Furnish verification that the glass installed
in the door and sidelights is safety glass.
Provide handrail on open side of the ramp
which leads into the garage. Handrail to be
located 30 to 34 inches above the ramps
surface.
Provide guardrail on the west side exterior
landing. Guardrail shall be 36 inches minimum
height, and shall have intermediate rails which
prevent the passage of a sphere with a diameter
larger than 6 inches.
Complete the installation of the smoke
detectors on each floor level.
The guardrail and handrail on the open side of
the stairway to the second floor must be able
to withstand a 200 pound load applied in any
direction.~Also, an additional handrail shall
be provided on the side of the winder ~teps

FINDINGS OF INSPECTION
All stairways must have uniform riser height
and tread depth. No riser may exceed 8 inches
in height. No tread may be less than 9 inches
wide, measured horizontally from nosing to
nosing. The basement stairway does not meet
this requirement.
The required handrail must serve the full
length of the basement stairway.
Furnish verification from the I-Joist
manufacturer that the joist hangers used do
not require additional web stiffeners where
the sides of the hanger do not laterally
support the TJI joist top flange. This issue
was brought up during our framing inspections.
The following are recommendations, not
requirements of the UDC:
A.) Install a 20 minute rated door and frame
assembly on the elevator equipment room.
B.) Install drywall on the storage area wall
under the basement stairway which is open
to the elevator equipment room.
c.) Install closer on the door to the elevator
equipment room, in conjunction with the
lock which is already installed; or
install some other form of protection for
the occupants from the exposed moving
parts which potentially could cause
ser~ous ~nJury.
All above items must be corrected, and final
inspection called for within 30 days of this
notice in order to secure the certificate of
occupancy. Please call 236-5051 if you have
any further questions in this matter.
Deficiencies must be corrected and approved before concealment. Call
36-5051 to arrange for a final inspection.

FLOOD PROORNG OF FOUNDA nON:
As consultant for Maher Architectural Systems, I have made the following provisions on
the above property, to provide adequate protection from flood waters in the event they
occur. Basis for regional flood elevation is 749 feet.
1. Exterior footing and foundation wall is protected externally by 3 feet of gravel fill
rising two feet above regional 100 year flood elevation.
The foundation wall is protected by I 1/2" rigid insulation on a minimum 6 mil V.B.
The foundation wall itself is structurally reinforced with two horizontal rows of #4's,
two feet on center and #4's vertically four feet on center.
The footing is structurally reinforced with four #4 rebar -4 inches on center and tied to
the foundation with hooked #4 rebar, four inches on center to provide structural integrity
and prevent flotation and lateral movement.
2. The basement slab is placed at 749.4' elevation and is structurally reinforced with 4x4-
lOxia WWM. It is prevented from floating by one foot of compacted gravel placed
between the existing grade and the slab bottom.
3. Rood water penetration is prevented by the use of water stop at all slab I foundation
joints. In addition, the slab itself is water proofed by the use of 2ea 4 mil vapor barriers.
4. Drainage tiles in gravel beds surrounding the foundation are drained to a sump and
,while not crucial to flood protection, will add protection by relieving soil pressure and
reducing uplift.
5. The house is placed on the property so as to offer unimpeded flow of flood waters
around the site to Lake Winnebago.
6. The site is graded as to provide maximum runoff of both rain and receding flood
waters.
7. All interior crawl spaces and floors have bases that are a minimum of two or more feet
above regional flood elevations.
