Please ask Jeff to give us another estimate to remove the foundation
and backfill with clean fill and include 2-3-4 inches of topsoil (so if he does not build, it can be seeded this spring.)
Go ahead and have STS start their work.

I don't think we need a note. If he does not pay, it gets put on his tax bill. However, I left him a voice mail on his phone
advising of the fOllowing: Advised Kelly the additional cost to remove the foundation and fill will clean soil is in the
neighborhood of $1275. Since he requested us to proceed with demolition and asked ustoalso remove the foundation
and did not request a cost estimate on this, we will proceed if he does not contact me to object by the end of the day on
Tues. 2/13/07.

Dear Mr. Burnett;
This is to confirm our previous phone conversation and the message I left you earlier this week regarding
Raze and Removal ofthis house. Per our phone conversation of February 7, 2007 you requested that we
remove the foundation of this house when we execute the Raze Order vs. our standard procedure of
collapsing the foundation and burying it with clean fill. You requested this because you are planning to
build anew home on this site. We have secured a bid to Raze this house per your direction and the
estimated cost to Raze and Remove the house, including the foundation, is $5,610.
As I previously advised, this cost does not include costs to review for asbestos containing materials or
removal of such materials should they be discovered. These costs will be above and beyond the
aforementioned demolition costs.
If you reconsider your direction regarding the foundation, you must notify us no later than February 21,
2007. If you have any other questions, please call me at 920-236-5045.

Re: Proposal for Asbestos Building Demolition Survey and Demolition Assistance for the Residential

Dear Mr. La Fontaine:
This proposal outlines STS Consultants, Ltd. (STS), scope of work to perform asbestos and demolition
related services associated with residential structure located1713 Ashland Street, Oshkosh, Wisconsin.
We understand that your request for services will include performing an asbestos demolition survey of the
structures in order to comply with the Environmental Protection Agency (EPA) National Emission
Standards for Hazardous Air Pollutants (NESHAP) and the Wisconsin Administrative Code, Chapter NR
447, Control of Asbestos Emissions (NR 447). At your request, STS will provide the additional demolition
assistance with coordination of asbestos abatement and to comply with the Wisconsin Department of
Natural Resources assistance with the submittal of the Notification of Demolition and/or Renovation Form
4500-113 (Notification Form).
We propose to have a Wisconsin-Certified Asbestos Inspector from STS perform asbestos demolition
surveys in accordance with EPA NESHAP and NR 447. The procedure for conducting the asbestos
surveys will consist of a visual inspection, collection of samples, laboratory analysis of samples, and
presentation of results in tabular form. Representative samples of suspect materials will be obtained by
removing a small amount of suspect asbestos-containing building material (ACBM) and placing it in a
leak-tight container. Prior to sampling, the surface of the material will be wetted to reduce fiber release to
the atmosphere during sampling. Samples will be analyzed by Schneider Laboratories, Inc.; using EPA
recommended polarized light microscopy analysis with dispersion staining. ACBM is defined as material
that contains greater than 1 % asbestos.
Upon receipt of the laboratory test results and the subcontractor's report, STS will prepare a brief
letter-report, which will identify materials that contained asbestos, the approximate quantities, and
condition of these materials. A sketch map identifying the location of materials containing asbestos will
also be prepared, if necessary.
Abatement coordination assistance will consist of obtaining bids from qualified asbestos abatement
contractors, of behalf of the City of Oshkosh, and verifying the abatement of the ACBM~ Assistance with
the Notification Form will consist of review of the completed document and informing the City of Oshkosh
when demolition can commence.

For budgetary purposes we estimate 40 samples (@ $10 per sample) will be submitted for asbestos
analysis. We have attached to this proposal our Fee Schedule and General Conditions of Service, which
are expressly incorporated into, and are an integral part of, our contract for professional services. Please
ind.icate your acceptance of this proposal by having an authorized representative of your firm execute a
complete copy, and return it to our Oshkosh office. If we are given verbal or written notification to
proceed, it will be mutually understood that both of us will, nonetheless, be contractually bound by the
proposal, even in the absence of a written acceptance by you. In any event, a signed copy of this
proposal will need to be returned to STS before a written report can be submitted.
Your acceptance of our proposal confirms that the terms and conditions are understood, including
payment to STS upon receipt of the invoice, unless specifically arranged otherwise in writing. Of course,
