Our gas mechanic has reported that an undesirable cond~tion exists on/in your thermostat.

This problem is class.ified as an immediate hazard by 'YPS, If not corrected, personal safety is at risk.
For health and safety reasons, we have turned off the gas ~upplY to this equipment. It is recommended that
use ofthis equipment be discontinued until the condition!indicated with an (x) in the tabulation below has
been corrected. ;
1. ( ) Leak in house piping,
2. ( ) Unvented appliance,
3. ( ) Obstructed or rusted out vent or chimney. .
4. (x) Defective safety or no safety pilot. Switch for thermostat is defective. Tenant says he has to
wedge a wrench into connection for burner to co~e on; Needs to be repaired.
5. ( ) Underground fuel run uses unapproved plastk material.
6. ( ) Appliance producing carbon monoxide. '
7. ( ) Other
