Ms. Richey called the meeting to order at 8:00 a.m. B.) PRESENT: Sue Ashton; Julie Kiefer; Ron Duerkop; Judy Richey; Julie Davids; Deb Allison-Aasby; Julie Maslowski EXCUSED: Lindsay Temple; Heidi Basford-Kerkhof ABSENT: STAFF: Jean Wollerman, Senior Services Manager; Bobbie Luft, Activities Coordinator; Anne Schaefer, Marketing Fund Development Coordinator C.) CITIZEN COMMENTS ON AGENDA ITEMS: None D.) PUBLIC COMMENT ONAGENDA ITEMS: None E.) APPROVAL OF MINUTES: February 2, 2021 minutes approved as written: Mrs. Maslowski to approve the minutes as written; Mrs. Allison-Aasby seconded; ALL IN FAVOR, Motion carried (7) to accept the minutes as written. F.) PRESENTATION: a. Ellen Koski and Josh Nelson: Fox Valley Advance Care Planning Partnership: Advanced Care Planning (ACP)-see (Attachment A-ACP Partners) Ms. Koski explained the importance, of having your choices for medical care completed, be deemed incompetent to make choices. She stated the importance to name a person that you trust to make decisions in your best interest whether that person agrees with your decisions or not. Ms. Koski stated Wisconsin is one of ten states that do not have the next of kin law. She explained if a person does not have an advanced care plan in place and something should happen, the next of kin has no say in decisions. Mrs. Maslowski questioned what happened if the ACP is not in place; Ms. Koski replied, the hospital would be forced to take guardianship until a judge determines a power of attorney. She stated it is a long and costly process. See (Attachment B-Community Classes Card, Attachment C-What is Advance Care Planning) Ms. Koski stated there are classes free of charge to offer help in completing the forms correctly. Mrs. Wollerman questioned if the meeting were all in person or all on-line; Ms. Koski explained that some classes are in person, virtual classes or private appointments. Those classes in person have people who can legally witness, virtual classes do not and can impose a problem to make sure witnesses are valid. She stated to reach out for help, call 920.997.8412 or www.FVACPP.org. Mr. Duerkop questioned the experience with attendance; Ms. Koski replied, many are referred to the agency. She stated the agency will make sure a copy gets to their doctor. She stated the saying goes…”Always too soon, until it’s too late.” Mrs. Ashton questioned if Fox Valley Care Planning uses the same form that is used throughout the state of Wisconsin; Ms. Koski replied, Wisconsin is one of the strictest states, the most common form used is the document from the Department of Health website. ThedaCare and Ascension have approved forms that are geared more around one’s wishes and personal documents. She explained all three of these are similar with the three same questions. Ms. Koski explained that the 5 Wishes is no longer used much, however it is used as a conversation piece. She stated a person can always add more information as an addendum or include with important papers. Mrs. Davids questioned if her attorney has documents should she still have them at her doctor’s office; Ms. Koski replied, yes the doctor should have them as well as your appointed POA, each agent should or others that would be first notified, should something happen to you. She urged people that go on vacation to bring a copy along, or use a wallet card and to update every 10 years. The last place to put a copy is a safety deposit box. b. Melissa Sell, ADRC Specialist, ADRC Outreach: Mrs. Sell explained some of the outreach the she had done to reach out to individuals regarding ACP; April 2nd, two years ago a presentation was done with the human service employees at the ADRC, she works with Lakeland and Iris programs and at the Oshkosh Seniors Center for community members. She stated at the Seniors Center there had been about 15 participants and of those 10 had completed the forms. She stated the ADRC uses the State Form. She stated she had met with the behavioral health team last week in hopes to have a workshop in the fall. Mr. Duerkop questioned if there is a policy routine for I & A (information and assistance) person; Mrs. Sells responded there is no set policy, they receive interested people through conversation and more with people in long term health care. She stated the ladies do a great job educating the public on the importance of this form. Mr. Duerkop explained how so many people didn’t take the step and had to go through the process of guardianship with a psych evaluation and court process. Ms. Koski stated this is a rough thing to put people in a crisis through and to go through the process could cost $4,000-$6,000 and many times the family goes through PTSD. Mrs. Sells shared a story about a young woman that had a stroke during child birth. She explained how you just never know the importance until it’s needed. G.) OLD BUSINESS: NONE H.) NEW BUSINESS: a. Mr. Duerkop explained that the Better Business Bureau stated people who receive the Covid-19 vaccine card should not share it on social media. Predators can steal your information, Great Britain already had a scam of people selling cards and it is a matter of time before it starts here. b. Mrs. Allison-Aasby asked to be added to the April’s agenda as that will be her last meeting. Ms. Richey stated there are two openings for alternates. I.) OTHER BUSINESS: NONE J.) ADJOURNMENT: Mrs. Allison-Aasby moved to adjourn at 8:52 am, Mrs. Davids seconded, ALL IN FAVOR, Motion carried to adjourn at 8:52 am.
