AGREEMENT
CITY CLERICS OFFICE
THIS AGREEMENT, made on the 2� day of Tp rs1 , 2020 by and between`
the CITY OF OSHKOSH, hereinafter referred to as CITY, and ( c)€3 (t4-bat_AP vj
Oshkosh, Wisconsin, hereinafter referred to as EMPLOYEE.
WHEREAS, the United States Department of Health and Human Services, the
State of Wisconsin and the City of Oshkosh have declared a Public Health Emergency in
response to a novel strain of the coronavirus, named COVID-19; and
WHEREAS, the City of Oshkosh desires to provide temporary facilities for
isolation of its employees who may have come in contact with the virus during the course
of their employment; and
WHEREAS, City has entered into an Agreement with a Hotel that is willing to
provide accommodations and services in support of such isolation, and
WHEREAS, Employee has requested and wishes to voluntarily isolate at the Hotel
facility as an alternative to home isolation/quarantine; and
Upon potential exposure of an Employee to coronavirus during the course of their
employment, the Employee shall immediately
• Follow any departmental procedures to remove themselves from the potential
exposure keeping public safety in mind,
• Contact their direct supervisor or on duty supervisor to report the exposure
• Contact the County Health Department hotline at 920-232-3026 8:15-4:15 M-F, Sat-
Sun from 12pm-4:30pm or afterhours number at 920-232-3000 for further
instructions on potential quarantine. Leave a message if no answer and proceed
to the following step:
• Contact Safety & Risk Management Officer or designee at 920-379-3496 for any
additional questions.
The City is choosing to accommodate Employee's request for isolation. Employee
understands that the City is acting in an expeditious manner to accommodate Employee's
request and that the City is not completing an investigation of the circumstances
surrounding the alleged exposure; is not acknowledging that the alleged exposure is
work-related,and is not by entering into this Agreement accepting liability for the alleged
exposure.

City recognizes the stress to an employee related to this situation and desires to provide
this accommodation regardless of the eventual result of any investigation and the City
agrees that it will not later seek reimbursement from Employee for City costs related to
this Agreement (except as specifically enumerated below in relation to damages or
personal charges incurred by Employee during Employee's stay) regardless of the
outcome of an investigation into the potential exposure or whether Employee becomes
symptomatic.
ARTICLE I. HOTEL SERVICES
City shall through an Agreement with a hotel provide and pay for a room for Employee
for the period recommended for isolation/quarantine.
Hotel shall provide basic services during the period of isolation/quarantine including
picking up trash and linens from outside the hotel room and providing toiletries or
cleaning supplies as requested by Employee. The Hotel shall notify Employee of and the
Employee shall be responsible to comply with proper means for packaging trash and
linens during the period of isolation/quarantine.
ARTICLE II. CITY REPRESENTATIVE
The following individual shall serve as a contact liaison for the Employee during the
period of isolation/quarantine:
Paul Greeninger, Safety and Risk Management Officer—920-379-3496
If Paul Greeninger is not available this number will be forwarded to another City Contact.
ARTICLE V. EMPLOYEE REPONSIBILITIES
The Employee shall at least daily check-in with the Contact Liaison and notify the Liaison
of any changes in the Employee's health status or of any concerns related to isolation or
the hotel facility.
Employee shall follow current CDC and WDHS guidelines pertaining to
isolation/quarantine during the period Employee is housed within the Hotel facility
Employee shall be responsible for daily cleaning and upkeep of the hotel room including
packaging up rubbish and trash for pickup, removing bed linens and remaking beds and
light cleaning as necessary

Food, Beverage or Additional Services. The Hotel may provide food, beverages or
additional services such as pay per view movies, paid Wi-Fi or other services for a fee to
Employee and may directly invoice and seek payment from Employee for such services.
Employee shall be responsible for payment for any food,beverages or additional services
requested during the period of isolation/quarantine.
Employee shall cooperate with City and the Hotel so that Hotel staff shall have no direct
contact with Employee during the period of isolation/quarantine.
Damages. Employee shall be responsible for any damages caused by Employee. Hotel
will invoice City for such damages and the City will invoice Employee. Employee shall
pay any invoice for damages paid by the City within 30 days of the date of invoice.
ARTICLE VI. TERMINATION
This Agreement may be terminated at any time by the City of Oshkosh. Upon
termination,Employee shall be given a reasonable time to make alternative arrangements
if necessary.
ARTICLE VII. WHOLE AGREEMENT
This document and any specified attachments contain all terms and conditions of the
Agreement and any alteration thereto shall be invalid unless made in writing, signed by
both parties and incorporated as an amendment to this Agreement.
ARTICLE VIII. SEVERABILITY
If any term, covenant, condition or provision of this Agreement shall be invalid or
unenforceable, the remainder of this Agreement shall not be affected thereby and the
remainder of the Agreement shall remain valid and enforceable to the fullest extent
permitted by law
ARTICLE IX. CHOICE OF LAW AND VENUE
The laws of the State of Wisconsin shall govern the interpretation and construction of this
Agreement. Winnebago County shall be the venue for all disputes arising under this
Agreement.
