St~e¥, C.RC, LAKESIDE NEUROC&RE 27 Oct 04 To Whom It May Concern: RE: DKVID FISS DOB: 9/1/37 Please be informed that Mr. Fiss has been under our care while a patient on the physical rehabilitation unit. This unfortunate gentleman has severe peripheral vascular disease resulting in osteomyelitis which resulted then in amputation of his fight lower extremity below the knee. The patient also suffers from an underlying diabetic neuropathy. He is largely fimetiunal at this time from a wheelchair and it is felt medically necessary that his home should be ramped to facilitate mobility and community access.
