reminded that HV AC plans, calculations, and appropriate fees are required to be submitted for review and
approval prior to installation. The submitted HV AC plans shall match the approved building plans.
Reminders
· Comm 61.30(3) This review does not include lighting. Comm 63.0001 Prior to installation, lighting plar,lS and
calculations shall be prepared in compliance with the code and properly signed and sealed. The plans shall be
available at the job site as requested by the Department representative or local official.
· Comm 62.1109(13)(a) Controls, operating mechanisms and hardware intended for operation by the occupant,
including switches that control lighting and ventilation, and electrical convenience outlets, in accessible spaces,
shall be accessible in compliance with the requirements ofICC/ANSI Al17.1.
· Comm 62.1110(1)(b) Accessible parking spaces required in Comm 62.1106 for the general public shall be
identified with a sign complying with the accessible parking sign requirements specified in s. Trans 200.7.
· Comm 61.36(1)(a) & (b) This approval will expire 2 years after the date of this letter if the building shell is not
closed in within those 2 years. Also, this approval will expire 3 years after the date of this letter if the work
covered by this approval is not completed and the building ready for occupancy within those 3 years.
· The submitted structural component materials HAVE NOT BEEN REVIEWED for compliance with all
applicable administrative rules. The department will rely on, and hold responsible, the building design
professional and/or supervising professional of record for compliance with the rules. The department reserves

the right to formally review the plans in the future if the department determines that such a review is warranted,
and to order corrective actions with respect to the outcome of that review. An exact duplicate of the plan we
have on file shall be available for inspection at the job site. When the total building volume exceeds 50,000
cubic feet, the plan shall bear an indication of review which has been signed or initialed by the building designer
of record.
A copy of the approved plans, specifications and this letter shall be on-site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. If plan index sheets
were submitted in lieu of additional full plansets, a copy of this approval letter and index sheet shall be attached to
plans that correspond with the copy on file with the Department. All permits required by the state or the local
municipality shall be obtained prior to commencement of construction/installation/operation.
In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this revi(~w
shall relieve the designer of the responsibility for designing a safe building, structure, or component.

the finished floor if the floor space allows a forward approach by a wheel chair or if the clear floor space allows
a parallel approach.
· Comm 61.36(1)(a) & (b) This approval will expire 2 years after the date of approval of the building plans if
the building shell is not closed in within those 2 years. Also, this approval will expire 3 years after the date of
building plan approval if the work covered by this approval is not completed and the building ready for
occupancy within those 3 years.
· IFGC 619 Unit heaters shall be supported by hangers and brackets which are noncombustible. Suspended type
unit heaters require clearances to combustible materials of not less than 18" at the sides, 12" at the bottom and
6" above the top where the unit has an internal draft hood or 1" above the top of the sloping side of the vertical
draft hood. Floor mounted type unit heaters require not less than 6" clearance on the back and one side. Where
the flue gasses are vented horizontally, the 6" clearance shall be measured from the draft hood of the vent
instead of the rear wall of the unit heater. Floor units shall not be installed on combustible floors unless liisted
for such use. Reduced clearances are only allowed if part of the unit listing. Ductwork may not be attach(~d
unless listed for such installation.

A review of the inspection records revealed a Certificate of Occupancy has not been
issued. You have failed to arrange the necessary inspections( s) to verify code
compliance. Our records indicate:
1- A final electrical inspection was never approved
