Dear Mary,
This letter is a request to VOID the permit for the siding of 813 Vine Avenue. Per your
instruction, enclosed are the permit, receipt, and placard.
We will be reapplying for the siding of 819 Vine Avenue, which is the correct address, as soon
as BQ Electrical sends us the Electrical Verification Form to submit with the new request.
Please accept my apologies for the extra work from this mistake.

Send the request to VOID the permit for 813 Vine accompanied by the permit, receipt, and placard. You will need
to send an application for a permit for 819 Vine. Please note that BQ Electric took out and Electrical Permit to
reconnect fixtures at 813 Vine. Should BQ Electric wish to VOID their permit also, they would need to send in a
request accompanied by the permit and receipt. We would then require a new application and permit be issued.

Mary,
While reviewing and filing the paperwork from the four permits 1 picked up last week from you, I noticed
that one of them, Permit #106524, was listed for the wrong address, 813 Vine rather then 819 Vine. I am
unsure if I made the mistake or what happened. Perhaps I wrote the wrong address when I applied for the
permit.
What can be done to make the correction, as it is 819 Vine that we intend to side. We own 813 Vine and
sided it a few years ago.
