Amendment No 1- Professional Services Industries, Inc. 2019 Geotechinical Exploration
AMENDMENT NO. 1 The CITY OF OSHKOSH, hereinafter referred to as CITY, and PROFESSIONAL SERVICES INDUSTRIES, INC., 3009 Vandenbroek Road, Kaukauna, WI 54130, hereinafter referred to as the CONSULTANT, entered into an agreement for SUBSURFACE EXPLORATION AND GEOTECHNICAL ENGINEERING EVALUATION FOR 2019 CAPITAL IMPROVEMENT PROJECTS on December 12, 2019 Paragraph C in ARTICLE XII. PAYMENT included the provision that costs for additional services shall be negotiated and set forth in a written amendment to this AGREEMENT executed by both parties prior to proceeding with the work covered under the subject amendment. THEREFORE, the CITY and the CONSULTANT agree to insert the following paragraph after the first paragraph in ARTICLE III. SCOPE OF WORK. The CONSULTANT shall provide the services described in the CONSULTANT's Proposal dated October 31, 2019 The CITY and the CONSULTANT further agree to insert the following bullet point after the first bullet point in Paragraph A of ARTICLE XII. (PSI), an Intertek Company, this opportunity to propose our services. PSI is submitting this proposal to conduct a geotechnical exploration for the proposed West 28th Street Lift Station in Oshkosh, Wisconsin Presented below is a review of furnished project information, along with our proposed scope of services and fee information. PROJECT UNDERSTANDING Based on the information provided, it is understood that two soil borings are being requested along West 28th Avenue for roadway and utility construction, and that two soil borings are being requested adjacent to the existing lift station located along the south side of West 28th Avenue for the construction of a new lift station. Based on the information provided by Jacobs, who is the project engineer, it is understood that bedrock will likely be encountered at a depth of about 10 to 35 feet in the area of the existing lift station. It is understood that the proposed utility line will consist of a force main and be installed beneath the creek crossing along West 28th Avenue by directional drilling, which will have entry and exit bore pits. In addition, open trench/cut installation may be utilized from the proposed lift station to West 28th Avenue No design details for the proposed lift station, or for the proposed utility construction were available at the time of proposal preparation. SCOPE OF SERVICES Lift Station Per your request, PSI proposes to drill a total of two (2) soil test borings until auger refusal depth, which is understood to be at a depth of about 10 feet for OS-2 and about 35 feet for OS- 1 Rock coring has been requested within the two lift station borings. It is understood that approximately 30 feet of rock coring will be required for OS-2 and that 5 feet of rock coring will www.intertek.com/building tel PSI Proposal No. PO-0094-290069-R-2 October 31,2019 Page 2 be required for OS-1 in order to extend the borings to the planned termination depth of 40 feet. If bedrock is not encountered before a depth of 40 feet, PSI will contact Jacobs to determine if additional drilling is required. In total, 45 feet lineal feet of drilling and 35 feet of rock coring is proposed for the lift station borings. Upon completion, a piezometer will be installed at OS-1 It is understood that the borings will be located, and the surface elevations will be provided by the City of Oshkosh As requested, 24-hour delayed groundwater readings will be taken within the two lift station borings. Per your request, after completion of boring OS-1 for the proposed lift station PSI will contact Jacobs prior to setting the screen for the piezometer so Jacobs can confirm the depth that the screen shall be placed at. The proposed piezometer will consist of the installation of one (1) temporary 2-inch diameter Schedule 40 PVC piezometer The bottom of the piezometer will consist ten feet of 0.010 slot screen, sand filter pack and bentonite annular seal. The remaining portion of the piezometer will consist of Schedule 40 PVC riser It is understood that the surface of the piezometer will consist of a pro-top/stickup covers. Delayed groundwater level readings have been requested at approximately 48 hours, 1 week, 2 weeks and 4 weeks after installation. PSI has not been requested to abandon the piezometer following completion of its use and no cost for such is included herein. It is understood that a staff geologist will be required to be on-site during the performance of the borings and piezometer for the Lift Station project. Rock coring has been requested in order to extend the borings to the planned termination depth of 20 feet. It is understood that approximately 20 feet of rock coring will be required. If bedrock is encountered before the planned termination depth of 20 feet, PSI will contact the City of Oshkosh to confirm that rock coring shall be performed. In total, up to 20 lineal feet of drilling and 20 feet of rock coring is proposed for the for the roadway and utility construction along West 28th Avenue. Where a significant alteration to the scope of work may be warranted, prior authorization from the client will be obtained. It is anticipated that the site is not accessible with standard drilling equipment. Therefore, the soil borings will be performed with an all-terrain (ATV) mounted rig drill rig utilizing continuous flight hollow stem auger to advance the boreholes. Representative soil samples will be obtained at 2'/z-foot intervals. As an exception, soil samples will be continuously sampled from OS-1. The samples will be secured by employing split-barrel sampling procedures in general accordance with ASTM D-1586 Soil samples will be visually classified by a soils engineer in general accordance