PURPOSE:
ACCEPT PROPOSAL AMBULANCE BILLING SERVICE
INITIATED BY:
CITY ADMINISTRATION
WHEREAS, the City of Oshkosh requested proposals for ambulance billing services
for the City of Oshkosh; and
WHEREAS, The proposals received were reviewed by a committee of employees
from the Fire Department, Finance Department, Information Technology Division and
Purchasing Division; and
WHEREAS, South West General Services best met the performance criteria being
sought; and
WHEREAS, South West General Services agreed to amend their proposal to
provide for a fixed commission percentage fee of 5.75% for both short and long term
collections; and
NOW, THEREFORE, BE IT RESOLVED by the Common Council of the City of
Oshkosh that the proper City officials are hereby authorized and directed enter into an
appropriate agreement for Ambulance Billing Services with:

Dallas, TX 75243
Fixed Commission Percentage Fee 5.75% of Net Collections
BE IT FURTHER RESOLVED that the proper City officials are hereby authorized
and directed to enter into an appropriate agreement for the purpose of same. Money for
this purpose is hereby appropriated from:

Fixed Commission Percentaqe Fee
S9uthwest General Services. is offering the City of Oshkosh a Comprehensive
EMS Reimbursement Solution which includes our proposed billing and collection
services and all training and adjunct support for our proposal, as stated in the
Detailed Proposal section of this response.
Southwest General Services is proposing a Fixed Commission Percentage Fee
on Net Collections (Gross Collections less refunds) as payment for services and
products that we are proposing to provide.. Our Fixed Commission Percentage
Fee will be invoiced at the beginning of the month and based on the previous -
months net collections. The invoice will be included with the month-end reports.
Our price is detailed below.

Lona-term Collections
Your request for proposal did not specify the time frame that determines the point
at whicn accounts that are not paid turn into long-term. conectiol1s. Normally, this
time' frame is at the 90-120 day mark from the initial billing. Southwest General,
Services will work with the City of Oshkosh to specify a definition of long-term
collections. We additionally propose a long term collections fee at a Fixed
Commission Percentage Fee for Long-Term Collections .that will be invoiced at
the beginning of the month and based on the previous months net collections.
The invoice will be included with the month-end reports. Our price is detailed
below.

APPENDIX A
PROPOSER GUARANTEES AND WARRANTIES
I. The proposer certifies it can and will provide and make available, as a minimum, all services
set forth in Section II, Nature of Services Required.
II. Proposer warrants that it currently has and will furnish an errors and omissions insurance
policy providing the amount of coverage identified. in this RFP for the willful or negligent acts, or
omissions of any officers, employees or agents thereof.
III. Proposer warrants that it will not delegate or subcontract its responsibilities under an
agreement without the prior written permission of the City of Oshkosh.
IV. Proposer Warrants that all information provid~d by it .in connection with this proposal is true.
and accurate.
V. Proposer must identify below any specific area(s) where they cannot meet the requirements
set forth in this RFP.

Collections" as stated in our original proposal under our "Fee Schedules" section
from 10% to 5.75%. Our commission percentage for all collections regardless of'
