Signature
1. Owner shall request or confirm all change orders in writing. Change orders will be charged on a time and materials basis.
2. Payment terms: 1/2 down at least one month prior to start, 1/4 upon completion of 50% of the project, the remainder on completion.
Customer to pay a $250 late fee for any payments late by more than seven calendar days. Customer to pay interest of 2% per month,
compounded monthyly for late payments. Customer to pay court costs and actual attorney fees and expenses, including contractor's time and
travel expenses for unpaid amounts. All payments due are to be considered the cost of labor and materials necessary to complete contractors
performance under conditions of the job.
Acceptance of Proposal
The above prices, specifications and conditions are hereby accepted. You are authorized to do the work as specified. Payment will be made as
outlined above.
