Reviewing the file for 280 W 35th Avenue it was noted that a Certificate of Occupancy has not been
issued. Certain items are missing,therefore, the required Certificate of Occupancy cannot be issued. ,

obtaining a re-inspection of the attached noted code violations.
❑ A Final Electrical Inspection conducted on revealed the attached code violation(s)which
have not been re-inspected for compliance. These code violations prohibited Occupancy

The submittal described above has been reviewed for conformance with applicable Wisconsin Administra[ive Codes
and Wisconsin Sta[utes. The submittal has been COND[TIONALLY APPROVED. The owner,as defined in
chapter 101 A1(l0), Wisconsin Statutes, is responsible for compliance with all code requirements.
The following conditions shall be met during construction or ins[allation and prior to occupancy or use:
• COMM 50.12� This review does not include heating,ven[ilating or air conditioning. The owner should be
reminded that HVAC plans and calculations are required to be submitted for review and approval prior to
ins[allation.
• COMM S0.l2 This review does not include ligh[ing. The owner should be reminded ihat lighting plans and
calculations are required to be prepared and signed and sealed by a properly licensed engineer,architect, or
lighting designer. These plans shall be kept at die job site and available for inspecror review.
• COMM 50.155 This structure is indicated as being fully protected by an automatic fire sprinkler system(see
NFPA 13). This approval does no[ include a review of the sys[em. The owner shall have and make available
upon request by the department a copy of the reporis documenting the acceptability of Ihe comple[ed sys[em
(see NFPA 13-1999).
• COMM 50.17(1)(a) This approval will expire 2 years after the date of this letter if the building shell is not
closed in within[hose 2 years. Also, this approval will expire 3 years after the date of[his letter if the work
covered by this approval is not completed and the building ready for occupancy within those 3 years.
• COMM 54.12(1) The plans do not indicate that any additional toilet rooms or fixtures are included as a part of
this project. The additional occupant load created by this projec[may require additiona]sanitary facilities.
COMM 54.12(l)(c) The plans do not show adequate sanitary faeili[ies for the caleulated oecupa�t load of this
building—when calculating the building capacity it appears that appoximately 250 people could be employed
here, please verify[ha[there are an adequate number of sanitary fixtures.
A copy of the approved plans, specifications and diis letter shall be on-site during construction and open to
inspection by authorized representatives of the Deparmient, which may include local inspectors. All permits
required by the state or ihe local municipality shall be obtained prior to commencement of
