Please read the following information and sign at the bottom to acknowledge th t you have read

1. All habitable rooms must be provided with 8% light and 3.5%ventilatio . Bedrooms
located on the second floor or below ground level must be provided with egress windows �
in addition to the light and ventilation requirements. Basement window are also �
required to have window well areas in accordance with ILHR 21.03. ;;
�

2. Garage spaces shall be separated from the dwelling unit in accordance th ILHR 21.08. �
The door and frame assembly between the dwelling unit and an attached arage shall be �
labeled by an indenendent testin�a�nc,y as having a minimum fire-resis ive rating of 20 �
minutes. All drywall joints are required to be taped/sealed unless the jo' ts are fitted so �
that the gap is no more than a dimes thickness ( 1/20-inch)and backed b solid wood or �
drywall with the joints staggered. �
�

3. All penetrations in exterior walls must be sealed prior to the insulation in pection. This �
includes holes in the top and bottom plates, gaps around heating ducts, d holes in the �
return air spaces. �
4. Smoke detectors are required on every floor and within 6' of all bedroom . The smoke
�
detectors are required to be hardwired and interconnected. �

5. Hydromassage tubs are required to have an access panel to motor. T'he a cess panel must �
be accessible without removing the finish of the house. �

6. All overframing must be designed by the truss manufacturer, according t standards set �
by the truss plate institute, or structural calculations must be provided pri r to the rough �
building inspection. �

7. Truss plans must be submitted and approved prior to framing the roof. �

8. Truss plans including the truss layout must be provided on site at the rou building �
inspection. In addition to the truss plans, microlam calculations must be rovided on site '
at the rough building inspection for all microlams with point loads on the .
9. Calculations must be provided for all cantilevered floors with point loads. Floor trusses
and TJI's must be designed for the cantilevered areas. F

� 10. The property owner and applicant is responsible for compliance with IL 21.125
3 (Erosion Control). When installing access drives, a minimum of 2"-3" d ameter stone is
! required in addition to a minimum of 6" depth, 30'-50' length, and 14'-2 'width. Erosion
i
; control measures must be maintained until the lawn is established.
11. All storm sewer inlets must be protected with approved erosion control evices. Tracking
(i.e. sediment)must be removed from the street at the end of each work y. Repair any
siltation or erosion damage to adjoining surfaces and drainageways resul ing from land
development or disturbing activities.
12. The building placard must be posted on site at all times. Once the rough framing is up
the placard must be posted on the patio door or on a kitchen window. If e placard is not
present at any inspection, stop work orders will be posted. Additional pl cards can be
obta.ined through our department if the cards are misplaced. �
13. All mechanicals must be roughed in prior to requesting the rough build' inspection. If
on the inspection the mechanicals are not roughed in,the inspection will ail and a$100
re-inspection fee will be charged.
14. Attached is a copy of the final inspection checklist. At the final inspecti n a11 items on
this list must be completed to avoid the $100 re-inspection fee per inspe ion.
15. When requesting final inspections please note that a minimum of 2 days otice is needed
from the time the inspection is ready. Furniture is only allowed to be pla ed in the center
of the garage floor and a minimum of a 3'pathway must be provided aro d the walls of
the garage. At the final inspection no fizrniture or personal belongings ar allowed in the
house. Appliances such as the stove,refrigerator, washer, and dryer are ot to be set in
place due to the electrical inspector needing to look at the power serving e appliances.
A 6' stepladder must be on site at the final inspection to provide access t the attic.
16. The building placard must remain on the patio door or on a kitchen wind w until
occupancy is approved. Occupancy is not allowed prior to the placard h ving all of the �
final inspection approved stickers on the backside. If occupancy occurs rior to the �
inspections being approved daily citations will be issued with no further otice being �
given. �

17. All site grading must be done in accordance with the approved drainage lan on file in the �
public works office, 3rd floor.
