The submittal described above has been placed on HOLD and Three (3) sets of plans are being returned. The review and
approval is pending subject to receipt of the ADDITIONAL INFORMATION and resubmittal of a minimum of Four (4) sets
ofrevised plans as requested by this letter. Upon receipt of the additional information and the revised plans, the plans will be
reviewed for compliance to applicable Wisconsin Administrative Codes and Wisconsin Statutes.
The following must be corrected and revised plans must accompany the resubmittal:

. COMM 50. 12(4)(a) The plan submittal did not include the required structural calculations.
Foundation calculations were not included
. COMM 51.03(6)( a)2. The plans indicate the pipe access mezzanine structural framing is of combustible
materials. This construction class requires the combustible framework and supporting walls to retain a I-hour
fire-resistive rating.
. COMM 51.06(3) The foam plastic insulation is required to be separated from the interior of the building by a
tested I5-minute thermal barrier. The department will accept Y2-inch gypsum wallboard as a thermal barrier.
COMM 51.06(6)(a)2. Provide automatic sprinkler protection for the coolers and freezers and for the room in
which the coolers and freezers are located. Or a Insulated Panel system that carries a Wisconsin Approval
Number per Comm 51.06(7) that allows the panels to be installed without the 15-minute thermal barrier. The
Approval Number must be shown on the plans.
. COMM 54. 12(2)(b) 1. The occupancy of this building exceeds the limit of 10 employees or 25 patrons.
Separate toilet rooms are required for males and females.

. COMM 69.35(1) The toilet room shown with a single watercloset and lavatory does not meet the minimum
space requirements as shown in Fig. 69.35-1 to 69.35-5.
. COMM 69.03(1) All new construction and additions shall comply with the requirements of Chapter 69.
. ADAAG 4.2.3 A clear wheelchair turning space of 60" is required in the shower room.

. COMM 51.043(1)(a) The design numbers for the fire-resistive assemblies proposed for use on this project are
required to be shown on the plans. The referenced designs must be from testing laboratories recognized by the
department.
. ADAAG 4.21.2 A roll-in shower shall be 30" x 60".
. ADAAG 4.21.3 The shower seat shall be between 17" and 19" above the floor and extend the full width of the
shower.
. ADAAG 4.21.4 The shower grab bars shall be mounted as shown in Fig. 37.
. ADAAG 4.22.3 An unobstructed turning space complying with ADAAG 4.2.3 shall be provided in all toilet or
bathrooms.
Revised plans are required to be submitted. The fee for a re-submittal received prior to a denial action is $50 + $100
submittal fee = $150. Be sure to include a copy of this letter with your re-submittal.
Send your resubmittal into the address listed above, unless otherwise noted, and the department will review the
resubmittal within 5 working days of receipt date. A copy of this letter is to accompany the resubmittal.
If the above requested information and/or plans are not received within 30 days of the date of this correspondence,
this submittal will be returned denied. No fees will be refunded, and a new fee, application form, and submittal of
plans/specifications will be required should you desire to continue with this project. The code in effect at the time of
new submittal would apply.

The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes
and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. The owner, as defined in
chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements.
The following conditions shall be met during construction or installation and prior to occupancy or use:

The wall panels with the Wisconsin Approval Number as listed on the plans are approved for unlimited interior
height without a thermal barrier and without an automatic sprinkler system normally required under s. Comm
51.06, provided FM-approved through-panel connections are used.
All joints and connections shall be in accordance with the manufacturer's recommendations.
Submit
. Submit, prior to installation, one (1) set of properly signed and sealed precast plans, a completed SB-118
application form including this transaction number and signed by the building designer, and $100 submina1 fee
to Safety & Buildings, P.O. Box 7162, Madison, WI 53707-7162.

A copy of the approyed plans, specifications and this letter shall be on-site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. All permits
required by the state or the local municipality shall be obtained prior to commencement of
construction!installation! operation.
In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review
shall relieve the designer of the responsibility for designing a safe building, structure, or component.
