This permit is not valid unless YOU have obtained all reauired approvals from other LOCAL, STATE and FED~RAL jurisdictions. 2. The riprap layer: a. Must be no less than 12 inches, consisting of clean, well graded fieldstone or quarry stone 6 inch to 24 inch diameter rock, plus a 6 inch filter rock layer, consisting of well graded 1/8 inch to 1 inch diameter rock or synthetic, sun resistent, filter cloth. b, Must have a finished slope on the bank of no steeper than 2 feet horizontal to 1 foot vertical (2H:1V). c. May not be placed in a manner that adversely impedes surface or subsurface flow into or out of any wetland. d. May not extend waterward more than 6 feet or 25% of the stream width, whichever is less, from the ordinary high watet mark: (OHWM), to provide a stable footing for the riprap. 3. The riprap project: a. May not damage fish or wildlife habitat, degrade water quality, obstruct navigation or other lawful public use of the stream, or adversely affect other owners of property abutting the stream. b. Must be constructed in such a manner as to prevent upland soil losses and sediment deposition into surface waters. c. May not reduce the effective flood flow capacity of a stream. d. May not result in the permanent or temporary deposition of fill in any surface. water or wetland. e. May not result in the waterward extension of the upland other than what is created by the necessary placement of rock riprap to stabilize or protect the existing shoreline. . . f. Must follow the natural contour of the shoreline as closely as possible, g. Must limit the removal of trees, shrubs and other shoreline vegetation to the minimum necessary for construction. Any vegetation removal above the ordinary high water mark: must comply with the requirements of the local county zoning ordinance, h. May not allow operation of equipment in the waterway unless otherwise. approved under "Other Conditions below". 4. The banks: a. Shall have any grading needed to comply with the condition 2b above done on the upland and the area of bank graded and/or filled may not exceed 10,000 sauare feet. Note: County Zoning ordinances may require permits for graded areas of less than 10,000 square feet. b. Shall have any grading or excavation confined to the minimum area necessary for construction. c. Shalf have any erodible areas riprapped, seeded, mulched or sodded to prevent erosion. Temporary and final erosion control measures are to be installed immediately and shall be kept in place until site stabilization is completed. Temporary erosion control measures such as straw bales and silt fences shall be put in place prior to any precipitation or if work ceases for longer ~.an 24 consecutive hours. 5. This permit expires on November 1 of the calendar year following the date of ISsuance. No construction may take place after that date unless you obtain a wmten extension of this permit from the Department before the date the permit expires, ;;' ....,~J .......l.'~: 6. You must maintain the riprap in good condition. 7. A photograph of the completed project shall be submitted to the Department within 10 days of project completion. ~, 8. You must keep a copy of this permit at the project site at all times until the project is completed.. 9. You must allow Department employees access to the project site to make inspections at any time to determine whether the work is in compliance with this permit and any other legal requirements. 10. If necessary, the Department may: a. Amend this permit by Changing or adding conditions to ensure that the project is in compliance with all applicable' legal requirements. b, Revoke this permit to protect the public interest or prevent material impairment of navigation. 11.
