OSHKOSH TRUCK 2005 OPEN HOUSE
THlS AGRBEMEN1' ma.dc and enlered into this fl d.ay of fVJoÞL. . 2005, by and
betWeen OSH1{OSfl !l<UCK CORPORATlON, 2307 OREGON ST~H, WI 54902,
'hereinafter called ..OSHKOSH TRUCK", and the City of Oshkosh, a municipal cotporatlon\
hcroinaner called "ChylJ.
WHEREAS, OSHKOSH TRUCK has requested tho City provide a dedicated paramedic
. ambulance during specified hours of the Oshkosh Truck 2005 Open House ¡Event'.
WHEREAS, the Common Council for the City of Oshkosh has authoTÌ?ed the appropriate
C,ty officials to entor into such an agreement
IT IS AGREED by and between tho parHcs lhat the City shaH provide one (1) lwo-þerson
paramedIc MEDTEC Ambulance to be present at 333 W. 29111 Ave" South Plant - Test Bay
Salurday, May 21, 2005, 'belween the hours of 9;45 a..m.10 3: I S p.m. The role of this unilshal1 b;
to provide Basic Lite Suppon (BLS) and Advance Lira Support (AL.S) for emergencies, if they
OOC\.ll, Emergençi~~ requiring a response beyond the capabilities of the unit on scene will be
supplemented by a response of on-duty resources.
IT IS FURTHER AGREED, UUI.t OSHKOSH TRUCK. shaJl pay S65,OO pet" hour for the
M'BDTEC Ambulanoe. BlUed time will include 30 minutes before and after the specified. hour¡ of
9:45 a.m. to 3: 1 S p.m. for travel and setup. AJ1yaâðblonal hOLl.T$ requested or required to provido
adequate service shall be biUed to OSHKOSH TRUCK at the same hourly rate. A three hour
minimum win be charged ¡fthe evenL is not cancelled at )east 2 hours prior to the scheduled start
time, Pre-event canu11aiion shall be made b)' çaHing 920-236-5271.
"!'he City shall invoice OSHKOSH TR.UCK following the event, with payment to be made
within 30 days of receipt ofinvoice.
An patients requiring lransport by 1he Oshkosh Fire Department MEDTEC Ambu1ance wiIJ
be billed for seIVic~s rendered according to the City's ususl and cuslomary procedures and T1.tes for
service.
