An Occupancy Permit is hereby granted for the two-family residence located at 2471-

Application number(s) 92856 & 93314.
This building is to be used only as a duplex and is located in the R-2PD, Two Family
Residence District with a Planned Development Overlay.
LIMITATIONS:
Maximum persons and/or living units: One family per dwelling living unit
NOTE:
1) Copies of inspection results are available upon request in room 205, City Hall.
2) Future permits may be required for additional work to your property.
A new Certificate of Occupancy shall be required prior to occupancy, should additional
building(s) be erected, or should any buildings mentioned above be altered or moved.
The use of land, or buildings, shall not be changed until a Certificate of Occupancy is
issued for that occupancy. All conditions noted above must be complied with in order
for this certificate to be valid.

Statement of Substantial Compliance as evidence of code compliance as allowed and
r;Jò required by the State of Wisconsin Building Code (section Comm 50.10(3)).

building(s) be erected, or should any buildings mentioned above be altered or moved.
