No. 541-1810-1799-00000 - Water Utility - Constr. in Progress CHANGE ORDER CITY OF OSHKOSH, WI CITY CONTRACT 02-01, RAW WATER INTAKE AND TRANSMISSION MAIN Order No. 1 Date: June 9, 2003 Agreement Date: May 28, 2002 NAME OF PROJECT: City Contract 02-01, Raw Water Intake and Transmission Main OWNER: City of Oshkosh CONTRACTOR: Lunda Construction Company The following changes are hereby made to the CONTRACT DOCUMENTS: Contractor is hereby authorized to access Lake Winnebago from the temporary storage and work area on and adjacent to Lake Shore Drive east of the Water Treatment Plant (WTP) site. This authorization is subject to the conditions stipulated in the attached letter dated June 26, 2002 from David C. Patek of the City of Oshkosh to Paul Gevers of Lunda Construction Company. Contractor's designated storage and work areas east of the WTP and in the vicinity of the proposed Divemion Vault in Menominee Park are also revised to the areas as shown on attached Figures COl-1 and CO1~2, respectively. The proposed location and approximate dimensions of the access pier are shown on the attached exhibits A and B to the June 27, 2002 letter from Lunda Construction Company to Kristy Rogers at the Wisconsin Department of Natural Resources (WDNR). All other conditions and requirements of the Contract Documents shall remain unchanged. Reason for change: During the preparation ofbidsl the Contractor had reached a verbal agreement with a property owner to lease a parcel of land for loading and unloading materials' along the river across from the Pioneer Resort. Following the award of Contract, the property owner refused to lease the land to the Contractor. The Contractor diligently pursued acquiring an alternative site, but sites that were technically suitable and within a reasonable distance from the project site were not available. Use of remote sites, would have resulted in a claim for additional costs from the Contractor. The Contractor requested permission from the City to construct the access pier described earlier and offered a credit therefor. APPROVED Comptroller City Attorney 6/24/2003 CO1 ~ 3 CHANGE ORDER CITY OF OSHKOSH, WI CITY CONTRACT 02-01, RAW WATER INTAKE AND TRANSMISSION MAIN Order No. 2 Date: June 9, 2003 Agreement Date: May 28, 2002 NAME OF PROJECT: City Contract 02-01, Raw Water Intake and Transmission Main OWNER: City of Oshkosh CONTRACTOR: Lunda Construction Company The following changes are hereby made to the CONTRACT DOCUMENTS: Contractor is hereby author/zed to provide gearbox operators for the t;vo 24-inch diameter gate valves to be installed on the existing raw water intake pipelines. Gearbox operators shall be suitable for buried service. All other conditions and requirements of the Contract Documents shall remain unchanged. Reason for change: Geared operators were requested in order to facilitate operation of the valves under high differential head conditions. The valve manufacturer did not normally furnish this type of opel:ator as standard equipment for a valve of this size. The Contractor's proposal of December 10, 2002 to furnish and install the operators was reviewed by the Engineer and the Owner, and found to be reasonable. The barrier shall be provided at the Contract unit price of $72 per lineal foot. The quantity of deep-water turbidity barrier provided during the 2002 construction season is independent of any future work during the 2003 construction season. This change order in no way implies that additional deep water turbidity barrier will be approved. All other conditions and requirements of the Contract Documents shall remain unchanged. Reason for chan~e: The Wisconsin Department of Natural Resources (WDNR) has been regularly monitoring construction on the lake for compliance with the Section 30 Permit for this project. Based on its assessment of the effectiveness of the construction methods and means being employed by the Contractor to control turbidity, the WDNR ordered that additional deep water turbidity barrier be placed around construction areas in the lake. CITY OF OSHKOSH, WI CITY CONTRACT 02-01, RAW WATER INTAKE AND TRANSMISSION MAIN Order No. 4 Date: June 9, 2003 Agreement Date: May 28, 2002 NAME OF PROJECT: City Contract 02-01, Raw Water Intake and Transmission Main OWNER: City of Oshkosh CONTRACTOR: Lunda Construction Company The following changes are hereby made to the CONTRACT DOCUMENTS: In accordance with discussions in the field between the Contractor, the Engineer and the Owner, the Contractor is hereby authorized to reconfigure the site piping in the vicinity of the Low-lift/High-lift Pumping Station to extend the existing north 24-inch diameter intake pipe to reconnect to the previously abandoned portion of the 24-inch intake. This would be done in lieu of removing and capping the two existing 24-inch and two existing 14-inch diameter pipes at the Pumping Station, as provided for in the Contract Documents. In accordance with discussions in the field between the Contractor, the Engineer and the Owner, the Contractor is hereby authorized to reconfigure the site piping in the vicinity of the Low-lift/High-lift Pumping Station to extend the existing 14-inch diameter backflush line together with the 30-inch raw water transmission main so as to allow backflushing of the new 60-inch intake pipeline. This would be done in combination with item No. 1 above and in lieu of the 30-inch raw water transmission ma/n configuration as provided for in the Contract Documents. Contractor shall provide an ISCO, Inc flowmeter and transmitter in accordance with the verbal request in the field on October 1, 2002. This flowmetering system is to be provided in lieu of the Krohne flowmetering system specified in Section 15165 of the Specifications. Contractor shall use fused joints in lieu of quick-cormect couplings at ail locations in the three chemical system manholes along Merr/tt Street where said couplings are shown on the 3-inch diameter HDPE piping. All other conditions and requirements of the Contract Documents shall remain unchanged. 6/6/2003 CO4 - 1 The Contractor has proposed to make the above changes at no change in:fl~e Contract Price in accordance with the attached correspondence dated November 14, 2002 and January 10, 2003. Reason for changes: The Contractor proposed a modification to existing site piping that would provide the City with the capability to use both of the 24-inch diameter intakes, one of which was previously abandoned. The Contractor proposed to do this work concurrently with the work described under item 2 below, which together would result in no change in the Contract Price. The proposal was reviewed by the Engineer and the Owner, and found to be reasonable. The City requested a reconfiguration of the existing 30-inch diameter transmission main and 14-inch diameter intake backflush line to facilitate backflushing of the new 60-inch diameter transmission main. The Contractor proposed to do this work concurrently with the work described under item 1 above, which together would result in no change in the Contract Price. The Contractor's proposal was reviewed by the Engineer and Owner and found to be reasonable. The City requested a change in the brand of flowmeter to be provided for the chemical feed system. This request was based on the excellent performance of a replacement flowmeter and transmitter previously placed in service by the City. The Contractor proposed to make this change at no change in the Contract Price. This proposal was reviewed by the Engineer and the Owner and found to be reasonable. The Contractor proposed using fused joints in lieu of 3-inch quick disconnects for the HDPE piping in the three chemical system manholes along Men'ia Street. The Contractor proposed to do the work at no change in the Contract Price. This proposal was reviewed by the Engineer and the Owner and found to be reasonable. This change is accordance with ihe request in Field Memorandum No. 4, dated July 15, 2002, and the Contractor's proposal dated January 22, 2003. These documents are attached hereto. All other conditions and requirements of the Contract Documents shall remain unchanged. Reason for change: The City requested this modification to facilitate implementation of measureS to protect the water system from potential terrorist activities. Reason for chan~e: This modification was requested to limit potential inflow of stormwater drainage into the casing pipe for the chemical feed pipes along Merritt Avenue.
