PURPOSE:
GRANT CONDITIONAL USE PERMIT/1130 N. WESTFIELD
STREET
INITIATED BY: EVERGREEN RETIREMENT FACILITY
PLAN COMMISSION RECOMMENDATION: Found to be consistent with Section 30-11(D)
and approved
BE IT RESOLVED by the Common Council of the City of Oshkosh that a
conditional use permit is hereby granted under Section 30-11 of the Oshkosh Zoning
Ordinance for expansion of the existing retirement home/nursing home facility and
parking lot, per the attached, with the following conditions:
1)
The appropriate DNR permits be provided to the City prior to building permit
issuance.
2)
An additional 19 feet of right-of-way be dedicated along the east side of the site
with said dedication to be done prior to approval of occupancy of the building.
Dedication is to be done in coordination with the Department of Public Works.
Proposed Expansion of Evergreen Retirement Community
Evergreen Retirement Communitylnc. proposes to construct in 2003-04 on vacant land that is part
of our present campus a single story (without basement) 44-bed skilled nursing facility and a
wellness center attached to our current main building.
The new skilled nursing facility (SNF) will replace 44 beds in our existing building which are very
institutional in design and inefficient for staff. This will be phase II of a plan to provide a more
residential living environment for all residents of Evergreen requiring skilled nursing care. Phase I,
which was the replacement of 36 of our total 108 licensed SNF beds, opened in 1997. Phase II will
consist of four "households" of eleven (1 I) residents each, grouped around a neighborhood center
with a fireplace and library. Each household will have seven private and two semi-private rooms
around a living/dining/kitchen area which opens to a fenced courtyard shared by two households.
-While most food will be prepared in the Evergreen central kitchen, the household kitchens will be
used to provide light meals and snacks any time of day and for cooking activities. Two households
will share a spa providing a whiripool bath and beauty/barber shop. All of the equipment and
supplies required for the care of residents will be located close to the point of use in the household.
This significantly increases staff efficiency and their ability to be responsive to residents. A utility
room in each household includes a washer and dryer where residents' personal clothes can be
washed and returned to them the same day.
The Wellness Center will include two therapeutic swimming pools designed for ease of access by
elderly including those with significant physical limitations. The two pools are needed in order to
provide two different.water temperatures appropriate to different medical conditions and exercising
abilities. The changing area for the pools consists of eight private bathrooms with showers where
assistance can be provided by spouse, careglver, or therapist.
Also included in the wellness center is a room with a variety of exercise equipment designed for the
elderly, a group exercise room for a variety of classes, and an area for physical, occupational, and
speech therapy. At the entry to the wellness center is a small caf6 offering light meals and snacks to
residents and their families, visitors, volunteers, and staff.
Two new parking areas for visitors and staffprovide spaces for 40 cars including three for
· handicapped. The vacated space in the present building will be used for 20 additional private
assisted living accommodations and to increase the number of private SNF beds.
The landscaping of the site will include a buffer zone of native plants between the parking lots and
the adjacent Sawyer Creek to naturally filter nmoff from the parking lot. Several rain gardens
around the building utilizing native plants, will detain and naturally filter storm water from the site
before it enters Sawyer Creek.
The new building will use the same receiving dock and trash collection area as the present building.
Part of the building site is on the Sawyer Creek flood plain and will be filled to place the building
the required two feet above the 100-year flood level.
Approvals for the project have been received from the Wisconsin Department of Natural Resources,
the Wisconsin Department of Health and Family Services, and the Oshkosh Plumbing Inspector.
