If plan index sheets were submitted in lieu of additional full plan sets, a copy of this approval letter and index sheet shall be attached to plans that correspond with the copy on file with the Department. If these plans were submitted in an electronic form, the designer is responsible to download, print, and bind the full size set of plans along with our approval letter. A Department electronic stamp and signature shall be on the plans which are used at the job site for construction. The following conditions shall be met during construction or installation and prior to occupancy or use: KEY ITEM(S): • IBC 907.6 - Provide a fire alarm system installed in accordance with NFPA 72. • NFPA 72 (2013) 18.5.5.4.2 - Provide synchronization of appliances if more than two visible notification appliances are viewed at the same time. Ensure notification devices are synchronized. JASON KARNZ MARTIN SYSTEMS INC 2744 MANITOWOC RD GREEN BAY, WISCONSIN 54311 • NFPA 72 (2013) - 18.5.5.5.5 Visible notification appliances shall be located not more than 15 ft (4.57 m) from the end of the corridor with a separation not greater than 100 ft (30.5 m) between appliances. Ensure proper installation spacing. • NFPA 72 (2013) 17.4.4 Initiating devices shall be installed in a manner that provides accessibility for periodic inspection, testing, and maintenance. Duct detectors shall be installed where they can be accessible for required inspection, testing, and maintenance. • NFPA 72 (2013) 17.4.7 Where smoke detectors are installed in concealed locations more than 10 ft above the finished floor or in arrangements where the detector’s alarm or supervisory indicator is not visible to responding personnel, the detectors shall be provided with remote alarm or supervisory indication in a location acceptable to the authority having jurisdiction. If duct detectors are installed over 10 feet above the finished floor, install a remote test station in an approved location. • NFPA 72 (2013) 21.2.4 - Relays must be located within 3 ft. of the controlled circuit or appliance and must be monitored for integrity. Ensure relays are located properly. • IBC 901.6.2 - Fire Alarm systems shall be monitored by an approved supervising station. • IBC 903.4 – All valves controlling the water supply for automatic sprinkler systems and water-flow switches on all sprinkler systems shall be electrically supervised. Alarm, supervisory and trouble signals shall be automatically transmitted to an approved supervising station per NFPA 72. Actuation of the sprinkler shall activate the alarm. Ensure proper installation. • IBC 907.4.1 - If not located in an area that is continuously occupied, an automatic smoke detector shall be provided at the fire alarm control unit(s), NAC power extenders, and supervising station transmitting equipment. A heat detector may be used where ambient conditions prohibit smoke detectors or sprinklers. Ensure proper installation. REMINDERS: • IBC 907.7 - Acceptance tests and completion - Upon completion of the installation, the fire alarm system and all fire alarm components shall be tested in accordance with NFPA 72. A record of completion in accordance with NFPA 72 verifying that the system has been installed and tested in accordance with the approved plans and specifications shall be kept for the life of the system and be available to an authority having jurisdiction. • SPS 361.31(2)(b) - A copy of this approval letter and index sheet shall be attached to plans that correspond with the copy on file with the Department. Changes to the approved plan must be submitted for review and approval. Failure to properly attach the approval and index page to the plans that match the copy on file with Department may result in enforcement action under ss. 101.02 or 443.13, Statutes. • SPS 361.33 - Where plan approval is required by this code, one set of plans, inclusive of a copy of all specifications, bearing the stamp of conditional approval and a copy of the specifications shall be kept at the building site. The plans and specifications shall be open to inspection by the department or its authorized representative. • Municipal Building Official, please forward a copy of this letter to the official in charge of conducting fire department inspections in your municipality. The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements. Only those object types listed above have been approved; other submittals such as plumbing and those listed above under REQUIRED SUBMITTAL(S), may also be required. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation/operation. You are responsible for complying with state and federal laws concerning construction near or on wetlands, lakes, and streams. This plan has not been reviewed for compliance with fire code requirements, including those for fire lanes and fire protection water supply, so contact the local fire department for further information. In granting this approval, the Division of Industry Services reserves the right to require changes or additions, should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. The Division does not take responsibility for the design or construction of the reviewed items. Per s. SPS 361.40(4), projects for buildings of over 50,000 cubic feet total volume shall have supervising professionals who file compliance statements with this agency and the local code officials prior to occupancy of the project.