with the Unified Soil Classification System Borings greater than 10 feet will be backfilled with bentonite upon completion. However, once backfilled and surface patched, it is possible that the backfill within the borehole will settle when surface water or groundwater enters the hole, thus causing the surface patch to fail. PSI cannot be responsible for monitoring the condition of said boreholes; therefore, the condition of the boreholes should be monitored by the property owner once PSI has demobilized from the site. In addition, the soil cuttings generated from the soil borings will be disposed of at a location determined by the City of Oshkosh, which is understood to be a few blocks away from the project site. Representative soil samples obtained during the field exploration program will be returned to the laboratory for classification and a limited number of engineering properties tests. The nature and extent of this laboratory testing program will be dependent up on the subsurface conditions encountered during the field exploration program and may include testing selected samples to evaluate the soils' moisture content, plasticity, grain size, and relative strength characteristics. www.intertek.com/building PSI Proposal No. P0-0094-290069-R-2 October 31,2019 Page 4 IBC SCOPE EVALUATION Please note that the project site is located within a municipality that employs the International Building Code (IBC) 2015 edition. As part of this code, the design of structures must consider dynamic forces resulting from seismic events. These forces are dependent upon the magnitude of the earthquake event as well as the properties of the soils that underlie the site. Part of the IBC code procedure to evaluate seismic forces requires the evaluation of the Seismic Site Class, which categorizes the site based upon the characteristics of the subsurface profile within the upper 100 feet of the ground surface. To define the Seismic Site Class for this project, and in accordance with your requested level of assessment, PSI proposes to interpret the results of our soil test borings drilled within the project site and estimate appropriate soil properties below the base of the borings to a depth of 100 feet, as permitted by Section 1613.5.5 of the code. The estimated soil properties will be based upon data available in published geologic reports as well as PSI's experience with subsurface conditions in the general site area. This proposal is based on the Seismic Design Category for this site being determined by others. However, should the site have a Seismic Design Category C, D, E or F, then the code requires an assessment of slope stability, liquefaction potential, and surface rupture due to faulting or lateral spreading. For certain types of buildings and sites, an evaluation of seismic stresses on retaining and basement walls may also be required. Detailed evaluations of these factors are beyond the scope of this proposal. However, PSI's evaluation of conditions will provide a qualitative assessment of these issues and will provide recommendations for more detailed study if the risk of one or more of these hazards is considered moderate to high. REPORT At the conclusion of PSI's field and laboratory work, a geotechnical exploration report will be prepared The report will include the following: • Soil boring logs containing descriptions of the soil and groundwater conditions at the test boring locations and boring location diagram; • A summary of the laboratory test results, • Evaluation of soil and groundwater conditions, • Guidelines for subgrade preparation for foundations and floor slabs, and for the selection and placement of structural fill in these areas; • Shallow foundation design recommendations including allowable bearing pressures, • Recommendations for design and construction of slabs on grade including subgrade modulus values; • Equivalent fluid pressures for active and at rest earth pressures for below grade walls; • IBC 2015 seismic site classification www.intertek.com/building PSI Proposal No. PO-0094-290069-R-2 October 31,2019 V , Page 5 SPECIAL INSTRUCTIONS Communication of any available site development plans or previous geotechnical investigations to PSI is needed. Upon project start-up, PSI will contact your designated representative regarding this information about project scheduling. Some damage to ground surface may result from the drilling operations near the work areas and along ingress/egress pathways. PSI will attempt to minimize such damage, but no restoration other than backfilling the soil test borings and surface patching is included. PSI will contact Diggers Hotline for public utility clearance prior to the start of drilling activities. However, this service does not mark the locations of privately-owned utilities. PSI's proposal is based on private utility lines and other subsurface appurtenances being located in the field by others prior to our mobilization. The cost of private utility locating is not included in PSI's proposal. If PSI is asked to provide this service, an additional fee will apply FEES It is proposed that the fee for performance of the geotechnical scope of services be charged on a unit rate basis. For the base scope of services outlined above, the total fee for the geotechnical services will be $20,943.50 Boring, sampling, and testing requirements are a function of the subsurface conditions encountered. This fee is based on adequate bearing materials being encountered with the proposed boring depths. Should conditions be encountered which require deepening borings or additional investigation, PSI will notify you to discuss modifying the outlined scope of work. Additional work beyond the lump sum fee will not be performed without your prior authorization.
