Dear Sir,
On December 1, 2005 a notice was mailed to you indicating that the license for your rooming house located at 630
Amherst Ave., Oshkosh, would expire on January 20, 2006. Having received payment, no contact has been made
for the required re-inspection of 630 Amherst Ave. A copy of the Rooming House License 60 Day Expiration Notice
has been enclosed with a section highlighted stating a re-inspection is required 30 to 60 days PRIOR to license
expiration. Therefore, occupancy at 630 Amherst Ave. shall be reduced immediately to no more than four unrelated
persons unless a re-inspection is arranged by 1/26/06. Please call Inspection Services Division at 236-5137
immediately to schedule an appointment.

According to our records, the license for operating the rooming house at
630 AMHERST AVE
will expire within
60 days. In order to renew the license, please fill out the application form enclosed with this letter and return it to this office within 15 days.
The fee for renewal is $145.00 per year. Make your check or money order payable to the City of Oshkosh.
A reinspection must be conducted 30 to 60 days prior to expiration of Rooming House License. If code violations are found during this
inspection, owner must make corrections and have reinspection conducted to verify corrections within the 30 days preceding expiration of
Rooming House License.
The Oshkosh Municipal Code 16-40 (C), states that no person may operate a rooming house unless he/she holds a current iicense or
a temporary use permit issued by the Housing inspector in his/her name for the specific rooming house.
Penalties for operating a rooming house without a current iicense are listed in Section 16-46.
