This problem is classified as an immediate hazard by WPS. If not corrected, personal safety is at risk. For health and safety reasons, we have turned off the gas supply to this equipment. It is recommended that use ofthis equipment be discontinued until the condition indicated with an (x) in the tabulation below has been corrected. 1. ( ) Leak in house piping. 2. ( ) Unvented appliance. 3. ( ) Obstructed or rusted out vent or chimney. 4. ( ) Defective safety or no safety pilot. 5. ( )Ul1derground fuel run uses unapproved plastic material. 6. (x) Appliance producing carbon monoxide. Boiler producing carbon monoxide; Shut off at appliance valve; Will need to fix before boiler can be operational again. 7. Call your appliance dealer or other service agency to assist you in making the necessary repairs. For informational purposes, a copy of this notice and the Caution Notice Tag are being forwarded to the building inspector or code official in your area.
