The CONTRACT made and entered into by and between the City of Oshkosh and the Wisconsin
Department of Transportation and Strand Associates, Inc., dated February 28, 2b'007 is hereby amended
as set forth on the following six (6) pages.
This CONTRACT is amended to provide plans for: Bridge Rehabilitation and Approaches for Structure
P-70-0707 on Sawyer St. in the City of Oshkosh for Project ID 4994-01-05.

AMENDMENT NO. 3
A. SCOPE OF SERVICES
Senfces under this Amendment include:
(1) Road and Structure plans for P-70-0707 under project ID 4994-01-05.
(2)
Incorporate the City's plans and specifications for sanitary sewer and watermain
into the PS&E package for ID 4994-00-83.
B. PUBLIC INVOLVEMENT
Section VI.E. 1 .a is revised as follows:
(a)
The CONSULTANT shall conduct or assist the MUNICIPALITY in holding two
informational meetings for ID 4994-00-85 and ID 4994-00-99 combined, two
informational meetings for D 4994-00-83, and one informational meeting for D
4994-01-05 to acquaint the public with the concepts and probable impacts of
these PROJECTS.
C. STRUCTURE PLANS
(1)
A Structure Survey Report/Hydraulic Report, which includes a discussion of
structure sizing, shall be prepared by the CONSULTANT in accordance with the
procedures set forth in the MANUAL. The completed preliminary plans shall
show the structure plan, elevation, and typical cross section, and ali pertinent data
shall appear on the first sheet(s) of the completed structure plans. Four prints of
this plan and the Structure Survey Report/Hydraulic Report shall be submitted
to the MUNICIPALITY for review. The MUNICIPALITY will review this
preliminary plan and the Structure Survey Report/Hydraulic Report and return
one print to the CONSULTANT showing requested revisions, if any.
(2)
Plans and Structures shall be fully dimensioned besides showing controlling
elevations. The plans shall be prepared with such precision and detail to allow
for the convenient layout in the field with customary degree of accuracy, and to
allow for the production of an accurate estimate of quantities for ail pertinent
items of work to be performed in the PROJECT.
(3)
Plans and Structures shall include schedules for bar steel reinforcement. Such
schedules shall provide all of the necessary detail required for the fabrication of
the reinforcement without the necessity of making separate shop drawings for
that purpose.
(4)
When the plans for a structure have been completed the CONSULTANT shall
furnish the MUNICIPALITY with three sets of prints of such plans for review
and examination.

The CONSULTANT shall submit to the lVlUNICIPAL1TY three copies for review
and examination of all specifications for items of work in the Structure Plans
which are not covered by the STANDARD SPECIFICATIONS and such
amendments to or revisions of the STANDARD SPECIFICATIONS as may be
required to properly cover the work contemplated by the plans.
(6)
Plans will be subject to review and examination by the MUNICIPALITY. Such
review and examination may be made on the site of the PROJECT.
(7)
Along with the plans for structures the CONSULTANT shall submit one copy
or reproduction of the design computations for the MJJ'NICIPALITY's review
and permanent file.
(8) The CONSULTANT shall famish such other pertinent information and data with
respect to the plans and design as the MUNICIPALITY may request.

Subsection VI.M. is amended to include the following:

services performed by CONSULTANT, a lump sum of $141,246. (No change)
(b)
For geotechnical investigation services sublet to STS Consultant Ltd., the
CONSULTANT's actual cost not to exceed $2,620. (No change)
(c)
For archaeological and historical investigation services sublet to Archaeological
Consulting and Services, the CONSULTANT's actual cost not to exceed $1,500.
(No change).
(d)
For completing an Environmental Assessment (3-FONSI) including a noise
analysis, if authorized by the DEPARTMENT a lump sum of $3,909. (No
change)

The total compensation amount for I.D. 4994-00-85 shall be $149,275. (No
change)

For historical investigation services sublet to Archaeological Consulting and
Services, the CONSULTANT's actual cost not to exceed $1,500. (No change)
(d)
For hazardous material Phase 2 - environmental sampling for up to three sites
performed by the CONSULTANT, if authorized by the DEPARTMENT, a lump
sum of $3,305 per site not to exceed a combined lump sum of $9,914 for three
sites. (No change)
(e)
For hazardous material Phase 2 lab work for up to three sites, sublet to US Oil,
if authorized by the DEPARTMENT, the CONSULTANT's actual cost up to
$450 per site not to exceed a combined amount of $1,350 for three sites. (No
change)
