For health and safety reasons, we have turned off the gas ~upplY to this equipment. It is recommended that use ofthis equipment be discontinued until the condition!indicated with an (x) in the tabulation below has been corrected. ; 1. ( ) Leak in house piping, 2. ( ) Unvented appliance, 3. ( ) Obstructed or rusted out vent or chimney. . 4. (x) Defective safety or no safety pilot. Switch for thermostat is defective. Tenant says he has to wedge a wrench into connection for burner to co~e on; Needs to be repaired. 5. ( ) Underground fuel run uses unapproved plastk material. 6.
