Ms. Vaughan,
I am writing this letter as a follow up to a Correction Notice that was issued on 6/15/05
for your property located at 1125 N Main St. in Oshkosh. I have enclosed an updated
copy for your review as well as some new photos. Our records indicate that the notice is
still open. A re-inspection is required to verify compliance before the notice can be
closed out. Please contact me on or before January 19,2007 with your schedule for the
required repairs or to schedule the necessary re-inspection.

Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as specified
nd retum signed confirmation. Please read Frequently Asked Questions flyer & follow instructions. Any future violations of
his/these ordinance(s) will be cited $212.00, when found, along with written orders to correct.
Violations must be corrected and approved within 30 days unless otherwise noted. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections, the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 7/15/2005 ONLY NOTICE
Office hours for obtaining permits are Monday through Friday 7:30-8:30 a.m. and 12:30-1 :30 p.m. or by appointment. To schedule
inspections please call the Inspection Request line at 236-5128 noting the address, permit number (when applicable), and the
nature of what needs to be inspected.
