The above-reference plans have been stamped CONDITIONALLY APPROVED based upon review for
confoITllance to the current edition of the Wisconsin Administrative Building and Heating, Ventilating
and Air Conditioning Code, chapters COMM 61-65. These plans have NOT been reviewed for
confoITllance to the Plumbing Code (ch. COMM 16) and any COMM code not specifically mentioned.
This approval is not a Building PeITllit. Necessary city peITllits must be secured before commencing
work.
You are hereby advised that the owner, as defined in Chapter 101.01 (I) ofthe Wisconsin State Statutes,
is responsible for all code requirements not specifically cited herein. Code requirements are set forth in
Chapters 61 through 65 of the rules of the Department of Commerce.
The building will be inspected during construction and a final inspection will be made after completion
to insure compliance with city and state codes.
The architect, professional engineer, builder or owner shall keep at the building as evidence of approval,
one set of plans bearing the stamp of approval.
COMM 64.0004 This approval does not include heating and ventilating. Such plans are required to be
submitted and approved prior to installation of such equipment.
COMM 62.2902.1 Provide a source of drinking water.
COMM 62.2702.2.4 Provide emergency illumination per 1003.2.11.2.
COMM 62.1003.2.11.3 Emergency illumination shall meet the prescribed levels in this section. Plans
must be prepared documenting where lighting will be provided and the illumination levels such lighting
will provide along the exit access corridors.
COMM 62.2.11.2 Emergency illumination shall be provided outside the east entrance/exit doors.
COMM 62, Table 602 An easement addressing two items must be obtained from the east property
owner. The easement must allow for the construction ofthe entrance canopy on the east property
owner's property. This easement must also provide a 30' no build easement extending from the south,
east and north faces of said canopy. The no build easement need only extend to the public right of way
when applying this from the north face of the canopy if the distance to the ROW from the canopy is less
than 30 feet. This easement must then become a recorded document for both the east property and the
property at 318 N. Main St.
COMM 62.1100, ICC/ANSI Al17.1 The ramps at entrances/exits shall not have a slope steeper than
1 :20 unless level p1atfoITlls are provided between the doors and the head or base ofthe ramps.

designer) observing construction of projects within buildings with total areas 50,000 cubic feet or greater and bleachers
(Comm 50.1 O/Comm 61.50). Failure to submit this form may result in penalties as specified in Comm 50.26/Comm 61.23
and/or local ordinances. This form must be submitted prior to the plan approval expiration date or another submittal may
be required.
