HomeMy WebLinkAbout03-438.docOCTOBER 28, 2003 03-438 RESOLUTION (CARRIED 7-0 LOST LAID OVER WITHDRAWN PURPOSE: APPROVE AMENDMENT NO. 4 - ENGINEERING SERVICES WITH DEPARTMENT OF TRANSPORTATION AND STRAND ASSOCIATES FOR SAWYER STREET INITIATED BY: DEPARTMENT OF PUBLIC WORKS BE IT RESOLVED by the Common Council of the City of Oshkosh that the attached Amendment No. 4 to the contract between the City of Oshkosh, Wisconsin Department of Transportation and Strand Associates, Inc. for Sawyer Street is hereby approved and the proper City officials are hereby authorized to execute and deliver the amendment in substantially the same form as attached hereto, any changes in the execution copy being deemed approved by their respective signatures, and said City officials are authorized and directed to take those steps necessary to implement the terms and conditions of the Amendment. BE IT FURTHER RESOLVED that money for this purpose is hereby appropriated from: Acct. City of Oshkosh (Village Lane - 9th Avenue) Local Street Winnebago County Project ID 4994-00-99 Oakwood Road Bridge and Approaches, City of Oshkosh (Sawyer Creek~ Local Street Winnebago County The CONTRACT made and entered into by and bet~veen the City of Oshkosh and the Wisconsin Department of Transportation and Strand Associates, Inc., dated February 28, 2000, is hereby amended as set forth on the following three (3) pages. SCOPE OF SERVICES Services under this Amendment include the following: (1) Traffic control and staging plans for project ID 4994-00-83. (2) Incorporating the 2003 Standard Specifications and updating the standard special provisions into the PS&E package for project ID 4994-00-83. (3) Incorporating the Factuality Development Manual changes regarding sheet borders for project ID 4994-00-83. B. PROSECUTION AND PROGRESS Section VI.L.5. is revised as follows: Final PS&E Date October 2003 G. BASIS OF PAYMENT Subsection VI.M. is amended to include the following: I.D. 4994-00-85 (a) For reports, agency coordination, public involvement, meetings, surveys, road plans, PS&E package, right-of-way plats, descriptions, and contaminated site services performed by CONSULTANT, a lump sum of $141,246. (No change) For geotechnical investigation services sublet to STS Consultant Ltd., the CONSULTANT's actual cost not to exceed $2,620. (No change) (o) For archaeological and historical investigation services sublet to Archaeological Consulting and Services, the CONSULTANT's actual cost not to exceed $1,500. (No change) For completing an Environmental Assessment (3-FONSI) including a noise analysis, if authorized by the DEPARTMENT, a lump sum of $3,909. (No change) (e) The total compensation amount for I.D. 4994-00-85 shall be $149,275. (No change) (e) For hazardous material Phase 2 lab work for up to three sites, sublet to US Oil, if authorized by the DEPARTMENT, the CONSULTANT's actual cost up to $450 per site not to exceed a combined amount of $1,350 for three sites. (No change) For hazardous material Phase 2 drilling for up to three sites, sublet to Groundwater Management Services, if authorized by the DEPARTMENT, the CONSULTANT's actual cost up to $828 per site not to exceed a combined amount of $2,485 for three sites.
