PURPOSE:
APPROVE AGREEMENT WITH STATE OF WISCONSIN,
DEPARTMENT OF HEALTH AND FAMILY SERVICES, DIVISION
OF PUBLIC HEALTH
INITIATED BY:
DEPARTMENT OF COMMUNITY DEVELOPMENT,
HEALTH DIVISION
BE IT RESOLVED by the Common Council of the City of Oshkosh that the
proper City officials are hereby authorized and directed to approve and accept a grant in
the amount of $63,203 from the State of Wisconsin, Department of Health and Family
Services, Division of Public Health.
BE IT FURTHER RESOLVED by the Common Council of the City of Oshkosh
that the proper City officials are hereby authorized and directed to execute any and all
documents and assurances required by the State of Wisconsin, Department of Health
and Family Services for purposes of same.
