Mission Statement: Cultivate communities that empower and support every family to build a solid
foundation for children in their first five years.
Defining the Mission statement: First 5 Fox Valley (F5FV) is an organization that is leveraging
collective impact work and strategic partnerships to cultivate communities that empowers every family
to build a solid foundation for children in their first five years.
Purpose: Support early childhood efforts in the state, region and local communities to strengthen
families and assure equitable access to culturally relevant resources that parents want and need.

PROPOSAL DETAILS
(Please limit to 700 words)
1. PROGRAM/PROJECT APPROACH – include the following details, as applicable:
a) Briefly describe the program/project you are requesting funds for.
First 5 Fox Valley (F5FV) launched Help Me Grow (HMG) in the Oshkosh area in 2019. HMG is
a national, evidence-based system model that builds on existing community resources to create
a comprehensive support system available to all families with young children living in that
community (attachment A). This project will expand the capacity of HMG in supporting families
with connection to local resources, increasing developmental screenings with ongoing family
follow-up through the HMG database, and providing developmental screening training for
childcare centers.
b) Describe the need for your program/project.
First 5 Fox Valley (F5FV) published the State of the Child Report showing children living in
Oshkosh historically have poorer outcomes than other areas of the region and state
(attachment B, pages 6-10).
c) Identify any other organizations in Oshkosh that address this need.
Other programs which support families provide resource connection, but what makes HMG
unique is the universal access and comprehensive, ongoing care coordination. HMG acts as a
safety net when children do not qualify for other programs.
d) Describe your level of collaboration with other agencies on this project.
F5FV builds the capacity of other programs by providing an Early Childhood Resource Directory
and access to free developmental screening tools and database. Collaborating programs
include: the Early Intervention Program, Family Services - Parent Connection, Catalpa Health,
Oshkosh Area School District, UW-Oshkosh Head Start, Winnebago County Health Department
and 7 childcare programs in Oshkosh.
e) Is this a new, existing, or changed program?
HMG currently has limited reach in Oshkosh and this funding would establish a more robust
presence and build capacity of existing programs.
f) Specifically, what will you use ARPA funds for?
Funds are for a short-term, high-impact outreach campaign for adding new partners to the HMG
network, resulting in increased developmental screenings and more families supported by
resource navigators.
g) Who will benefit and how?
HMG benefits families through a comprehensive system of resource connection, ongoing follow-
up and free developmental screenings. Programs benefit from increased collaboration and
access to free developmental screening tools with improved data collection and reporting. HMG
benefits the community by providing data which helps stakeholders understand the state of the
children in their community.

h) How will you prevent the duplication of benefits to end users?
HMG prevents duplication by supplementing the work of existing programs, acting as a safety
net to provide ongoing care coordination for families who are not receiving support elsewhere.
i) How many individuals/families will be served by this program/project?
HMG currently works with 629 families in Oshkosh. With additional funding, we anticipate a 50%
increase in the number of families supported.
j) How will these funds help you respond to, or recover from COVID-19?
These funds will support HMG in handling the influx of families who need connection to local
resources and ongoing follow-up as a result of the social isolation young children experienced
during and after the COVID-19 pandemic.
k) For existing programs: How many people were served during the last program
year? How many were from the City of Oshkosh?
In 2023, HMG served 4,102 families in the Fox Valley. 629 of those families were from Oshkosh.
l) If existing, describe measurable impact has the program achieved to date (with
examples)
In the Fox Valley region, HMG has added 5,727 children to the HMG database, provided 7,731
developmental screenings, and completed 1,358 referrals to community resources.
2. PROJECT OUTCOMES
a) If this is a continuing activity, describe a measurable outcome of your previous
year's work regardless of funding source.
411 families in Oshkosh received support through HMG in 2022, and 629 in 2023.
b) Describe two anticipated measurable outcomes for your proposed
project/program.
1. Increase the number of families and children in Oshkosh supported by HMG
2. Increase the number of childcare programs in Oshkosh doing developmental screenings
c) Describe (if applicable) how proposed use of funds may achieve sustainability
objectives as described in the City’s Sustainability Plan.
The HMG systems-level approach benefits the community by providing connection to resources,
encouraging community engagement, and promoting health and wellness for all families.
d) Describe how project and proposed use of funds will benefit underrepresented or
marginalized individuals/groups, and/or achieve objectives contained in the City’s
Diversity, Equity and Inclusion Plan.
HMG aligns perfectly with the City’s DEI Plan because its primary purpose is to remove barriers
for all families and ensure equitable access to community support.

3. DESCRIBE THE AGENCY'S AUDITING AND FISCAL CONTROLS
a) Briefly describe your agency's fiscal oversight / internal controls to minimize
opportunities for fraud, waste and mismanagement.
F5FV contracts with Non-Profit Bookkeeping Associates to support fiscal needs, Erickson and
Associates for tax and auditing services, and Amundsen Davis Attorneys at Law for legal
services.
b) How does your agency plan to segregate ARPA funds from other agency funds for
purposes of identification, tracking, reporting and audit?
F5FV has worked with Outagamie County ARPA funds and fully understands the expectations
around fiscal control. F5FV has a separate account to track ARPA expenses separately from
other funding sources.
4. CONTINGENCY PLAN
a) If your grant request is not fully funded, what adjustments are you prepared to
make?
Partial funding would affect HMG’s capacity to broaden its reach, and the number of children
served in Oshkosh would be scaled back.
5. PROJECT BUDGET
a) Briefly explain project revenues and expenses related to this proposal. This
should match with the Budget Overview sheet.
Current work to implement HMG in Oshkosh has been funded by Basic Needs Giving
Partnership and ThedaCare Community Health. For explanation of expenses, please see 5b.
b) Be specific about how ARPA dollars would be spent.
ARPA dollars would be used for additional staff time, outreach materials, licensing for the
Resource Directory, a one-time upgrade to the HMG database to support expansion, and the
cost of developmental screenings for childcare centers.
c) Provide details about how funds would be used by December 31, 2024.
The funding amount was determined by what was needed to complete specific project outcomes
by the end of 2024. F5FV will pursue other funding to continue expansion.
