SITE REQUIREMENTS
• A full size copy of the approved plans, specifications, and this letter shall be on-site during construction and open to inspection
by authorized representatives of the Department, which may include local inspectors. If plan index sheets were submitted in
lieu of additional full plan sets, a copy of this approval letter and index sheet shall be attached to plans that correspond with the
copy on file with the Department. If these plans were submitted in an electronic form, the designer is responsible to download,
print, and bind the full-size set of plans along with our approval letter. A Department electronic stamp and signature shall be on
the plans which are used at the job site for construction.
The following conditions shall be met during construction or installation and prior to occupancy or use:

2. SPS 382.20(8). REVISIONS. All changes or modifications, which involve the provisions of chs. SPS 382 to 384, made to
plumbing plans and specifications, which have been granted approval under sub. (1), shall be submitted to the department or
agent municipality for examination. All changes and modifications shall be approved in writing by the department or agent
municipality prior to installation of the plumbing.
3. Electronic plans were submitted for this review. The plans included 84 pages, including an index.
REMINDER(S)
• SPS 382.20-1 Line 6. This exterior storm sewer system has not been reviewed for subsurface infiltration as defined in SPS
381.01(133s) for compliance to specific plumbing provisions as detailed in SPS 382.365. RE: Per plan, WDNR type A
Approved liner appendix D have been added. See details.
• SPS 382.36(6)(g)3. By design, ground surface ponding shall drain down within 24 hours after a storm event.
• SPS 382.20. This Storm water system approval does not include review for NR 151 requirements or for any local Storm water
management requirements. Be advised a DNR WPDES permit may be required for this Storm water discharge. NR 151 is under
the Department of Natural Resource's jurisdiction.
• SPS 382.21(2)(e) Private water mains and water services. Private water mains and water services shall be inspected before being
covered. The private water mains and water services shall be tested and proven watertight under water pressure not less than the
working pressure under which it is to be used. The water used for testing shall be obtained from a potable source of supply. Note:
Standard NFPA 24 for combination water services and combination private water mains may include more stringent requirements
for testing.
• SPS 382.40(8)(i). Flushing and disinfection of potable water supply systems. 1. a. Before a newly constructed water supply
system is to be put into use, the piping of the system shall be filled with water and allowed to stand for at least 24 hours per this
section. After 24 hours each water outlet shall be flushed beginning with the outlet closest to the building control valve and then
each successive outlet in the system. The flushing at each water outlet shall continue for at least one minute and until the water
appears clear at the outlet.
• SPS 382.40(8)(i)4. New or repaired combination water services or combination private water mains shall be flushed and
disinfected prior to use in accordance with NFPA 24.
• SPS 382.40(7). The size of the combination domestic/fire main has been approved for the domestic demand only. It is the
responsibility of the designer to ensure that the combination domestic/fire main size is adequate for the overall system demand in
accordance with the applicable NFPA guidelines. Review of NFPA 24 was not included with this approval. Standard NFPA 24
for combination water services and combination private water mains may include more stringent requirements for testing.
• SPS 382.20-2. This plumbing approval does not include the dedicated water main that serves only fire hydrants or a dedicated
private fire service. The local municipality or fire department may have jurisdiction regarding such water mains. New or
renovated fire service mains, if part of an automatic fire protection system requires plan review per SPS 361.30.
The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Code §§ SPS 381, 382,
and 384 only. The submittal has been CONDITIONALLY APPROVED.
The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements. You are
responsible for complying with state and federal laws concerning construction near or on wetlands, lakes, and streams.
No person may engage in or work at plumbing in the state unless licensed to do so by the Department per Wis. Stat. § 145.06.
All permits required by the state or the local municipality shall be obtained prior to commencement of
construction/installation/operation. Pursuant to Wis. Stat. § 145.05(1), a city of the first, second, or third class shall supervise the
installation, alteration, and repair of plumbing within its jurisdiction. Should the project fall under jurisdiction of the Department,
please contact the DSPS inspector copied at the bottom of this letter.
In granting this approval, the Division of Industry Services reserves the right to require changes or additions, should conditions arise
making them necessary for code compliance. As per Wis. Stat. § 101.12(2), nothing in this review shall relieve the designer of the
responsibility for designing a safe building, structure, or component. The Division does not take responsibility for the design or
construction of the reviewed items.
