RE: 15 BALDWIN AVE., OSHKOSH
Dear Mr. Miller:
The areas tested and found to be lead hazards atthe above property are now considered to be lead
safe by the Oshkosh Health Services Division after visual assessment was done on November 22,
2002, and clearance dust sampling also done on November 8, 2002. Please keep in mind that
periodic moIÚtoring is essential to prevent further problems. Please note that compliance with these
lead hazard reduction orders may not be sufficient to meet the standards to obtain a lead-safe
certificate in the future. The State Department of Health and Family Services (DHFS) may require
you to obtain a 12 month lead safe certificate as required in Chapter 254.171 of the State Statutes.
Thank you for your cooperation on this matter. If questions arise in the future, please feel fTee to
contact the Health Division.
Sincerely,
Jayne Tebon, RN
