Mr. Sundquist,
I am writing this letter as a follow up to a Correction Notice that was issued on 10/19/04
and re-issued 6/20/05 for your property located 737 Division St. in Oshkosh. I have
enclosed an updated copy for your review as well as some new photos. Our records
indicate that the notice is still open. A re-inspection is required to verify compliance
before the notice can be closed out. Please contact me on or before January 19, 2007
with your schedule for the required repairs or to schedule the necessary re-inspection.

and/or occupancy. Upon completing the corrections, the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 7/1/2005 FINAL NOTICE
Office hours for obtaining permits are Monday through Friday 7:30-8:30 a.m. and 12:30-1 :30 p,m. or by appointment. To schedule
inspections please call the Inspection Request line at 236-5128 noting the address, permit number (when applicable), and the
nature of what needs to be inspected.
