FROM CITY OF OSHKOSH FIRE DEPARTMENT TO LIFESTAR EMERGENCY
MEDICAL SERVICES, LLC FOR SOUTHERN WINNEBAGO EMS AMBULANCE
SERVICE AREA
Whereas, the Town of Black Wolf, Town of Nekimi, Town of Omro, City of Omro,
Town of Poygan, Town of Rushford, Town of Utica, Town of Winneconne and Village of
Winneconne previously contracted with the City of Oshkosh for ambulance services
provided through the Oshkosh Fire Department; and
Whereas, the City of Oshkosh terminated ambulance services to these townships
under the previous Ambulance Service Agreement effective December 31, 2025; and
Whereas, the Town of Black Wolf, Town of Nekimi, Town of Omro, City of Omro,
Town of Poygan, Town of Rushford, Town of Utica, Town of Winneconne and Village of
Winneconne have jointly created the Southern Winnebago EMS Ambulance Board for the
purpose of providing future ambulance services within these areas (hereinafter “the
Southern Winnebago EMS Ambulance Service Area”); and
Whereas, the Southern Winnebago EMS Ambulance Board has contracted with
Lifestar Emergency Medical Services, LLC to provide ambulance services the Southern
Winnebago EMS Ambulance Service Area beginning January 1, 2026; and
Whereas, in the transition of services it may be necessary or expedient for the City of
Oshkosh Fire Department to continue to provide services for a short period of time within
the Southern Winnebago EMS Ambulance Service Area to facilitate the smooth transition of
services and to assure that there is no disruption in services to the residents of the Southern
Winnebago EMS Ambulance Service Area; and
Whereas, the City of Oshkosh is willing to provide continuing ambulance services
during the transition period pursuant to the terms of this Agreement.
Now, therefore the City of Oshkosh, the Southern Winnebago EMS Ambulance
Service Area and Lifestar Emergency Medical Services, LLC agree as follows:
1. The Southern Winnebago EMS Ambulance Service Area, Lifestar Emergency
Medical Services, LLC and the City of Oshkosh agree that the City of Oshkosh
shall continue to provide emergency ambulance service to the Southern

Winnebago EMS Ambulance Service Area according to the same terms and
conditions of the Agreement with the Town of Black Wolf, Town of Nekimi, Town
of Omro, City of Omro, Town of Poygan, Town of Rushford, Town of Utica, Town
of Winneconne and Village of Winneconne for a period not to exceed 7 calendar
days beginning at 12:01 a.m. on January 1, 2026.
2. The City of Oshkosh shall invoice Lifestar Emergency Medical Services, LLC for
the services provided on a per capita basis as follows:
Per capita rate for 2026 is $40.40 / 365 = 0.11 per day
0.11 x 19,354 = $2128.94 per day
Populations:
Total: 19,354
Lifestar Emergency Medical Services, LLC shall pay for services for each full day
from midnight to midnight; if the final day of service is less than 12 hours in
length, that day shall be billed for only a half day of service.

3. When services are no longer needed, Lifestar Emergency Medical Services, LLC
shall notify the Oshkosh Fire Department on duty Battalion Chief by telephone
(920) 379-3515
4. Upon completion of services, the City of Oshkosh shall invoice Lifestar
Emergency Medical Services, LLC for the services rendered and Lifestar
Emergency Medical Services, LLC shall pay the amount due within 30 days of the
date of invoice. Any unpaid or delinquent amounts shall accrue interest at the
rate customarily used by the City of Oshkosh.
