SITE REQUIREMENTS
• Contact both the State Inspector and the local municipality PRIOR to the start of construction.
• A full size copy of the approved plans, specifications and this letter shall be on -site during construction and open to inspection
by authorized representatives of the Department, which may include local inspectors. If plan index sheets were submitted in
lieu of additional full plan sets, a copy of this approval letter and index sheet shall be attached to plans that correspond with the
copy on file with the Department. If these plans were submitted in an electronic form, the designer is responsible to
download, print, and bind the full size set of plans along with our approval letter. A Department electronic stamp and
signature shall be on the plans which are used at the job site for construction.
The following conditions shall be met during construction or installation and prior to occupancy or use:
KEY ITEM(S):
• IBC 907.3 - Provide automatic fire detectors connected to the fire alarm control panel which perform fire safety functions.
Automatic fire-extinguishing systems provided in a commercial building shall be connected to the building fire alarm system.
• IBC 907.5.2.3.1 - Provide visible alarm notification appliances in public and common use areas. This includes, but is not
limited to, corridors, public restrooms, dressing rooms, shared offices, classrooms, medical exam rooms. Visible notificatio n is
not required in private offices or mechanical rooms.
Provide as required for the Computer Lab (044) unless existing notification is present.

• IBC 907.6 - Provide a fire alarm system installed in accordance with NFPA 72 (2013).
• NFPA (10.19.1) - If speakers are used to produce audible fire alarm signals, the required trouble signal for 10.19.1.1
through 10.19.1.3 shall be in accordance with Section 10.15.
• NFPA (12.2.4.3) - Fire alarm system wiring and equipment, including all circuits controlled and powered by the fire alarm
system, shall be installed in accordance with the requirements of this Code and of NFPA70.
• NFPA (14.4.11.1) - Voice communication using prerecorded messages and manual voice announcements shall be verified
as being intelligible in accordance with the requirements of 18.4.10.
• NFPA (14.6.2.1) - Maintenance, inspection, and testing records shall be retained until the next year and for 1 year
thereafter and shall follow the requirements set forth by this section.
• NFPA (26.6.1.1) - The methods of communications between the protected premises and the supervising station shall
comply with the requirements in Section 26.6.
Ensure compliance with the requirements of the appropriate section in 26.6.3.
ALSO ADDRESS:
• IBC 901.6.2 - Fire Alarm systems shall be monitored by an approved supervising station.
REMINDERS:
• SPS 361.33 - Where plan approval is required by this code, one set of plans, inclusive of a copy of all specifications, bearing
the stamp of conditional approval and a copy of the specifications shall be kept at the building site. The plans and
specifications shall be open to inspection by the department or its authorized representative.
• IBC 907.7 - Acceptance tests and completion - Upon completion of the installation, the fire alarm system and all fire alarm
components shall be tested in accordance with NFPA 72. A record of completion in accordance with NFPA 72 verifying that
the system has been installed and tested in accordance with the approved plans and specifications shall be kept for the life of
the system and be available to an authority having jurisdiction.
The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes and Wisconsin Statutes.
The submittal has been CONDITIONALLY APPROVED. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsib le
for compliance with all code requirements. Only those object types listed above have been approved; other submittals such as plumbing and
those listed above under REQUIRED SUBMITTAL(S), may also be required.
All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installat ion/operation.
You are responsible for complying with state and federal laws concerning construction near or on wetlands, lakes, a nd streams.
This plan has not been reviewed for compliance with fire code requirements, including those for fire lanes and fire protectio n water supply,
so contact the local fire department for further information.
In granting this approval, the Division of Industry Services reserves the right to require changes or additions, should condi tions arise making
them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the desi gner of the responsibility for
designing a safe building, structure, or component. The Division does not take responsibility for the design or construction of the reviewed
items.
Per s. SPS 361.40(4), projects for buildings of over 50,000 cubic feet total volume shall have supervising professionals who file compliance
statements with this agency and the local code officials prior to occupancy of the project. Compliance statements shall be filed online
at https://esla.wi.gov/PortalCommunityLogin.
