This AMENDMENT ("Amendment") is to the Service Agreement ("Agreement") which was entered
into June 1, 2017, ("Effective Date") by and between the City of Oshkosh ("City") and Life Line
Billing Systems, Inc. d/b/a LifeQuest Services n/k/a Life Line Billing Systems, LLC., d/b/a LifeQuest
Services ("Agency") the Parties are entering into this Amendment to that Agreement for the sole
purpose of Extending the term of this Agreement to become effective on April 1, 2022.
The following shall be added to the original Agreement by this Amendment as if fully set out in the
original Agreement and shall take precedence over any conflicting provision of the Agreement or
previous Amendment(s). All other provisions of the original agreement remain unchanged.
ARTICLE III.TERM
The term of this Agreement shall be extended from April 1, 2022, until March 31, 2025.
