Upon completing the corrections, the owner/contractor/agent must sign and date at the bottom of this notice and return it to the Inspection Services Division by the Compliance Date of 5/31/03 Introduction Inspection on 5-1-03 revealed the following items to be corrected immediately. Item # Code COMM 51.02(15) Compliance No Compliance Date 05/31/2003 IMMEDIATELY Description Per the 8-7-02 plan review letter, provide the Wisconsin Material Approval Number and assembly description that verifies the EIFS is a non-combustible material. Also make this installed system available for inspection to verify all the elements used rein compliance with the Wisconsin Material Approval. Failure to comply will result in denial of Occupancy Approval. Item # Code COMM50.12 Compliance No Compliance Date 05/31/2003 IMMEDIATELY Description Revised plans shall be sent to DOC for review and approval to include the mezzanine. A copy of the stamped state approved plans shall be provided to this office. Item # Code 7-8 Compliance No Compliance Date 05/31/2003 IMMEDIATELY Description fi'. Building Permit shall be secured for the mezzanine described in #2. The mezzanine floor system and surrounding walls þhall not be covered until plans have been approved, permit obtained and an inspection performed to determine compliance. Item # 4 Code 7-31 Compliance No Compliance Date 05/31/2003 IMMEDIATELY Description The General Contractor has failed to request inspections prior to proceeding beyond various stages of construction. This was brought to their attention at the start of this project when footings were being placed. Continued violation of this codesection ill result in citation issuance to the General Contractor.
